Contact: Steve Rutledge (323) 669-4121 (Los Angeles) Richard G. Boles, M.D., co-division head of Medical Genetics at Childrens Hospital Los Angeles, and an assistant professor of Pediatrics at the Keck School of Medicine of the University of Southern California, has had a paper selected to be the lead article in the April 2003 issue of the American Journal of Medical Genetics. Dr. Boles’ research, which led to his paper, “Maternal inheritance in cyclic vomiting syndrome with neuromuscular disease,” found that defective cellular energy production is an important cause of that illness. Cyclic vomiting syndrome (CVS) refers to a condition involving multiple, severe, painful episodes of nausea and vomiting with the absence of these symptoms between episodes. CVS mostly affects children, is not rare, and results in much suffering, time away from school and hospitalizations for dehydration. These children usually have episodes every two weeks, to every two months, and episodes last usually between 12 hours and five days each. Until recently, the cause of CVS was unknown, except for its many similarities to migraine. Dr. Boles’ group has found out that defective energy metabolism is an important aspect leading toward the development of CVS, especially in those children who also suffer from other disease conditions such as weak muscles, developmental delay or seizures.

Fot those of you who read my "book" below regarding my 3 year old son, after yet another doctor visit this morning, we've just about decided he has Cyclic Vomiting Syndrome. He had a Nissen at age 3 months. He's not completely able to tell us where he hurts yet. He's been saying it's hard to swallow so we thought maybe he was having episodic esophogeal spasms. Last week, out of desperation, we tried Emetrol because he was saying he felt like throwing up. It worked! At least for a while. Then he would have problems again. As soon as we would give him another dose of Emetrol, he would be fine. He wasn't "sick". He was having the same symptoms he's been having for 3 years. These episodes are so traumatic for him. He breaks out with horrible ulcers after each one. I've taken him to 2 doctors about the ulcers. They both agree that they are caused by stress relating to these episodes! My question for those of you with a Nissen is this, "When you feel nauseated, is it difficult to swallow?"

Sandi, I'm so glad that you've identified what's going on and that the drug helps. I hope your little guy can get on to an even keel now. He has been through a lot, and it doesn't surprise me that he reacts by getting ulcers. You asked about nausea post-fundo. I'm 16 months post-fundo, and I have never felt nauseated. The patients with cyclic vomiting syndrome (CVS) have experienced a minimum of three severe distinct episodes of vomiting and nausea usually involving more than four emesis at the peak. They are entirely well between episodes. In the differential diagnosis of CVS, mitochondrial DNA (mtDNA) mutations should be considered. Boles et al. founded an 8.1 kb deletion in blood of a 2-year-old white girl who suffered from CVS. The mother's blood showed the same mtDNA deletion.
We recruited a family with four members suffering from CVS: a boy of 5 years old, the mother, the maternal grand-mother and aunt. All patients suffered of intermittent lactic acidosis. Laboratory, clinical and instrumental findings of all four patients excluded organic diseases.
To search for unknown mutations, several fragments of mtDNA were amplified by PCR, analyzed by SSCP (Single Strand Conformational Polymorphism ) and by sequencing. Several pairs of primers were used to amplify by CPR some regions of mtDNA where 8.1 kb deletion has been found. The mtDNA A3243G transition, which was for the first time described in a patient with MELAS was also analyzed as previously described.

Tools to Diagnose, Treat Cyclic Vomiting By Kelli M. Donley Cyclic vomiting syndrome (CVS) predominantly affects young children, with the median age of onset being 5.2 years. The symptoms are so strange and sporadic bouts frequently occur after months of good health that this rare illness often goes undiagnosed. However, a renewed dedication to researching the causes of CVS, in addition to the creation of an international health center to treat children with the illness, is giving endoscopy team members new diagnostic perspective. History Samuel Gee, MD, was the first to write about CVS in 1882. Remembered for his clinical observations, the British physician recorded information about patients who were having, "fits of vomiting which recur at intervals of uncertain length. The intervals themselves are free from sign of disease. The vomiting continues for a few hours or a few days. When it had been severe, the patients are left much exhausted ... the closest observation fails to discover anything which can be called a cause." It wasn't until more than 100 years later, in 1993, when scientists became committed to understanding the etiology of CVS. Officials from the Cyclic Vomiting Syndrome Association (CVSA) gathered researchers for the first International Scientific Symposium in 1994 at St. Bartholomew's Hospital in London and again in 1998 in Milwaukee. Researchers discussed a plethora of topics from the pathogenisis of migraines associated with cyclic vomiting to problems of clinical management of the illness. However, to date, the cause is still widely speculated.

starts vomiting and is unable to stop. This has been going on for almost 5 years now - he has
just turned 28.
They have diagnosed him with cyclical vomiting as everything else they test him for comes back clear (irritable bowel, allergies, crohn’s disease etc etc)
He has had pretty much every test under the sun (including gastroscopy, endoscopy, small bowel
series, cat scan, MRI, infra-red scans, blood tests), all of which are always clear.
The strangest thing about these attacks is that the only thing that stops them is if he is given
an injection of maxalon and morphine. Maxalon alone does not work, neither does anything else
that they have ever tried (including tramal, stemitel (), valium etc etc alone or in any
combination). Because morphine is such a strong narcotic and so addictive, Dr’s are reluctant to give it to him in the long-term, which means we are desperately searching for an answer or an alternative.
