Overview Even if you're not familiar with the term "dysmenorrhea," if you're a woman, chances are you've experienced it at some time in your life. Dysmenorrhea is simply the medical term for menstrual cramps, that dull or throbbing pain in the lower abdomen many women experience just before and during their menstrual periods. For some women, the discomfort is merely annoying. For others, it can be severe enough to interfere with everyday activities for a few days every month. Dysmenorrhea can be primary or secondary. Primary dysmenorrhea involves no physical abnormality and usually begins within three years after you begin menstruating. Secondary dysmenorrhea involves an underlying physical cause, such as endometriosis or uterine fibroids.

Menstrual Cramps (Dysmenorrhea)
What are menstrual cramps?
Menstrual cramps are pain or discomfort in the lower abdomen just before or during a menstrual period. Dysmenorrhea is the medical term for menstrual cramps. Dysmenorrhea can be either primary or secondary. Primary dysmenorrhea usually starts 1 to 2 years after your first period, but it may start earlier. Secondary dysmenorrhea results from a specific disease or disorder.
How do they occur?
Cramps are related to hormonal changes during your menstrual period. They are caused by chemicals called prostaglandins. These chemicals cause the uterus to contract to pass menstrual fluid. Women who have painful periods have larger amounts of prostaglandins or are more sensitive to these chemicals. Secondary dysmenorrhea tends to be caused by the following:
  • endometriosis (tissue from the uterus growing outside the uterus) pelvic inflammatory disease (PID) (a bacterial infection that enters the uterus and may spread to the fallopian tubes, ovaries, and other tissues in the pelvic region; may be sexually transmitted)

What are menstrual cramps?
Menstrual cramps cause pain in your the lower abdomen during the first few days of your menstrual period. Sometimes the pain radiates to lower back or both thighs. Some girls also have nausea, vomiting, diarrhea, or dizziness along with cramps.
What is the cause?
More than 50% of girls and women have cramps during their menstrual periods. The cramps are caused by strong contractions (and sometimes spasms) of the muscles in the uterus as it expels menstrual blood. Menstrual periods usually are not painful during the first 1 to 2 years after a girl has started having periods. However, once ovulation (the release of an egg from the ovary) begins, the level of progesterone in the bloodstream increases and leads to stronger contractions and some cramps.

What are menstrual cramps?
Menstrual cramps are abdominal and pelvic area (belly) pains that are experienced by a woman around the time of her menstrual period. Menstrual cramps are not the same as the discomfort felt during premenstrual syndrome (PMS) although the symptoms of both disorders can sometimes be experienced as a continual process. Many women suffer from both PMS and menstrual cramps. Menstrual cramps can range from mild to quite severe. Mild menstrual cramps may be barely noticeable and of short duration - sometimes felt just as a sense of light heaviness in the belly. Severe menstrual cramps can be so painful that they interfere with a woman's regular activities for several days.
