Definition: Congenital cytomegalovirus is a cluster of findings associated with infection of the fetus by cytomegalovirus.

Causes, incidence, and risk factors: Congenital cytomegalovirus is caused when an infected mother passes CMV virus to the fetus through the placenta. The mother's illness may be subclinical (without symptoms or asymptomatic ), and she may be unaware that any problems exist. The majority of congenitally infected children are asymptomatic. The symptomatic infant is characteristically born with a petechial rash (a rash that looks like fine purplish-colored dots), a large spleen and liver, jaundice , inflammation of the retina , intracranial calcifications (mineral deposits within the brain), and a small head (microcephaly).

Cytomegalovirus encephalitis and radiculomyelitis The recent decline in incidence of CMV encephalitis associated with highly active antiretroviral therapy has resulted in closure of the ACTG 305 study which planned to evaluate aggressive ganciclovir and foscarnet therapy for neurologic CMV diseases. These conditions should still be considered in the differential diagnosis of AIDS dementia in particular, and should be ruled out with PCR evaluation of CSF. If CSF is positive, current recommendations are for aggressive, combined CMV therapy with ganciclovir plus foscarnet. The place of cidofovir in treatment of these conditions is presently unknown. Our CMV Story
(not spina bifida related) My husband Patrick became very ill in the spring of 1996. At first his doctor had no idea why he was so tired, why he had muscle aches and a fever all the time . He felt terrible and the symptoms lasted for about 8 weeks. The first thought was that he had mononucleosis, but weeks later after blood tests showed an enlarged liver, he was tested for CMV. Pat rick , tested positive. We were told to have Andrew and myself tested. This got me intrigued, I wondered "how did Andrew get this, if not from me" and "Why didn't Andrew get sick like Pat rick did ?" This got me looking for information which le ad me to a blood bank site on the net. There I read that a newborn baby could get it from their mothers but that CMV could also be transmitted by a blood transfusion. Then the story started to unfold; because at 6 weeks old Andrew had a blood transfusion during his (2nd surgery) for decompression. He had to have gotten the CMV then. I looked into Andrew's medical files and I found out that at about 10 weeks old

What is cytomegalovirus?
Cytomegalovirus (say: "si-toe-meg-ah-low-vi-russ"), or CMV, is a virus that infects cells and causes them to become enlarged. People are usually infected by the time they are 2 years old or during their teenage years. Many people are infected with CMV and don't even know it. CMV usually does not cause long-term problems. However, CMV can cause problems in a newborn if the mother gets the infection during pregnancy. CMV is only spread through contact with an infected person's body fluids (such as saliva, blood, urine, semen or breast milk). It can be sexually transmitted. Careful handwashing with soap and water can help prevent the spread of CMV. Return to top

Usually, there are no symptoms. A few people will have symptoms that are similar to mononucleosis (such as a sore throat, fever, headache and tiredness). Cytomegalovirus (CMV) A virus that infects 50-85% of adults in the US by age 40 and is also the virus most frequently transmitted to a child before birth. Persons with symptoms have amononucleosis-like syndrome with prolonged fever and mild hepatitis. Once a person becomes infected, the virus remains alive and usually dormant within that person's body for life. Recurrent disease rarely occurs unless the person's immune system is suppressed due to therapeutic drugs or disease. CMV infection is therefore a concern because of the risk of infection to the unborn baby, people who work with children, and immunodeficient people such as transplant recipients and those with HIV. Cytomegalovirus (CMV) is a common virus found among people worldwide. Infection rates are higher and exposure to the virus occurs at an earlier age in developing countries, in lower socioeconomic groups in industrialized nations, and in Asian populations. In many underdeveloped countries, virtually all of the people have had CMV, while in the United States, only 40-50% of women in upper and middle socioeconomic groups have had the infection by the time they reach their child-bearing years. How does a person get CMV?
Cytomegalovirus is transmitted from one person to another through direct contact with bodily fluids. Urine may contain large amounts of the virus, which can also be found in saliva, tears, breast milk, semen and cervical secretions, and blood products. How do you know when a person has CMV?

What is cytomegalovirus (CMV)?
Cytomegalovirus (CMV) is a virus related to the herpes virus group of infections. Like herpes, it is inactive at times, but it is incurable and is a life-time infection. CMV may be passed from a mother to her baby during pregnancy and is the most common congenital viral infection. It is also called congenital cytomegalovirus or cytomegalovirus inclusion disease.
What causes cytomegalovirus (CMV)?
CMV without symptoms is common in babies and young children. It is found in saliva, urine, semen, and other body fluids. The virus is easily spread in households and in daycare centers. It can be transmitted to the fetus during pregnancy and to the baby during delivery or in breast milk.
Why is cytomegalovirus (CMV) a concern?
Over half of women of childbearing age become infected with CMV at least six months before becoming pregnant. There appear to be few risks for complications of CMV for this group and only a few babies have the infection at birth. These babies appear to have no significant illness or abnormalities.
According to the Centers for Disease Control and Prevention (CDC), about 1 to 3 percent of women first become infected with CMV during pregnancy. With a first infection during pregnancy, there is a higher risk that after birth the baby may have CMV-related complications. About 5 to 10 percent of babies with congenital CMV will have signs of the infection at birth. Of these, over 90 percent will have serious complications including hearing loss, visual impairment, mental retardation, or epilepsy. Premature babies may be at increased risk for these problems.

Cytomegalovirus (CMV) What is CMV?
CMV is short for cytomegalovirus. This virus commonly causes disease in the eyes or the intestines (gut) in people with HIV. CMV infection of the eyes is called CMV retinitis. It is the disease that most often causes blindness in people with AIDS. CMV infection of the colon is called CMV colitis. CMV can also attack your lungs, brain, liver and other organs. What are the signs of CMV?
People with CMV retinitis might have blurry vision, unusual changes in eyesight or see small-moving spots called floaters. They may also feel as if a shade is drawn over their eyes. People with CMV colitis might have diarrhea, abdominal pain, weight loss, fevers, loss of appetite, or trouble swallowing. In other parts of the body, CMV can cause painful neuropathy (tingling in the hands or feet), dementia or jaundice (yellow skin). If you have any of these signs, you should see your doctor right away. Early diagnosis is important.

    DESCRIPTION: Cytomegalovirus (CMV) is a beta herpesvirus. CMV may remain latent throughout a person's life and rarely reactivates. CMV cannot be propagated in laboratory animals or in most cell cultures.
    • The disease occurs world-wide and almost two thirds of the U.S. population is seropositive from prior exposure during childhood or early adulthood. In most of these, the disease is mild and overlooked. Populations at risk for active disease include neonates, transplant recipients, and AIDS patients.
    • CMV infection during pregnancy can be hazardous to the fetus and may lead to stillbirth, brain damage, birth defects or to severe neonatal illness
    • The name derives from the infected cells that are large and bear intranuclear inclusions
    • Infection may be in almost any organ, but the salivary glands are the most common site in children and the lungs in adults. This infection can occur congenitally, postnatally, or at any age.
    • Categories of CMV infections include:
      • Congenital: These infections vary greatly from mild cytomegaloviremia in a normal infant to the cause of abortion, stillbirth, postnatal death from hemorrhage, anemia, liver or CNS damage.
      • Acute infection in a normal host
      • Infections in bone marrow transplant patients - usually pneumonia
      • Infection in patients with AIDS - usually retinitis or colitis
      • Infections in other immunocompromised patients - pulmonary, gastrointestinal, or renal disease
