Water-related diseases Diarrhoea Diarrhoea occurs world-wide and causes 4% of all deaths and 5% of health loss to disability. It is most commonly caused by gastrointestinal infections which kill around 2.2 million people globally each year, mostly children in developing countries. The use of water in hygiene is an important preventive measure but contaminated water is also an important cause of diarrhoea. Cholera and dysentery cause severe, sometimes life threatening forms of diarrhoea. The disease and how it affects people Diarrhoea is the passage of loose or liquid stools more frequently than is normal for the individual. It is primarily a symptom of gastrointestinal infection. Depending on the type of infection, the diarrhoea may be watery (for example in cholera) or passed with blood (in dysentery for example). Diarrhoea due to infection may last a few days, or several weeks, as in persistent diarrhoea. Severe diarrhoea may be life threatening due to fluid loss in watery diarrhoea, particularly in infants and young children, the malnourished and people with impaired immunity. The impact of repeated or persistent diarrhoea on nutrition and the effect of malnutrition on susceptibility to infectious diarrhoea can be linked in a vicious cycle amongst children, especially in developing countries.

DIARRHOEA Three or more liquid or semi-liquid bowel movements in a day may mean you have diarrhoea. You may also have vomiting and/or a high temperature. You could also have abdominal cramps and nausea. Diarrhoea will make you feel weak because important body fluids and salts are lost. Diarrhoea is caused by various viral infections (gastro-enteritis) or by eating food that has gone off (food poisoning). Nearly all cases should clear by themselves in two to five days, without having to take any medicine so long as these self-care recommendations are followed.

Travellers' Diarrhoea Travellers' diarrhoea
(TD) is a condition characterised by a marked increase in the frequency of unformed bowel movements and is commonly accompanied by abdominal cramps, urgency, nausea, bloating, vomiting, fever, and malaise. It occurs in up to a half of European travellers who spend two or more weeks in developing parts of the world and should therefore be taken seriously.
Episodes of TD usually begin abruptly, occur during travel or soon after returning home, and are generally self-limiting. The onset of TD is usually within the first week of travel, but can occur at any time during the visit and even after returning home.

Causes
This condition can be a symptom of injury, disease or foodborne illness and is usually accompanied by abdominal pain, and often nausea and vomiting. There are other conditions which involve some but not all of the symptoms of diarrhea, and so the formal medical definition of diarrhea involves defecation of more than 200 grams per day (though formal weighing of stools to determine a diagnosis is never actually carried out). Diarrhoea
Diarrhoea kills over 1 million children every year through dehydration and malnutrition. Children are more likely than adults to die from diarrhoea because they become dehydrated more quickly. About one in every 200 children who contract diarrhoea will die from it. Diarrhoea is caused by germs that are swallowed, especially germs from faeces. This happens most often where there is unsafe disposal of faeces, poor hygiene practices or a lack of clean drinking water, or when infants are not breastfed. Infants who are fed only breastmilk seldom get diarrhoea. If families and communities work together, with support from governments and non-governmental organizations (NGOs), they can do much to prevent the conditions that cause diarrhoea. The term is used to describe an increase in frequency of bowel motions, or increased stool liquidity, or sometimes a sense of faecal urgency. A textbook definition of diarrhoea is: "the passage of greater than 300 ml of liquid faeces in 24 hours". However, this is not a very useful definition, as stool quantity is hardly ever measured. Common causes As mentioned above, the causes of diarrhoea are myriad. To determine which causes are more likely, a doctor will need to know, amongst other things, a patient's normal bowel habit, and how long the diarrhoea has been present. The more common causes are listed below:

Many parents wonder if their child has diarrhoea because bowel movements in a newborn tend to occur frequently. Your baby's ordinary bowel movements will depend very much on whether she's being breast or formula fed. The bowel movements of breastfed babies are usually yellowish, on the soft or liquid side and may occur up to five times daily. Sometimes a breastfed newborn will have a bowel movement during or immediately after each breastfeeding session. This occurs because as her stomach fills up, the milk stimulates her entire intestinal tract, prompting a bowel movement. Within a month, most breastfed babies are down to one or two bowel movements a day. Diarrhoea is common amongst people with HIV. It can be caused by some anti-HIV drugs, medicines used to treat some infections, and in people with a low CD4 count diarrhoea may be caused by some opportunistic infections.
Diarrhoea can take the form of a semi-loose to completely liquid stool, and may result in having to go to the toilet more frequently and urgently. It is common for diarrhoea to be accompanied by stomach pains, bloating, nausea, vomiting, fever and loss of appetite. Diarrhoea as a drug related side-effect
Diarrhoea has been reported as a side-effect of all the protease inhibitors as well as ddI and abacavir in the NRTI class and with some antibiotics.
With some drugs, diarrhoea goes away after the first few weeks or months of treatment, however for some people it becomes a permanent feature of living with the drug.
The severity of the diarrhoea also varies. Severe diarrhoea, involving several trips to the toilet each day, large, uncontrollable liquid bowel movements, and feelings of weakness and dizziness as a result of the loss of fluids and electrolytes is experienced by about a quarter of people starting treatment with nelfinavir and a fifth of people starting saquinavir. Similar levels have been reported in people taking amprenavir, lopinavir, ritonavir and indinavir. Less serious problems may be experienced by many other people taking protease inhibitors.

Chronic diarrhoea was caused only by certain genotypes of the parasite, infecting about one third of all the patients with microsporidiosis in the study. Of note, despite screening for a wide variety of potential sources, the only risk factor that investigators could identify for contracting these more serious strains was eating watermelons.

These guidelines are written for adults with no history of allergies or other serious underlying medical conditions. In all other situations you should arrange for specific guidelines from a doctor before your journey. Used sensibly these guidelines may help to lessen the severity of diarrhoea while abroad.
