Cushing's Syndrome

Cushing syndrome is caused by prolonged exposure to elevated levels of either endogenous or exogenous glucocorticoids. The degree of cortisol overproduction is very variable. The combination of often non-specific clinical manifestations and variable, cyclical biochemical parameters often makes the diagnosis difficult Epidemiology Risk factors
  • Cushing syndrome caused by an adrenal or pituitary tumour is more common in females (ratio 5:1).
  • Peak incidence of Cushing syndrome caused by an adrenal or pituitary adenoma is between the ages of 25 and 40 years.
  • Ectopic ACTH production due to lung cancer occurs later in life.
Common causes
  • Most common cause of Cushing's syndrome is the use of exogenous glucocorticoids.
  • Endogenous glucocorticoid overproduction may be due to:
    • ACTH secreting pituitary adenoma (Cushing's disease; 65% of endogenous cases). Presents with typical features of hypercortisolism, with little virilisation.

Adrenal Cortex And Cushing's Syndrome
Physiology The adrenal cortex produces: glucocorticoids (eg cortisol) which affect metabolic functions (carbohydrate, lipid and protein metabolism) and mineralocorticoids (eg aldosterone). Corticotrophin-releasing factor (CRF) from the hypothalamus stimulates ACTH (from pituitary) which stimulates cortisol and androgen production by the adrenal cortex. Cortisol is excreted as urinary free cortisol and as various 17-oxogenic steroids (17OGS). Cushing's syndrome Most cases of Cushing's syndrome are caused by exogenous corticosteroids. Of spontaneous cases, approximately 70% are caused by Cushing's disease of a pituitary ACTH-producing tumour, 15% are due to ectopic ACTH production, and 15% to a primary adrenal tumour. 90% of pituitary causes are microadenomas (defined as lesions less than 1cm in size) and MRI only detects 70% of them. The annual incidence of Cushing's disease is between 0.1 and 1.0 per 100,000

    Cushings Disease in Dachshunds What is it? Cushings disease/syndrome is a condition where the body produces too much hormone, particularly corticosteroids or cortisol. What causes it? The majority of the Cushings cases are caused by a benign tumor on the pituitary or adrenal glands which causes overproduction of a hormone called adrenocorticotropic hormone which stimulates the production of corticosteroids by the adrenal gland. Other cases have occurred as the result of over administration of corticosteroids like Prednasone. Typical Symptoms? Some of the typical symptoms of Cushings are, increased or excessive drinking, increased or excessive urination, an enlarged or distended abdomen, muscle weakness, hair loss or thinning hair, panting, weakening of the heart and skeletal muscles, obesity and chronic or frequent infections. Diagnosis and Treatment As with any suspected medical condition, you should contact your veterinarian if you become concerned about your pets health. As part of an examination for Cushings a veterinarian will usually perform a complete blood count and chemistry panel. These tests will normally suggest the existence of Cushings. To confirm it, a test known as a low dose dexamethasone test is done. A baseline blood sample is drawn in the morning, an injection of desamethasone given and a followup blood test done 8 hours later. In a normal dog, the dexamethasone should suppress cortisol levels in the blood stream. In Cushing's disease this effect does not occur.

    What Are the Symptoms?
    Symptoms vary, but most people have upper body obesity, rounded face, increased fat around the neck, and thinning arms and legs. Children tend to be obese with slowed growth rates. Other symptoms appear in the skin, which becomes fragile and thin. It bruises easily and heals poorly. Purplish pink stretch marks may appear on the abdomen, thighs, buttocks, arms and breasts. The bones are weakened, and routine activities such as bending, lifting or rising from a chair may lead to backaches, rib and spinal column fractures. Most people have severe fatigue, weak muscles, high blood pressure and high blood sugar. Irritability, anxiety and depression are common.

    Cushing's syndrome is a hormonal disorder caused by prolonged exposure to the hormone cortisol. It is called Cushing's disease when it is caused by a pituitary tumor. All other causes of the condition are termed Cushing's syndrome.
    Causes
    Prolonged or excess exposure to cortisol as a result of:

The drug is not currently available for general use in the United States. In England it is marketed by Arnolds Veterinary Products of Yorkshire under the name, Vetoryl. The company is attempting to gain FDA approval to market in the US.

Osteoporosis also may occur in the skull and ribs, and involvement of the pelvis may be associated with protrusio acetabuli. Exuberant callus may develop after rib fractures.
