Presentation
A 65-year-old man presents to the emergency room with lower abdominal pain, pneumaturia and fever. On physical examination, his temperature is 39 degrees Celcius and his pulse is 95. He has moderate left lower quadrant and suprapubic tenderness but no guarding or rebound. His stool is guaiac positive on rectal exam. His white blood cell count (WBC) is 16 K/µl with 80% neutrophils and 5% bands.
Imaging Findings
Computed tomography
Supine and upright plain radiographs of the abdomen are unremarkable. Axial tomograms from a pelvic CT after oral contrast and without intravenous contrast show thickening of the wall of the sigmoid colon with inflammatory changes in the surrounding mesenteric fat ( arrow ) The inflammatory process abuts the superior and posterior aspects of the urinary bladder. Stool, oral contrast and gas are visible within the bladder ( arrow ). There is also a small amount of extraluminal air ( arrow ). Numerous sigmoid diverticula are present.

Symptoms Prevention What Is Diverticulitis?
Diverticulitis is inflammation or infection of small pouches, called diverticula, that develop along the walls of your intestines. The formation of the pouches themselves is a relatively benign condition known as diverticulosis. The pouches can develop anywhere on the digestive tract, but they most commonly form at the end of the descending and sigmoid colons, and they also frequently occur on the first section of the small intestine (although they rarely cause problems there). If you have diverticulosis, you may not even be aware of it because the diverticula are usually painless and cause few, if any, symptoms. You may experience some cramping on your left side that disappears when you pass gas or move your bowels. And because diverticula on rare occasions bleed, bright red blood may appear in your stool. The more serious disease, diverticulitis, may involve anything from a small abscess in one or more of the pouches to a massive infection or perforation of the bowel. The symptoms are similar to those of appendicitis, except the location of the pain may differ. Diverticulitis may be acute or chronic. The acute form can manifest itself with one or more severe attacks of infection and inflammation. In chronic diverticulitis, inflammation and infection may subside, but they may never clear up completely. The inflammation of diverticulitis can eventually result in a bowel obstruction, which is indicated by constipation, thin stools, diarrhea, abdominal distention, and abdominal pain. If the obstruction persists, abdominal pain and tenderness will increase, and you may experience nausea and vomiting.

To understand diverticulosis, it is helpful to know about the anatomy and function of the intestine. The small intestine is the long, thin segment of bowel that begins at the stomach and ends at the large intestine or colon. The colon starts in the right lower abdomen and forms a large question mark across the entire abdomen ending in the rectum. Just above the rectum is the sigmoid (S-shaped) part of the colon. Liquid stool enters the right colon and, as it moves through the colon, is dehydrated so that a formed stool eventually enters the rectum. The Sigmoid Colon
The sigmoid colon has the specialized job of contracting vigorously to maintain a high pressure. This action regulates the movement of stool into the rectum. And since the sigmoid is a high pressure part of the colon, it is here that most diverticuli occur. Diverticulosis
Diverticulosis means that there are pockets or projections extending out from the colon. These occur very gradually over a long period of time. They occur along the natural weak points in the bowel wall. The pockets develop because of the pressure exerted within by the contracting colon. As noted earlier, since the sigmoid colon has the highest pressure in the colon, it is here that most diverticuli occur. Because of these balloon-like projections, the sigmoid often becomes thickened and narrowed. When this happens, significant changes in the bowel function can occur, such as discomfort, diarrhea, and/or constipation.

Diverticular disease of the colon includes both diverticulosis and diverticulitis. Diverticulosis refers to a non-inflammed diverticula or out pouchings are present with or without symptoms. Diverticulitis is present when one or more diverticula become inflammed. This inflammation can lead to perforation of the diverticulum with pericolic infection or abscess formation, free perforation with peritonitis, fistula formation, or obstruction. The usual symptoms of diverticulitis include: fever, lower abdominal pain, and lower abdominal tenderness. Most patients with an acute episode of diverticulitis severe enough to require hospitalization can be treated with intravenous fluids, bowel rest, and broad spectrum antibiotics and analgesics. Patients who have more than one episode of acute diverticulitis requiring hospitalization should be considered for surgery. In this instance, a section of the bowel needs to be resected or cut away and joined back into continuity. At this time it is possible to do this both by open conventional surgery and also the laparoscopic method. Inpatient Facilities

What is diverticular disease?
Many people have small pouches in their colons that bulge outward through weak spots, like an inner tube that pokes through weak places in a tire. Each pouch is called a diverticulum. Pouches (plural) are called diverticula. The condition of having diverticula is called diverticulosis. About half of all Americans age 60 to 80, and almost everyone over age 80, have diverticulosis.
When these pouches become infected or inflamed, the condition is called diverticulitis. This happens in 10 to 25 percent of people with diverticulosis. Diverticulosis and diverticulitis are also called diverticular disease.

In case of a suspected diverticulitis colonoscopy is contraindicated for risk of perforation. Stenose = stenosis An acute diverticulitis has developed into an inflammatory stenosis. An erythematous mucosa with interstitial edema is responsible for an extreme mucosal vulnerability. An endoscopic passaage is not possible. After antibiotic treatment a diverticulosis can be diagnosed, the endoscopic passage is smooth at the time of this second endoscopy.

Most people with diverticulosis have no symptoms. When symptoms do occur, they are usually mild.
These symptoms are nonspecific. This means that they are symptoms of many different digestive disorders. They do not necessarily mean that you have diverticulosis. If you have these symptoms, you should see your health care provider. Diverticulitis is a more serious condition and causes symptoms in most people with the condition.
  • Pain in the abdomen, usually in the lower left side
    Bleeding: Bright red or maroon blood may appear in the toilet, on toilet paper, or in your stool. Bleeding can be severe and often stops by itself.
    Fever
    Nausea
    Vomiting
    Chills
    Constipation (less often, diarrhea)
Diverticulitis, if not treated promptly, can develop some very serious complications. A complication is suggested by any of the following symptoms:
