A 15-month-old girl with celiac disease presented with microcephaly and developmental delay. Her head growth resumed when she was off of gluten and returned when she started it again. Additionally she developed a loss of reflexes. All the symptoms resolved when she eliminated gluten.     Introduction: Parents look at well check-ups as an opportunity to be reassured that their child is doing well, to ask their many questions, and to feel good about their child. When problems come up during the exam: whether they be behavioral, feeding problems, or physical, the interaction becomes something it was not supposed to be: stressful. The most stressful thing that can be told parents is that their child is not meeting normal motor or mental milestones. On the other hand, most of the time, when you bring up a possible problem, most parents will have had some awareness of this situation. So if you don’t mention anything, you are considered negligent or at best unobservant. How you handle this situation is how you practice the ART of medicine. The tablet contained large amounts of lead and mercury, leading to significant lead burden.     The IDEA Amendments of 1997 made two changes related to the provision on "developmental delay" under prior law, as follows:
  • the age range for children covered by the term was expanded from "ages 3 through 5" to "ages 3 through 9;" and
    the decision on whether to use "developmental delay" was changed from being at the sole discretion of the State to being at the discretion of both the State and the LEA.
  • These changes were addressed in proposed and in Note 2 following that section in the NPRM. Public comments indicated the need for further clarification
    Public comments received on the NPRM raised numerous questions and concerns about
  • the application of the term "developmental delay" and the respective roles of the State and LEAs in implementing the provision;

    Professional Development ... Who Can Help? Developmental Delay Developmental delay is a disability category used in Pennsylvania for infants, toddlers and preschool age children. While all children grow and change at their own rate, some children may experience delays in their development that can be cause for concern. Young children with developmental delays typically have skills below their same age peers in the ability to think and learn, move, understand, talk and express needs, relate to others, eat, dress, take care of themselves, or solve problems. Developmental Delay and Preschool Age Children: PA Chapter 14 Regulations define developmental delay as follows: "a child who is less than the age of beginners and at least three years of age is considered to have a developmental delay when one of the following exists:
    • The child's score, on a developmental assessment device, on an assessment instrument which yields a score in months, indicates that the child is delayed by 25% of the child's chronological age in one or more developmental areas. The child is delayed in one or more of the developmental areas, as documented by test performance of 1.5 standard deviations below the mean on standardized tests."

    (A) At least one norm referenced, standardized test in each area of suspected delay,
    (B) At least one additional procedure to confirm the child's level of functioning in each area of suspected delay;
    (C) At least one 20-minute observation of the child;
    (D) Review of previous testing, medical data, and parent reports; and
    (E) Other evaluative information as necessary to determine eligibility.
    (3) For a child suspected of being eligible for early childhood special education, the child shall meet the following minimum criteria:
    (a) The child meets the criteria for one of the disability categories in OAR 581-015-0051; or
    (b) The child experiences a developmental delay of 1.5 standard deviation or more below the mean in two or more of the developmental areas listed in OAR 581-015-0946(3)(a)(A)-(E).
