General Discussion
Cystic hygromas are non-malignant malformations of the lymphatic system characterized by single or multiple fluid-filled lesions that occur at sites where the lymphatic system connects to the venous system, most commonly the back of the neck. Typically, this disorder becomes apparent in infancy or early childhood. Ninety percent of cases are symptomatic by the age of two years.

Lymphatic Malformation (Cystic Hygroma) Lymphatic malformations (cystic hygroma or lymphangioma) are usually apparent in young children. These lymphatic birthmarks appear in various forms, including as localized small lesions, or in the diffuse involvement of an extremity or particular body part or organ system. lymphatic malformations (cystic hygroma or lymphangioma) are usually classified as microcystic, macrocystic, or mixed. The medical term cystic hygroma, which was commonly used for macrocystic lymphatic malformatins located in the neck, is no longer used in modern medical terminology. Most lymphatic malformations (approximately 75%) occur in the cervicofacial region. The overlying skin can be healthy, or it may have tiny characteristic vesicles. Various lymphatic malformation lesions. 1st and 2nd lesions are commonly called "Cystic Hygroma" which is currently called macrocystic lymphatic malformation. Macrocystic lesions (cystic hygroma) typically easily compressible. 2nd lesion is a small lymphatic malformation (cystic hygroma) in the elbow. 3rd lesion involving the 2nd and 3rd toes. 2nd and 3rd lesions are not easily compressible due to microcystic component of the malformations (see the text).

  • MRI (see below): T2 weighted coronal images show a typical mixed form (macrocystic and microcystic) lymphatic malformation (low-flow vascular birthmark). These lesions are commonly called cystic hygroma. Please note that there is signal abnormalities in the lower section of the tongue. Tongue involvement is relatively common in lymphatic malformations of the head and neck. These low-flow malformations do not demonstrate contrast enhancement on post-contrast scans (not shown).

Fifty percent present in the first year and 90% present by two years of age. Usually the lesion is just cosmetically annoying but they can compress the airway and esophagus if they invade the mediastinum. The clinical course is characterized by intermittent or progressive growth, spontaneous regression, hemorrhage, and infection. Etiology/Pathophysiology:
The lesion spreads and engulfs adjacent tissues without invading them. Pathology:
Multilobular, multiloculated mass composed of cysts measuring 1 mm to 5 cm in diameter that contain serous fluid and are lined by lymphoendothelial tissue. Imaging Findings:
Seventy five percent are in the neck, 20% in the axilla, and 5% in the retroperitoneum. Two to three percent of the cervical and axillary masses extend into the mediastinum. DDX:
  • Hemangioma - often calcified, causes skeletal abnormalities, see cardiomegaly and increased pulmonary blood flow if there is AV shunting.

Newborn full term female with a neck mass. Clinical History:
This was a term female who had a prenatal diagnosis of a large right sided cystic neck and facial mass as seen on a prenatal ultrasound at 34 weeks gestation. The mother had an elective cesarean section in order to ensure proper management of the airway during delivery. During the cesarean section, the baby's head was delivered onto the mother's abdomen with the umbilical cord still attached. A laryngoscopy was performed. The airway was suctioned and an endotracheal tube was placed. The cord was then clamped. The baby was taken to an adjoining operating room and a direct laryngoscopy was performed which showed a sufficient airway. The baby was then extubated and did well on room air for 3 hours, but then required CPAP for adequate oxygenation. Image 1 Image 2 Image 3 Clinical Physical Exam:
Right sided facial swelling, protuberant tongue, swelling of the right eyelid and right side of the neck.

Lymphatic malformation
Lymphatic malformations are a type of vascular naevus or birthmark . There are malformed dilated lymphatic vessels in the skin because the embryonic lymph sacs have not connected correctly with the lymphatic system. The reason why this happens is unknown. No particular race or sex is more at risk. They are non-cancerous. Lymphatic malformations are always present at birth, although they may become more obvious with time. They may vary in size from a small dot to occasionally involving a whole limb. Although lymph is a clear fluid, lymphatic malformations often appear red or blue because of the involvement of adjacent blood vessels. There are three main types of lymphatic malformation:
  • Cystic hygroma Cavernous lymphangioma Lymphangioma circumscriptum
Cystic hygroma
Cystic hygroma is more common in some congenital disorders including Turner syndrome, hydrops fetalis, Down syndrome and fetal alcohol syndrome. So it may be advisable for babies with this type of lymphatic malformation to have chromosomal analysis performed.

A cystic hygroma is a thin-walled, sac-like structure filled with lymph. It occurs most commonly in the head and neck area and often appears as a soft bulge under the skin.

Causes
This condition is a congenital (present before birth) abnormality caused by embryonic lymphatic tissues. Cystic hygromas are abnormal growths arising from these tissues.
Symptoms
A common symptom is a neck mass noted at birth, or discovered later in an infant after an upper respiratory tract infection.
Exams and Tests
Treatment
Treatment involves complete removal of the abnormal tissue whenever possible. However, cystic hygromas can often invade other neck structures, making this impossible. In these cases, other treatments have been attempted with only limited success. These include injection of sclerosing agents, steroids, chemotherapy agents, and radiation therapy.
