What is Cytomegalovirus infection?
Cytomegalovirus (CMV) is a common viral infection that rarely causes disease in healthy individuals. When it does cause disease, the symptoms vary depending on the patient's age and immune status. Who gets CMV infection? In the United States, approximately 1% of newborns are infected with CMV while growing in their mother's womb (congenital CMV infection). Many newborns however, will acquire CMV infection later by passage through an infected birth canal during the birth process or via infected breast milk (perinatal CMV infection). Children, especially those attending day-care centers, who have not previously been infected with CMV may become infected during the toddler or preschool years. Most people will have been infected with CMV by the time they reach puberty. How is CMV spread?

34 year-old man, infected with HIV, who presented with diarrhea. Sigmoidoscopy revealed mucosal inflammation with focal mucosal hemorrhage, edematous folds and polypoid lesions. Biopsies demonstrated viral inclusions consistent with cytomegalovirus.
70 year-old woman with AIDS, whose symptoms included diarrhea. At colonoscopy, a large, deep U-shaped ulcer was found in the cecum (left). A second ulcer was seen in the rectum at retroflexion (center). Biopsies were consistent with cytomegalovirus infection. Extensive ulceration in the poorly prepped colon of a 33 year old man with AIDS (right), who had presented with colonic pseudo-obstruction. Biopsies demonstrated CMV.

A common virus, it is estimated that up to 80% of Americans carry cytomegalovirus by the time they reach adulthood. Most experience no symptoms or mild symptoms such as enlarged lymph nodes, low fever, and fatigue that may or may not be noticed. Cytomegalovirus is present in body fluids (saliva, semen, cervical secretions, and urine) and can be spread from person to person by sexual contact, kissing, or the sharing of food. It can also be transmitted from mother to fetus. The virus usually remains dormant in the body, but it can reactivate and cause serious symptoms in immunologically suppressed patients, such as those with
