Cystocele is the name for a hernia-like disorder in women that occurs when the wall between the bladder and the vagina weakens, causing the bladder to drop or sag into the vagina. What are the Results of a Cystocele?
In addition to discomfort, the resulting dropped bladder can cause two kinds of problems to occur:
  • urine leakage incomplete emptying of the bladder
The dropped bladder stretches the opening into the urethra, and urine may leak when a woman does any action that causes pressure on the bladder, such as coughing.

Types of Prolapse
There are several specific types of prolapse depending on which muscles and organs are affected. A woman may have more than one of these conditions.
Cystocele
If the muscles in front of the vagina are weakened, the bladder will begin to bulge into the vagina. This is called a cystocele. The most common symptoms of cystocele are:
Rectocele and Enterocele
If the muscles behind the vagina are damaged, the rectum will begin pressing into the vagina. This is called a rectocele. If another part of the bowel presses on the vagina it is called an enterocele. The most common symptoms of these conditions are:
  • constipation and difficulty completing bowel movements painful sexual intercourse
Uterine and Vaginal Vault Prolapses
If the uterus and cervix begin to press down from above this is called a uterine prolapse. In women who have had a hysterectomy , a similar problem, called a vaginal vault prolapse may occur. In this situation, the upper part of the vagina is pressing on the lower portion. This may be due to muscle damage that occurred during surgery.

CASE: This article discusses a 34-year-old para 2 female with gestation and was originally diagnosed as a cystocele. The unusual timing of its occurrence and the characteristics of the anterior vaginal wall prompted ancillary testing and reexamination.

CYSTOCELE
CYSTOCELE AND/OR UTERINE PROLAPSE A cystocele or uterine prolapse is not life-threatening. It does not cause or become cancerous. Attempted correction of these situations is actually elective. If a woman notices these symptoms, she should see her physician for evaluation. Treatment may involve trying to keep the bowel contents soft so there is less bearing down at the time of a bowel movement. Certain exercises to strengthen the muscles of the area called Kegel exercises may be prescribed. Surgical correction is often available. A pessary is a device that is placed in the vagina to help hold up the bladder and uterus. Your doctor may discuss this with you and fit you with a pessary. When a pessary is in place, the patient should not be able to feel it. If you have further questions concerning cystocele and/or uterine prolapse, please contact our office to make an appointment.     Cystocele and pelvic floor tone
    by Peg Plumbo, CNM (see more from this expert) I am seven weeks postpartum and have a one to two degree cystocele and a small rectocele. I was told to wait six months before considering surgery. I was also told that Kegels will probably not remove the bulge. A midwife delivered my nine pound baby without performing an episiotomy. I believe that this would have prevented the cystocele. What do you think?

Alternate Names : Dropped Bladder, Fallen Bladder Pictures and Images Attribution Cystocele is a condition in which part of the bladder drops down, or protrudes, into the wall of the vagina. What is going on in the body? The bladder is the holding place for urine. It lies just above the vagina in a female. Between the bladder and vagina is a wall made of tissues and muscles that support the bladder and the urethra. The urethra is a tube that carries urine from the bladder to the outside of the body. When a woman has a cystocele, the wall supporting the bladder is weaker than usual. This weakening allows part of the bladder to protrude or drop down into the vagina. As the bladder droops into the vagina, the urethra becomes stretched, allowing urine to leak out of the urethra. What are the causes and risks of the disease?

The weakening of the muscular wall in this condition can cause urine to leak from the bladder when there is an increase in internal abdominal pressure such as in sneezing or coughing.     A patient information handout on using the pessary, written by the authors of this article, is provided on page 2729. The pessary is an effective tool in the management of a number of gynecologic problems. The pessary is most commonly used in the management of pelvic support defects such as cystocele and rectocele. Pessaries can also be used in the treatment of stress urinary incontinence. The wide variety of pessary styles may cause confusion for physicians during the initial selection of the pessary. However, an understanding of the different styles and their uses will enable physicians to make an appropriate choice. Complications can be minimized with simple vaginal hygiene and regular follow-up visits. (Am Fam Physician 2000;61:2719-26,2729.) T he pessary is one of the oldest medical devices available (Figure 1) . The type of pessary that is appropriate for each patient depends on the condition being treated. Although many physicians are unfamiliar with the pessary, it remains a useful device for the nonsurgical management of a number of gynecologic conditions
