Alternative Names: lymphangioma
Treatment: Treatment involves complete removal of the abnormal tissue whenever possible. However, cystic hygromas can often invade other neck structures, making this impossible. In these cases, other treatments have been attempted with only limited success. These include injection of sclerosing agents, steroids, chemotherapy agents, and radiation therapy.
Expectations (prognosis): The outlook is good if surgery can totally remove the abnormal tissue. In cases where complete removal is not possible because of involvement of neighboring nerves or blood vessels, recurrence is common.
Complications: Bleeding, infection, recurrence of the cystic hygroma, and damage to structures in the neck caused by surgery.
Calling your health care provider: Call your physician regarding any neck mass found in a child or adult.

Philip A. Matorin, M.D. The nature of lymphatic malformations (LM) has sparked great interest since they were first described by Redenbacher in 1828. Many of the early researchers believed that lymphatic malformations were neoplasms. Currently, most researchers agree that LM are not neoplastic and have adopted the term "lymphatic malformation" to emphasize this fact. Two major theories of the development of the lymphatic system have been proposed to explain the origin of LM. Sabin proposed that the lymphatic system develops from five primitive sacs which sprout from the venous system. In the head and neck, endothelial outbuddings from the jugular sac spread centrifugally to form the lymphatic system. McClure and Huntington proposed that the lymphatic system develops from mesenchymal clefts in the venous plexus reticulum and spread centripetally toward the jugular sac. LM develop from sequestration or congenital blockage of the primitive lymphatic anlage.

A 4-day-old male neonate weighing 2.8 kg presented with massive bilateral swelling of the neck, with involvement of the tongue and larynx resulting in airway obstruction (Fig. 1) . The tongue was so large that the child could not close his mouth. Emergency sub-total excision was done. Post-operatively, the child could not be extubated and required ventilatory support for 4 days. Eventually, he required tracheostomy, which needed to be kept for almost a month. The remaining part of the lymphangioma was excised at the age of six months. A cystic hygroma of the neck is soft, watery and easily transilluminated; there is no other lesion that is quite so characterstic. It consists of thin-walled cysts with flat endothelium. During surgery, it often seems that the mass has the consistency of water-filled soap bubbles and there is no end to the infiltration of normal tissue. In spite of the extensive invasion of tissue planes, these are benign lesions. About 75% of cystic hygromas are in the neck, 20% in the axilla

    CYSTIC HYGROMA OF CHEST WALL

    Sir,
    A two and a half year old female child presented to the pediatric department with a lump in the right chest wall (the mammary region) since birth. The swelling was slow growing. There was no overlying skin discoloration, ulceration or discharge. Transillumination test was positive. The child was in good health and showed normal developmental milestones. There was no evidence of any other associated anomaly. Fig 1: USG - a large cystic mass with septations within the subcutaneous tissue of the right chest wall. Fig 2: CT scan - a well defined lobulated cystic mass lesion abutting the pectoralis major muscle. No enhancement was seen on contrast CT. Ultrasonographic examination revealed a well marginated, lobulated, predominantly cystic mass with multiple septations of varying thickness (fig 1). On colour flow imaging, no vascularity was noted. The extent of the lesion was better delineated on CT. It appeared as a lobulated cystic mass of fluid attenuation within the subcutaneous tissue of the right chest wall abutting the right pectoralis major muscle (fig 2). A diagnosis of a lymphangiomatous malformation was considerd. Aspiration revealed chylous fluid and showed few endothelial cells and stromal elements suggestive of lymphatic malformation (cystic hygroma). Post sclerosant Ultrasonography revealed marked reduction in the size of the cystic component and increased echogenicity of the solid components within the lesion.

During Sonography a large multiseptate cystic hygroma was noted in fetal neck. Evidence of hydrops was seen around skull and fetal abdomen. Approximate sonographic gestational age by BPD and AC was 22 Wks. Fetal limbs were extremely short. Only four fingers were seen in both feet. Because of multiseptate cystic hygroma with evidence of hydrops and short limb dwarf diagnosis of Turners syndrome was considered. Patient opted for termination of pregnancy. Radiographs and photographs of fetus were taken. Radiographs of fetus showed the features of Achondrogenesis. Karyotype study confirmed the diagnosis of Turners syndrome. Most mediastinal cystic hygromas are extensions of cervical lesions, and cystic hygroma confined solely to the mediastinum is rarely encountered. The word is derived from the Greek meaning moist or watery tumor and can occur equally in boys and girls. Approximately 50 or 60 percent are present at birth and about 80-90 percent appear or are detected before the end of the second year of life. Embryology: Their cause is related to lymph channel development of the lymphatic system and may be due to poor communication with developing veins, which result in isolated sacs or little blind ends. These sacs slowly enlarge (filled with fluid) and may seem to infiltrate into surrounding tissues or push other tissues aside.

Characteristic large neck cystic hygroma present since birth in 3-month-old boy Cystic hygromas present generally as a multicystic mass with thin walls, and being fragile they may have areas of bleeding or accumulation of blood within them. They usually reside in close association with large veins and known lymphatic ducts in the neck, axillary area, groin area, or may infiltrate any areas of the skin along the trunk, extremities, or inside the chest or abdomen (Figure 1). They present as soft, discrete, and nontender swellings that are often compressible. Cysts within the mass vary from a very small size of a few millimeters to one of several centimeters. In some cases the hygroma may involve two or more areas by direct extension and seem to occur on the left side of the neck twice as often as the right, possibly related to the development of a large lymphatic structure in the chest which empties into the left internal jugular vein (Figure 2).
