A syndrome characterized by marked limitation of abduction of the eye, variable limitation of adduction and retraction of the globe, and narrowing of the palpebral fissure on attempted adduction. The condition is caused by aberrant innervation of the lateral rectus by fibers of the oculomotor nerve. There are three subtypes: type 1 (associated with loss of abduction), type 2 (associated with loss of adduction), and type 3 (loss of abduction and adduction). Two loci for Duane retraction syndrome have been located, one at chromosome 8q13 (DURS1) and another at chromosome 2q31(DURS2). It is usually caused by congenital hypoplasia of the abducens nerve or nucleus, but may rarely represent an acquired syndrome. Miller, M.D. Duane's retraction syndrome is a congenital condition that is characterized clinically by abnormalities of abduction, adduction, or both. In the most common form of this condition, there is mild limitation of adduction associated with narrowing of the lid fissure and retraction of the globe, and moderate to severe limitation of abduction with widening of the lid fissure. Although it was once thought that the condition was caused by primary fibrosis of the lateral rectus muscle, electromyographic and pathologic studies have shown that the condition is caused by anomalous innervation of the horizontal rectus muscles such that in most cases, there are no abducens motoneurons in the region normally occupied by the abducens nucleus, there is no abducens nerve, and the affected lateral rectus muscle is innervated by a branch of the oculomotor nerve. The areas of fibrosis within the lateral rectus muscle are areas that lack innervation.

Duane syndrome, also called Duane retraction syndrome (DRS), is a group of eye muscle disorders that cause abnormal eye movements. People with Duane syndrome have difficulty rotating one or both eyes outward (abduction) or inward (adduction). How do normal eye movements occur? Six muscles, which control the movement of the eye, are attached to the outside of the wall of the eye. In each eye, there are two muscles that move the eye horizontally. The lateral rectus muscle pulls the eye out towards the ear and the medial rectus muscle pulls the eye in towards the nose. There are four other muscles, which move the eye up or down and at an angle. Each eye muscle receives the command for movement from cranial nerves that exit the brain. What is the cause of Duane Syndrome? Duane’s syndrome is due to some miswiring of the eye muscles. The “mistake” probably happens around the 6 th week of pregnancy and is due to poor development of tiny parts of the brain stem that control the eye muscles.
