Clostridium difficile infection
What is Clostridium difficile?
Clostridium difficile (so called because when it was first discovered it was difficult to grow in the laboratory) is a cause of diarrhoea, which is usually acquired in hospital. Although in most cases it causes a relatively mild illness, occasionally and particularly in elderly patients, it may result in serious illness and even death. The bacterium produces two toxins which are responsible for the diarrhoea and which damage the cells lining the bowel. However, not all strains of C. difficile produce toxin; these strains are unlikely to cause disease and patients colonised by them remain healthy. In addition, the bacterium can form spores which enable it to survive in the environment outside the body and which protect the organism against heat and chemical disinfectants.
What does Clostridium difficile cause?
C. difficile infection is usually acquired in hospital , and almost all patients who develop C. difficile diarrhoea are taking, or have recently been given, antibiotic therapy. Diarrhoea is the most common symptom but abdominal pain and fever may also occur. In the majority of patients, the illness is mild and full recovery is usual, although elderly patients may become seriously ill with dehydration as a consequence of the diarrhoea. Occasionally patients may develop a severe form of the disease called 'pseudomembranous colitis' or 'antibiotic-associated colitis' which is characterised by significant damage to the large bowel. This may lead to a grossly dilated bowel possibly resulting in rupture or perforation. Unlike some other causes of diarrhoea, it is rare for C. difficile to spread to other parts of the body such as the blood stream.

Diarrhoea
... Typhoid Diarrhoea is frequent discharge of watery faeces from the intestines, sometimes containing blood and mucus. Diarrhoea due to infection may last a few days, or several weeks, as in persistent diarrhoea. Persistent diarrhoea may result in severe dehydration and shock. It is therefore necessary to replace the fluid lost by the body. Severe diarrhoea may be life threatening due to fluid loss in watery diarrhoea, particularly in infants and young children, the malnourished and people with impaired immunity. Diarrhoea is a concomitant of many infectious diseases, especially typhoid fever, bacillary or amoebic dysentery, and cholera Diarrhoea is a symptom of infection by a host of bacterial, viral and parasitic organisms most of which can be spread by contaminated water. It is a rare occurrence for most people who live in developed countries where sanitation is widely available, access to safe water is wide and personal and domestic hygiene is relatively good. Diarrhoea due to infection is widespread throughout the developing world. In Southeast Asia and Africa it is responsible for 8.5% and 7.7% of all deaths respectively. In 1998 it was estimated to have killed 2.2 million people, most of whom where under 5 years old (WHO, 2000). Treatment Description Diarrhoea is an abnormal increase in the frequency and liquidity of the stools. The amount of water in stool each day is generally not more than 200 ml for an adult. When it is above this amount, it is called diarrhoea. However, the definition of diarrhoea also depends on what is normal for each person. Diarrhoea is not itself a disease, but can be a symptom of several underlying diseases.
Diarrhoea and intestinal functioning
The colon, or large intestine, absorbs fluid from digested food particles that pass through it, forming semi-solid stools. If the intestinal lining becomes irritated, the intestines push stools through too quickly, fluid is not reabsorbed, and the stool produced is very watery.

Rice-based oral rehydration solution for treating diarrhoea (Cochrane Review)
Fontaine O, Gore SM, Pierce NF ABSTRACT What's new in this issue Search abstracts Browse alphabetical list of titles Browse by Review Group A substantive amendment to this systematic review was last made on 02 August 1998. Cochrane reviews are regularly checked and updated if necessary. Background: Oral rehydration therapy is used to treat dehydration caused by diarrhoea. However the rehydration solution does not reduce stool loss or length of illness. A solution able to do this may lessen the use of ineffective diarrhoea treatments as well as improve morbidity and mortality related to diarrhoea. Objectives: The objective of this review was to assess the effects of rice-based oral rehydration salts solution compared with glucose-based oral rehydration salts solution on reduction of stool output and duration of diarrhoea in patients with acute watery diarrhoea. Search strategy: We searched the Cochrane Infectious Diseases Group trials register, the Cochrane Controlled Trials Register, Medline, Embase, Lilacs and the reference lists of relevant articles. We also contacted researchers in the field.

Press Release: United Nations
As Tourist Season Nears Peak, UN Agency Issues Tips To Avoid Travellers' Diarrhoea New York, Jul 28 2005 2:00PM With travellers' diarrhoea affecting at least some 10 million people each year and the tourist season approaching its peak, the United Nations health agency today issued a paper outlining ways to avoid the scourge, including boiling water, disinfecting with chemicals such as chlorine and iodine and drinking bottled water from known sources. Waterborne disease is common in all parts of the world where there is poor hygiene and sanitation, and traveller's diarrhoea – whose principal causes are contaminated food and water – is estimated to affect 20 to 50 per cent of travellers, the World Health Organization (WHO) said in the new document – Preventing Travellers' Diarrhoea: How To Make Drinking Water Safe. It urges that before travelling, people check with their local disease prevention agency or physician for recommended vaccines and medication, seek more detailed advice from WHO's International Travel and Health site at http://www.who.int/ith/en/, and take along a medical kit containing water disinfectant agents such as iodine tablets or solution, chlorinating-flocculating products, and/or chlorine bleach tablets.

Diarrhoea incidence <5 per 1000
Definition The number of children under 5 years with diarrhoea per 1000 population under 5 years per year.
DEFINITION: The number of children with diarrhoea per 1000 children in the target population. Diarrhoea is formally defined as 3 or more watery stools in 24 hours, but any episode diagnosed and/or treated as diarrhoea after an interview with the adult accompanying the child should be counted.
USE: To determine the health status of children and identify potential environmental hazards (e.g. contamination of water sources).
COMMENTS: It is assumed that health personnel, through interviewing the person accompanying the child, confirm that the problem most likely is diarrhoea and not just a temporary running stomach due to e.g. intake of certain drinks/foodstuffs.
Diarrhoeal disease is one of the leading causes of infant mortality, and is closely related to both socio-economic situation and environmental health issues like access to clean water.

  • Tell your doctor if you have diarrhoea. They can investigate the cause, and prescribe some anti-diarrhoea medicines. Make sure you drink plenty of fluids to replace the water lost with the diarrhoea, but avoid alcohol, coffee and orange juice. Acidophilus or other bacteria found in live yoghurt can help to ease diarrhoea caused by antibiotics. Antibiotics can kill off the healthy bacteria normally found in the bowel, but the bacteria found in live yoghurt may replace them.
If your diarrhoea is caused by radiotherapy , changing your diet is unlikely to help, and it is important that you take the anti-diarrhoea medicines prescribed by your doctor. You may also have diarrhoea if you have had surgery for bowel cancer. If this happens you should discuss it with your doctor or specialist nurse before changing your diet.

BVD virus can also cause enteritis during acute or transient infection which is usually mild but occasionally severe enough to kill even adult cows. Transient BVD virus infection is also believed to cause significant suppression of disease resistance and may contribute to the pneumonia complex in calves. Infection immediately before or during the breeding season will reduce conception rates and cause early death of the embryo. Infection at any stage of pregnancy can result in abortion.

Note for Pet Owners:

If you have an animal with diarrhoea there are some important guidelines that you should follow:
  • If you haven't done so - Get your pet examined by your veterinarian.
  • Always give the medications that your veterinarian has prescribed at the correct dose and at the correct times.
  • Contact your veterinary practice if you are concerned that your animal is having a relapse, or if it appears to react abnormally following treatment.
  • Keep your animal on the strict diet that your veterinarian recommends and AVOID feeding high fat foods because fat intake can make the condition much worse. If your pet is a bin scrounger or likely to steal food - stop him/her . Many "snacks" are relatively high in fat - so cut them out !!

The definition of diarrhoea is as follows: Normal bowel movements are altered resulting in an increase in any one of the following:
  • Frequency of defaecation.
  • Fluid content in the stools causing a loose consistency. (NB The water content in normal faeces is 60-80%and 70-90% in loose stools).
  • Volume of faeces produced - this is a combination of water content and undigestible (e.g.
