Differential Diagnosis BACKGROUND AND CLINICAL INFORMATION Head Summary: The three essential features are: agenesis of the vermis, cystic dilatation of the fourth ventricle and enlargement of the posterior fossa. Hydrocephalus is a frequent but inconstant finding. Clinical features: The clinical manifestations of Dandy-Walker syndrome are mainly those of hydrocephalus. Therefore, the diagnosis is made in 75% of cases after 3 months of age and often only by the first birthday, as hydrocephalus usually becomes apparent by 1 year of age. Remarkably, there is no cerebellar sign or disturbances of stance. Mental retardation is present in 30-50% of patients. Prognosis: Prognosis is guarded and the mortality is about 27%. Other cerebral and visceral anomalies: it is the presene or absence of other cerebral and viseral abnormalies that determines the prognosis of individuals. About 68% of all cases have other developmental abnormalities of the CNS. Facial abnormalities may also be seen. Pathogenesis: agenesis of the vermis and cystic dilatation of the fourth ventricle may be the two primary events. Obstruction of the foramina of Magendie and Luschka may not be present in every cases. Other features may be secondary to the primary events.

Adult onset of the Dandy-Walker syndrome

In both patients there was normal motor and intellectual development during childhood, but as adults they had gradual evolution of brain stem and cerebellar signs and obstructive hydrocephalus. Aicardi et al . in 1965 described 8 cases with a triad of spasm in flexion, callosal agenesis and ocular abnormalities(1). This clinical syndrome now includes other features like vertebral anomalies, cortical heterotopias, hypotonia, mental subnormality, poor life expectancy, characteristic electrophysiological and radiological abnormalities(2). This disorder has been observed usually in females with the exception of two male infants(3). We report a typical case of Aicardi syndrome with Dandy walker malformation which is an unusual, association. Case Report A 7-month-old girl presented to us with infantile spasm since the age of two months. She was born by an uncomplicated normal vaginal delivery at term to a primigravida mother. The parents were non-consanguineous and there was no significant antenatal illness. The symptoms started initially with myoclonic jerks and occasional generalized tonic clonic convulsions. Her examination revealed marked hypotonia, micro-ophthalmia of right eye, microcephaly (

Dandy-Walker Malformation is a rare malformation of the brain that is present at birth (congenital). It is characterized by an abnormally enlarged space at the back of the brain (cystic 4th ventricle) that interferes with the normal flow of cerebrospinal fluid through the openings between the ventricle and other parts of the brain (foramina of Magendia and Luschka). Excessive amounts of fluid accumulate around the brain and cause abnormally high pressure within the skull, swelling of the head (congenital hydrocephalus), and neurological impairment. Motor delays and learning problems may also occur. Dandy-Walker Malformation is a form of "Obstructive" or "Internal Noncommunicating Hydrocephalus," meaning that the normal flow of cerebrospinal fluid is blocked resulting in the widening of the ventricles.

Hydrocephalus in Infants and Children
Hydrocephalus in infants and young children is frequently diagnosed at birth or shortly thereafter, but sometimes it is not diagnosed until the child is a little older.
With the advent of sophisticated imaging technologies such as magnetic resonance imaging (MRI) and ultrasonography, hydrocephalus can be diagnosed in utero, before the baby is born. Read more about prenatal hydrocephalus
Signs and Symptoms of Hydrocephalus
In an infant, the most obvious sign of hydrocephalus is an abnormal enlargement of the baby's head. The soft spot (fontanel) may be tense and bulging. The scalp may appear thin and glistening, and the scalp veins may appear to have unnatural fullness (prominence), as well. When you feel your baby's head along the suture lines, you may find that the bones are separated. Symptoms to watch for are vomiting, sleepiness, irritability and downward deviation of the baby's eyes (the sunsetting sign).
Toddlers whose sutures have not yet closed also show the signs of head enlargement. Older toddlers and children, once their sutures have closed, will show other symptoms of raised intracranial pressure (ICP) caused by their enlarged ventricles. Often these symptoms include headache, nausea, vomiting and sometimes blurred or double vision. The child might have problems with balance, delayed development in such areas as walking or talking, or poor coordination. As with infants, a child may be more irritable or tired than normal. The child may show a change in personality or be unable to concentrate or remember things, and their school performance may decline. Older children may have difficulty waking up and staying awake. While at times the symptoms are very noticeable, other times they can be very subtle and progress so slowly that only in retrospect are they appreciated.

The Dandy-Walker syndrome is hydrocephalus associated with a posterior fossa cyst and abnormal development of the cerebellum. What causes Dandy-Walker Syndrome? This malformation is thought to occur secondary to atresia (failure to open) of the foramina of Luschka and Magendie. This results in agenesis (failure to develop) of the cerebellar vermis with a large posterior fossa cyst communicating with an enlarged 4th ventricle. Hydrocephalus occurs in 90% of cases. How is Dandy-Walker Syndrome diagnosed? The clinical presentation of the Dandy-Walker syndrome depends to some extent on the combination of the developmental anomalies in the infant. If no other anomalies are present the sole manifestation may be increasing head size secondary to hydrocephalus. The normal signs of increased intracranial pressure may be absent in infants secondary to the ability of the head to increase in size. In older children, cerebellar dysfunction (mainly affecting balance), mental impairment, and double vision may be found. The definitive diagnosis depends upon radiological intervention. Ultrasound, MRI, and CT scan will show the malformation.

The Dandy-Walker syndrome is a malformation of the brain that involves the maldevelopment of the cerebellum (hindbrain), associated with a cystic enlargement of this area, and frequently hydrocephalus. This malformation occurs in approximately one in 25,000 babies. It accounts for approximately 1 - 4% of cases of hydrocephalus and is seen more frequently in females than in males. There are a spectrum of abnormalities in this area that resemble the Dandy-Walker malformations. This may help explain why some of the newer descriptions of this abnormality refer to posterior fossa cysts and cyst-like formations rather than the classic Dandy-Walker malformation. The normal structures in the back of the brain include the cerebellum which lies on top of the brain stem and encloses a space referred to as the fourth ventricle. This space connects the ventricles of the upper brain (the cerebrum) through a narrow canal called the aqueduct of Sylvius. Normally, cerebrospinal fluid (CSF) flows from the interior of the cerebrum through this canal through the fourth ventricle to bathe the outside of the brain and the spinal cord. In the Dandy-Walker malformation, the cerebellum is poorly developed, and a large cyst of CSF develops in this area. This cyst may lift up and displace the back portion of the brain as well as cause an internal obstruction of the normal flow of CSF with resultant hydrocephalus.
