I just don't want to miss anything! Should her head be hurting this much? Does she need another shunt? Do I wait the 6-8 weeks and watch her continue to have pain? quneuro 08-10-2004, 09:44 PM Keep in touch with the doctors. Unfortunately there can be pain and headaches after a shunt placement. My son, usually was a good couple of months before he felt like himself again. Do try to remember that her brain is still recovering, and while she might be ready to go, her brain may not. I found this especially apparent during the school year, and teachers felt my lad should be back in a week. It took him a month, and even then it was slow paced, but another month after that and he was fine.
Keep the journal of headaches, when they occur and how long they last, and keep in touch with the doctors. All Dandy Walker children are unique. Some have very little trouble while others have a great deal. As a good friend told me 16 years ago when we first heard about this, take a deep breath because you are doing a great job MUM. :) And remember it is always better to keep contact with the doctors, most do not mind.

    CONGENITAL ABNORMALITIES
    ARACHNOID CYST Arachnoid cysts are congenital lesions containing clear fluids similar to the cerebro-spinal fluid in composition. This should be differentiated these cysts from other types of cysts that result from CSF sequestration due to inflammatory or traumatic processes: these can be called secondary arachnoid cysts. Arachnoids cysts are a benign condition, which occurs throughout the cerebral spinal axis in relation to arachnoid membrane. The intra-cranial arachnoid cysts are invariably intra-dural while the spinal cyst may be either intra or extra dural. We will confine ourselves to congenital intracranial cysts. The incidence of these cysts is uncertain but recently we are seeing more and more cases with improved diagnostic means. The arachnoid cysts are classified according to their locations. SUPRASELLAR CYST These present with headaches, vomiting and hydrocephalus. Occasionally they present with overt endocrinological disorders. TEMPORAL FOSSA CYSTS Many of these cysts are asymptomatic but if they act as a space-occupying lesion. They present with headaches. The wall of the cyst is vascular and with minor trauma they might bleed sub-durally or into the cyst.

    As you know, Dandy Walker Syndrome is a congenital malformation of the brain, and it is often associated with other central nervous system problems.
    This condition requires treatment directed toward whatever anomalies typify it with a partcular child.     Dandy-Walker Syndrome
    The Dandy-Walker syndrome is a malformation of the brain that involves the maldevelopment of the cerebellum (hindbrain), associated with a cystic enlargement of this area, and frequently hydrocephalus. This malformation occurs in approximately one in 25,000 babies. It accounts for approximately 1 - 4% of cases of hydrocephalus and is seen more frequently in females than in males. There are a spectrum of abnormalities in this area that resemble the Dandy-Walker malformations. This may help explain why some of the newer descriptions of this abnormality refer to posterior fossa cysts and cyst-like formations rather than the classic Dandy-Walker malformation. The normal structures in the back of the brain include the cerebellum which lies on top of the brain stem and encloses a space referred to as the fourth ventricle. This space connects the ventricles of the upper brain (the cerebrum) through a narrow canal called the aqueduct of Sylvius. Normally, cerebrospinal fluid (CSF) flows from the interior of the cerebrum through this canal through the fourth ventricle to bathe the outside of the brain and the spinal cord. In the Dandy-Walker malformation, the cerebellum is poorly developed, and a large cyst of CSF develops in this area. This cyst may lift up and displace the back portion of the brain as well as cause an internal obstruction of the normal flow of CSF with resultant hydrocephalus. These patients may have other abnormalities in the development of the brain, including decreased connections from one side of the cerebrum to the other side. Developmental malformations outside the nervous system may occur in approximately 30% of these patients. These other malformations include cleft lip, cleft palate, cardiac malformation, orthopedic and respiratory problems, and urinary structural abnormalities.

    Dandy-Walker Syndrome
    Dandy-Walker syndrome is a congenital brain malformation involving the fourth ventricle and cerebellum. It is defined as an enlargement of the fourth ventricle, an absence (partial or complete) of the cerebellar vermis (the narrow middle area between the two cerebral hemispheres), and cyst formation in the posterior fossa (the internal base of the skull). Hydrocephalus (increased intracranial pressure) may also be present. Symptoms which often occur in early infancy include slow motor development and progressive macrocrania (an abnormally enlarged skull). In older children symptoms of increased intracranial pressure such as irritability, vomiting and convulsions, and/or signs of cerebellar dysfunction such as ataxia and nystagmus (jerky eyes) may occur. The syndrome can appear dramatically or be totally asymptomatic. Other symptoms include increased head circumference, bulging occiput (the back of the head), cranial nerve dysfunction, and abnormal breathing patterns.
