Pediatric EMU Adult EMU Neuroradiology Hemispherectomy Description Hemispherectomy involves the surgical removal or disconnection of one of the two halves of the brain. History Dr. Walter Dandy performed the first hemispherectomy in 1928 as a treatment for brain cancer. In the 1950s, Dr. H.G. McKenzie did the first hemispherectomy for epilepsy. Various techniques have been developed over the years, including the functional hemispherectomy developed by Dr. Theodore Rasmussen in the 1970s. All of these techniques include some modifications of the original anatomic technique and have been shown to have similar seizure-free rates.
Indications
The most common reason for hemispherectomy is medically intractable epilepsy localized to one side of the brain in the patient with weakness and peripheral visual loss. Diseases presenting with these symptoms include

Sturge-Weber-Dimitri disease without facial nevus

A patient with Sturge-Weber-Dimitri disease presented with intractable seizures and progressive intellectual deterioration. There was no facial nevus or focal neurologic abnormality. CT disclosed bilateral calcification in a parieto-occipital gyral pattern. Histopathology of the brain revealed extensive calcification of vessel wall in parieto- occipital cortices.

The term also includes hemidecortication or hemicorticectomy (removal of cortical gray matter with preservation of the ventricle and a layer of white matter) and functional hemispherectomy (removal of a large central block of tissue and disconnection of the anterior and posterior elements). Children who have medically intractable seizures arising diffusely from lesions in a single hemisphere are candidates for the procedure. We now describe the seizure status and functional outcome of 52 hemispherectomies, a series spanning the period 1975 to 1994. The age at operation was two years or less for 18 patients; thus, this is the largest series of infant surgeries yet reported.

    Bilateral neonatal Sturge-Weber-Dimitri disease: CT and MR findings

    Department of Radiology, University of Texas Medical School, Houston 77030. The CT and MR features of cortical calcification and meningeal angiomatosis are typical of Sturge-Weber-Dimitri disease but are unusual in children less than 1 year of age. This case describes a child presenting with both of these features bilaterally in the neonatal period and represents an unusual presentation of this disorder.

Nevus flammeus on the side of the face ipsilateral to angiomatosis sometimes extends to neck, chest, and back. Angiomatosis may occasionally involve the choroid plexus, thyroid, pituitary gland, lungs, gastrointestinal organs, pancreas, ovaries, and thymus. Correlation between the distribution of the nevus and the course of the trigeminal nerve is responsible for naming the syndrome "trigemino-encephalo-angiomatosis," but later findings found the relationship to be fortuitous. The syndrome frequently occurs in incomplete forms, presenting different combinations of symptoms.
