This may be associated with atresia of the foramina of Magendie and/or Luschka. According to one theory the failure of these foramina to open during embryonic development leads to enlargement of the fourth ventricle, with secondary compression of the cerebellum. The latter phenomenon results in failure of normal development of the cerebellum, accounting for the frequent small size of the cerebellum, as demonstrated in this slide. The large cyst in this slide is actually an enlargement of the fourth ventricle and not separate from it. The third ventricle and lateral ventricles are markedly enlarged secondarily.

The foraminae, not having opened during fetal development cause a blockage of the flow of the CSF and so produce an abnormal enlargement of the fourth ventricle (fluid cavity between the brainstem and the cerebellum). This results in failure of parts of the cerebellum to develop correctly. A large cyst is then visible on scans of the back of the brain. These are severe abnormalities with hydrocephalus being present in 90 per cent of cases. Dandy-Walker causes 2-4 per cent of all hydrocephalus cases. There are often many associated brain abnormalities including agenesis of the corpus callosum in 17 per cent of cases, occipital encephalocoele (failure of fusion of the bones at the back of the head) in 7 per cent, spina bifida syringomyelia (cysts in the spinal cord), or small heads and bone abnormalities of the neck (Klippel-Feil syndrome). Other associated abnormalities include facial, ocular and cardiac.

About 4% of all cases of hydrocephalus are complicated by DWS. Associated disorders may include Aase-Smith syndrome 1, Aicardi, Coffin-Siris, cryptophthalmos, Ehlers-Danlos, Ellis-van Creveld, Jones syndrome, Joubert, Ruvalcaba-Myhre-Smith, Walker-Warburg, Meckel, Ritscher-Schinzel, and Veradi syndromes.
