Story Tools JAKARTA, Indonesia (CNN) Indonesia is battling an early outbreak of dengue fever with health officials reporting 189 deaths so far this year. The country has about 9,500 cases of the disease, which does not usually spread until later in the year. Fumigation has begun around the country to kill the mosquitoes that carry the disease, Dr. Umar Fahmi Achmadi, Director General for Contagious Disease Control and Environmental Sanitation, said. Achmadi has requested additional government funding to pay for the effort. Because of the early outbreak of the disease, Achmadi said tests are being run to determine if this is a new subtype.

Dengue Fever What is dengue fever? Dengue fever is a disease caused by a virus that is transmitted by mosquitos. It is an acute illness of sudden onset that usually follows a benign course with headache, fever, prostration, severe joint and muscle pain, swollen glands (lymphadenopathy) and rash . The presence (the "dengue triad") of fever, rash, and headache (and other pains) is particularly characteristic of dengue. Dengue (pronounced DENG-gay) is now reaching the peak of a roughly five-year cycle. It strikes people with low levels of immunity. An attack of dengue produces immunity for a year or more. Once this outbreak ebbs, more people will be resistant to the viral disease and the cycle will begin again.

Dengue virus (DENV) which has four related but distinct serotypes:1, 2, 3 and 4. Dengue fever has been recognised since the latter part of the 18th century as causing epidemics in tropical and subtropical parts throughout the world. Dengue in Australia was first recognised in Townsville/Charters Towers late in the 19th century and early in the 20th century, outbreaks occurred in an area from the coast of north Western Australia to the Northern Territory and down through higher rainfall areas of Queensland and NSW. At that time Ae. aegypti mosquitoes were widely distributed in northern Australia and they even occurred as far south as the Victorian border in eastern Australia and south of Perth in Western Australia. By the 1970's, Ae. aegypti was restricted to northern and inland Queensland bounded by Torres Strait, Mount Isa, Gladstone and Dirranbandi. Epidemic dengue returned to north Queensland in 1981-82. Other outbreaks occurred there in the 1990s, and in the early 2000's.

Clinical features
Dengue Fever (Break Bone Fever)
Classically, dengue fever presents as an acute febrile illness of sudden onset. It is extremely debilitating, with fever lasting three to five days with a typically saddle back temperature curve with an apparent partial recovery for 1-2 days before resumption of symptoms of myalgia, particularly backache, arthralgia, retro-orbital pain, anorexia, vomiting, diarrhoea gastrointestinal disturbance, rash and increased vascular permeability. There is a high subclinical rate with milder disease in children compared to adults and a low fatality rate. Recovery from infection with one serotype of the dengue virus results in homologous immunity but does not provide protection against infection with other serotypes.

October 6, 1999 Web posted at: 2:58 p.m. EST (1858 GMT) From Correspondent Charles Zewe NUEVO LAREDO, Mexico (CNN) The biggest outbreak of "dengue fever" to hit south Texas in almost 20 years has sickened more than a dozen people in the border town of Laredo. Epidemiologists there have confirmed 14 cases of the viral illness, which is spread by mosquitoes and nicknamed "breakbone fever." Over 100 cases have been identified just across the Mexican border and many more are suspected. Dengue is very painful, but rarely fatal.

Dengue and dengue haemorrhagic fever Dengue is a mosquito-borne infection which in recent years has become a major international public health concern. Dengue is found in tropical and sub-tropical regions around the world, predominantly in urban and semi-urban areas. Dengue haemorrhagic fever (DHF), a potentially lethal complication, was first recognized in the 1950s during the dengue epidemics in the Philippines and Thailand, but today DHF affects most Asian countries and has become a leading cause of hospitalisation and death among children in several of them. There are four distinct, but closely related, viruses that cause dengue. Recovery from infection by one provides lifelong immunity against that serotype but confers only partial and transient protection against subsequent infection by the other three. There is good evidence that sequential infection increases the risk of more serious disease resulting in DHF. Prevalence The global prevalence of dengue has grown dramatically in recent decades. The disease is now endemic in more than 100 countries in Africa, the Americas, the Eastern Mediterranean, South-east Asia and the Western Pacific. South-east Asia and the Western Pacific are most seriously affected. Before 1970 only nine countries had experienced DHF epidemics, a number that had increased more than four-fold by 1995.

Overview Symptoms Treatment Prevention Definition: Dengue hemorrhagic fever is a severe, potentially fatal infection which occurs when someone with immunity to one type of Dengue virus is infected by another, different type. Causes, incidence, and risk factors: Four different dengue viruses have been implicated in both Dengue fever and Dengue hemorrhagic fever. Dengue hemorrhagic fever occurs when the patient contracts a different dengue virus after previous infection(s) by another type. Prior immunity to a different Dengue virus type is important in the production of this severe disease.
Worldwide, over 100 million cases of dengue fever occur every year. A small percent develop into dengue hemorrhagic fever. Most cases in the United states are brought in from other countries. It is possible to pass the infection from a traveler in the United States to someone who has not traveled.
