Current Treatment Options in Gastroenterology Published Abstract For the last century, empiric therapy has been used to treat the relentless vomiting and resulting dehydration associated with cyclic vomiting syndrome. Despite its unknown pathogenesis, in the last decade, uncontrolled trials of various antimigraine and antiemetic agents have demonstrated rates of efficacy of 40% to 90%. Antimigraine agents are used to prevent or abort episodes, whereas antiemetic agents are used to attenuate symptoms during episodes. A positive family history of migraine headaches renders the patient more likely to respond to antimigraine therapy. In addition to antimigraine therapy, antiemetic sedative/anxiolytic, neuroleptic and gastroinestinal prokinetic agents may be useful. Cyclic Vomiting Syndrome
Cyclic vomiting syndrome: A syndrome characterized by episodes, bouts, or cycles of severe nausea and vomiting that last for hours or even days and alternate with longer asymptomatic periods (with no symptoms). The cause of the syndrome is unknown. Each episode is similar to previous ones and tends to start at about the same time of day, last the same length of time, and present the same symptoms at the same level of intensity. Episodes of CVS can be so severe that a person may have to stay in bed for days, unable to go to school or work. Because other common diseases and disorders can also cause cycles of vomiting, people with the syndrome may initially be misdiagnosed. Age of onset : Cyclic vomiting syndrome (CVS) affects children more often than adults. It starts most commonly between ages 3 and 7. In adults, episodes tend to occur less often but last longer than in children and the events that trigger episodes in adults cannot be pinpointed as easily as they can in children. Phases of the syndrome : CVS has four phases:
  • Prodrome The prodrome phase signals that an episode of nausea and vomiting is about to begin. This phase, which is often marked by abdominal pain, can last from a few minutes to several hours. Sometimes, taking medicine early in the prodromal phase can stop an episode in progress. However, sometimes there is no warning: a person may simply wake up in the morning and begin vomiting.

    Cyclic vomiting syndrome is a rare digestive disorder that affects children and adults. This disorder is characterized by chronic nausea, vomiting, extreme fatigue, motion sickness, abdominal pain and, in some cases, dizziness (vertigo) that may last for hours to days. Boles, MD Associate Professor of Pediatrics, Clinical Scholar Research Interest Mitochondrial DNA sequence variation as a risk factor in a wide variety of multi-factorial pediatric disorders, including migraine, cyclic vomiting, depression, irritable bowel and SIDS Phone Fax Email rboles@chla.usc.edu Mitochondrial DNA sequence variation and multi-factorial pediatric disorders Most common disorders are multi-factorial, in which sequence variation in many genes interact together with the environment to result in disease manifestations. It is not generally appreciated that many common disorders, including migraine, bipolar depression, diabetes and hypertension, demonstrate preferential maternal inheritance. Since the mitochondria have their own DNA (mtDNA) that is asexually inherited from the mother, this suggests that mtDNA polymorphisms may be part of the genetic component in these disorders. In fact, disease-associated mtDNA polymorphisms have been published in the first 3 of those disorders. As mtDNA only encodes for proteins involved in energy metabolism, this implies that the cellular energy state is a factor in the pathogenesis of many common disorders. This likely has implications in treatment; for example, co-enzyme Q10, an electron carrier in mitochondria, has recently shown efficacy in the treatment of migraine. I am a physician-scientist who has been investigating the connection between mtDNA sequence variation and multi-factorial pediatric disease, both in the clinic and in the laboratory. Cyclic vomiting syndrome (CVS), a disabling condition resulting in frequent episodes of nausea and vomiting, was chosen as a model disorder to study because the majority of cases demonstrate the maternal inheritance of multiple disease manifestations and biochemical markers of mitochondrial dysfunction. My group has published that the matrilineal relatives of CVS patients, who carry the same mtDNA sequence, have a three to five fold-increased risk for developing migraine, irritable bowel and depression, among other conditions. Thus, the mtDNA sequence confers an increased risk for the development of CVS and several common disorders as well.
