An estimated 12 million Americans have dry eyes. This condition is the inability to produce enough tears to keep the surface of the eyes lubricated and comfortable.
Tears are made by tissue in the lacrimal glands. The major lacrimal glands lie under the skin and extend into the eye socket. Tears reach the surface of the eye via small openings from the gland. Tears drain from the eye's surface by flowing into other tiny openings (lacrimal puncta), then into canals in the eyelids (lacrimal ducts), into a small pouch (lacrimal sac) and finally into the nose.
Normally, blinking helps the eyelids spread a tear film over the eye. If blinking is prevented, dry spots develop on the surface of the eye and vision decreases. The continuous production and drainage of tears is very important. Tears keep the eyes moist, help ocular wounds heal and protect against eye infections. Persons with dry eyes are at greater risk of developing eye infections. What causes dry eyes?

Q:
This is in response to your article in yesterday's health section regarding the daughter who had eye problems. I, too, have been told I have dry eyes and to use baby shampoo and eye drops. My eyes bother me a lot because of working on computers all day in an office environment. My doctor has told me that women are more prone to dry eyes as they get older because of hormones, etc. I use the baby shampoo after I initially remove my eye makeup. It removes a lot more even after using other products for my eyes first. I just thought you might be interested to hear that this is quite a common problem.
A: I'm glad you've had good results with baby shampoo, a gentle and inexpensive solution to your problem. I have not heard of a connection between dry eyes and hormonal changes, but dry eyes are more common as people age because the tear glands don't produce tears as well. Dry eyes are worsened by the lack of humidity in the winter here and heating systems without built-in humidifiers. Having green plants in the office helps increase the moisture in the air. Some medications like antihistamines or antidepressants can also cause dry eyes. Diseases, like rheumatoid arthritis or lupus, may be associated with Sjogren's syndrome, a condition involving dry eyes and dry mouth.

What is Dry Eye Syndrome?
Dry eye syndrome is a chronic lack of sufficient lubrication and moisture in the eye. It is one of the most common problems treated by eye physicians. Dry Eye and Blepharospasm
One of the reasons that essential (dystonic) blepharospasm frequently fails to be diagnosed is the fact that dry eye (which can itself cause blepharospasm) often precedes and/or occurs concomitantly with. The following article by J. Daniel Nelson, M.D. describes these two disorders and how to distinguish them. The term "Dry Eye" refers to a group of diseases which cause symptoms of grittiness or foreign body sensation, burning and dryness. These diseases can be divided into two general categories: those diseases in which tear secretion is inadequate or decreased and those in which tear secretion is adequate or normal. The term "keratoconjunctivitis sicca (KCS)" is used to describe those conditions in which tear secretion is decreased. The most common cause of KCS is Sjogren's syndrome, which is characterized by dry eyes, dry mouth and a systemic autoimmune disease. In secondary Sjogren's syndrome, there is a definable systemic disease such as rheumatoid arthritis, systemic lupus or scleroderma. In primary Sjogren's syndrome, there is no definable systemic disease. Other causes of keratoconjunctivitis sicca include graft vs. host disease seen following bone marrow transplantation, AIDS and lymphoma involving the lacrimal glands. The most common cause of dry eye with normal tear secretion is blepharitis (bleph - eyelid; itis - inflammation). Blepharitis can be associated with skin diseases such as seborrhea or rosacea or can occur by itself. In this condition there is either a lack of oily secretion or an abnormality of the oil which leads to inadequate coverage of the aqueous layer, or both. This results in increased evaporation of tears, as well as an increased ability of environmental pollutants to contaminate the tear film.

In the winter months, with lower humidity, dry eye problems increase for some people. There are various types of dry eye syndrome, and between 10 and 15 percent of people in the United States have dry eye problems, said Kenneth Goins, M.D., associate professor of ophthalmology and visual sciences in the University of Iowa Roy J. and Lucille A. Carver College of Medicine. "Many different factors can affect the tear film quality and amount, which leads to secondary changes in the eye and loss of vision," said Goins, who specializes in treating patients with corneal, external and refractive eye diseases at UI Hospitals and Clinics.     What is dry eye? The eye depends on the flow of tears to provide constant moisture and lubrication to maintain vision and comfort. Tears are a combination of water, for moisture; oils, for lubrication; mucus, for even spreading; and antibodies and special proteins, for resistance to infection. These components are secreted by special glands located around the eye. When there is an imbalance in this tear system, a person may experience dry eye. What are the symptoms? When tears do not adequately lubricate the eye, a person may experience pain, a gritty sensation, a feeling of a foreign body or sand in the eye, itching, redness and blurring of vision. Sometimes, a person with dry eye will have excess tears running down the cheeks, which may seem confusing. This happens when the eye isn't getting enough lubrication. The eye sends a distress signal through the nervous system for more lubrication. In response, the eye is flooded with emergency tears. However, these tears are mostly water and do not have the lubricating qualities or the rich composition of normal tears. They will wash debris away, but they will not coat the eye surface properly.     An Overview Hydroeyes Medication Other Treatment Options Dry eye syndrome is one of the most common problems treated by eye physicians. Over ten million Americans suffer from dry eyes. It is usually caused by a problem with the quality of the tear film that lubricates the eyes. Tears are comprised of three layers. The mucus layer coats the cornea, the eye's clear outer window, forming a foundation so the tear film can adhere to the eye. The middle aqueous layer provides moisture and supplies oxygen and other important nutrients to the cornea. This layer is made of 98 percent water along with small amounts of salt, proteins and other compounds. The outer lipid layer is an oily film that seals the tear film on the eye and helps to prevent evaporation. Tears are formed in several glands around the eye. The water layer is produced in the lacrimal gland, located under the upper eyelid. Several smaller glands in the lids make the oil and mucus layers. With each blink, the eyelids spread the tears over the eye. Excess tears flow into two tiny drainage ducts in the corner of the eye by the nose. These ducts lead to tiny canals that connect to the nasal passage. The connection between the tear ducts and the nasal passage is the reason that crying causes a runny nose. In addition to lubricating the eye, tears are also produced as a reflex response to outside stimulus such as an injury or emotion. However, reflex tears do little to soothe a dry eye, which is why someone with watery eyes may still complain of irritation.

    How to Care for Dry Eyes
    Are your eyes dry, itchy, red and burning? Over-the-counter artificial tears provide only temporary relief. With proper care and nutrition, your eyes will create natural tears as needed. Steps: Keep in mind that tears are made up of three layers: a lipid (fatty) layer, a middle aqueous (water) layer, and an inner layer of mucin, which spreads tears across the surface of the eyeball when you blink. Take omega-6 fatty acids (found in borage, black currant seed and evening primrose oil) and omega-3 fatty acids (found in fish oil) to restore the lipid layer of the eye. This keeps tears from evaporating as quickly. Supplement with 50mg of vitamin B6 to improve utilization of these fats in the body. Improve your mucin layer by taking vitamin A. Good sources of vitamin A include carrots, yams, sweet potatoes, cantaloupes and fish oil. Also recommended are preservative-free vitamin A drops. Drink plenty of water to improve the middle aqueous layer of tear film.

'Dry eyes' occur when there are not enough tears to keep your eyes lubricated, or the tears that are produced do not spread evenly across the eyes.
