Dry Eyes
Introduction The eye depends on the flow of tears to provide constant moisture and lubrication to maintain vision and comfort. Tears are a combination of water, for moisture; oils, for lubrication; mucus, for even spreading; and antibodies and special proteins, for resistance to infection. These components are secreted by special glands located around the eye. When there is an imbalance in this tear system, a person may experience dry eyes. When tears do not adequately lubricate the eye, a person may experience:
  • Pain
  • A gritty sensation
  • A feeling of a foreign body or sand in the eye
  • Itching
  • Redness and blurring of vision
Sometimes, a person with a dry eye will have excess tears running down the cheeks, which may seem confusing. This happens when the eye isn't getting enough lubrication. The eye sends a distress signal through the nervous system for more lubrication. In response, the eye is flooded with emergency tears. However, these tears are mostly water and do not have the lubricating qualities or the rich composition of normal tears. They will wash debris away, but they will not coat the eye surface properly. In addition, because these emergency tears tend to arrive too late, the eye needs to regenerate and treatment is necessary.

Who gets dry eyes? Dry eyes can affect anyone, but it becomes more common with increasing age. Dry eyes affects about 7 in 100 people in their 50s, and about 15 in 100 people in their 70s. Women are affected more often than men. Understanding normal tears Just above, and to the outer side, of each eye is a small gland called the lacrimal gland. This constantly makes a small amount of watery fluid which is the main part of the tears. When you blink the eyelid spreads the tears over the front of the eye to keep it moist. Tiny glands in the eyelids (meibomian glands) make a small amount of lipid (fatty) liquid which covers the outer layer of the tear film. Cells at the front of the eye and inner part of the eyelids also make a small amount of liquid which adds to the tear film. The tears then drain down small channels (canaliculi) on the inner side of the eye into a tear 'sac'. What is Dry Eye?
Tears serve to lubricate the eye and they are produced around the clock, but when insufficient moisture is produced stinging, burning, scratchiness and other symptoms are experienced and may be referred to as Dry Eye, Keratitis Sicca, Keratoconjunctivitis Sicca (KCS) or Xerophthalmia. When we blink, tears form a film which spreads over the eye, making the surface smooth and optically clear and enabling good vision. There are three layers in the thin film of tears: an oily layer, a watery layer and a layer of mucus, each with specific function. The outermost, oily layer is produced by small glands at the edge of the eyelid (meibomian glands) and the main purpose of this layer is to smooth the tear surface and reduce evaporation.

Dry eye is a condition characterized by loss of water from the tear film. As a result, the tear film becomes too concentrated, pulls water from the surface of the eye and makes it dry. Dry eye causes sandy-gritty irritation and redness or burning of the eyes that generally gets worse as the day goes on.
Evaporation develops from long-standing eyelid inflammation, blepharitis, thyroid disease, or from having large eyes. Blepharitis, a dandruff-like buildup on the eyelids can cause the tears not to work properly. It can be treated with eyelid scrubs and warm compresses. With large eyes or thyroid disease, evaporation is directly proportional to the increased surface area of the eye that is exposed to air.
Another change that occurs in dry eye is the loss of corneal glycogen, a source for corneal wound healing. It becomes harder for dry eyes to heal when corneal glycogen is depleted. TREATMENT / PROCEDURE

The tears your eyes produce are necessary for overall eye health and clear vision. Dry eye means that your eyes do not produce enough tears or that you produce tears which do not have the proper chemical composition. Often, dry eye is part of the natural aging process. It can also be caused by blinking or eyelid problems, medications like antihistamines, oral contraceptives and antidepressants, a dry climate, wind and dust, general health problems like arthritis or Sjogren's syndrome and chemical or thermal burns to your eyes. If you have dry eye, your symptoms may include irritated, scratchy, dry, uncomfortable or red eyes, a burning sensation or feeling of something foreign in your eyes and blurred vision. Excessive dry eyes may damage eye tissue, scar your cornea (the front covering of your eyes) and impair vision and make contact lens wear difficult.

Dry eye Dry eye is a condition in which the tear glands are unable to provide adequate moisture to the eye. Each eye has two tear glands, one is located above the eye within the orbit and another, smaller gland is associated with the third eyelid. While this can occasionally be caused by trauma, hypothyroidism, or a drug reaction, most cases are inherited or due to unknown causes. The condition usually affects both eyes, is chronic, and requires management for the duration of the dog's life to prevent discomfort and loss of vision associated with scarring of the surface of the eye. Dogs with this condition are also more susceptible to eye infections which can complicate treatment. Symptoms include redness, a thick discharge from the eyes and, with time, cloudiness and loss of vision. Surgery may be necessary in rare cases where the tear glands are unable to respond to medication. The surgical procedure involves moving a salivary gland duct from the mouth to the eye (parotid duct transposition) to provide a constant source of moisture to the eye. The procedure is very successful in keeping the eye moist but it does not entirely eliminate the need for continued medication. After surgery, the operated eye appears to "tear" while the dog is eating because of stimulation to the salivary gland and it may be necessary to clean and dry the face after mealtime.
