Philipp Klages Introduction Cri Du Chat (CDC) is a rare genetic disorder that is caused by a missing piece (deletion) in the short arm of chromosome 5 (also known as 5p minus syndrome). Approximately 50 to 60 children are born each year in the United States with CDC. It is thought to be one of the most common human deletion syndromes with an occurrence rate of 1 in 20,000 to 1 in 50,000 live births. CDC was first identified in 1963 by Dr. Lejeune, the same physician who first wrote about Down syndrome. Features and Characteristics

Diagnosis The cry that is associated with CDC is so distinct, physicians can often make a diagnosis before a chromosome analysis is even completed. However, chromosome testing is used for a definitive diagnosis. Treatment There is no cure for CDC, however, with the proper intervention and education, individuals with CDC can lead full and meaningful lives. Speech and physical therapy may be particularly useful for children with CDC. There are varying levels of cognitive impairment - some children can succeed in a mainstream education, where others will need to be placed in a special education classroom. A recent study regarding cognitive function of children with typical CDC suggests that there was a discrepancy between receptive language skills and expressive language skills. The researchers indicate that these findings should provide optimism to parents regarding the capacities of their children, in that they can understand more complex language than what their expressive skills would suggest. Some children can learn to express themselves using short sentences, sign language, and/or gestures.

Definition
Cri du chat syndrome arises from the absence of a particular portion of chromosome 5. The absence of genetic material is called a deletion. Children born with this deletion in chromosome 5 have a characteristic mewing cry as newborns that is thought to be caused by abnormal development of the larynx (the organ containing the vocal chords). For this reason, the disorder is called cri du chat (or cat's cry) syndrome. In reference to the deletion and the area where it occurs, the disorder is also called 5p- (5 p minus) syndrome. Children with this syndrome have physical abnormalities, language and motor skill difficulties, and varying degrees of mental retardation . Cri du chat syndrome is a very rare disorder. Description
The original genetic material is arranged into 23 pairs of chromosomesone set of 23 chromosomes is inherited from each parent. Using a microscope, distinct chromosomes can be seen at a certain point in a cell's life cycle. When treated with certain stains, each chromosome bears a typical banding pattern, reminiscent of the stripes on a tabby cat's tail. Using each chromosomes' unique size and banding pattern, chromosomes 1-22 and the sex chromosomes, X and Y, can be identified exactly. Although all individuals have two copies of chromosome 5, the normal chromosome does not compensate for the genetic material missing from the flawed chromosome. The deletion can be inherited through generations of a family or it may appear at random. It has been estimated that the syndrome occurs in 1 in 50,000 births. According to the 5p- Society, approximately 50-60 children are born with cri du chat syndrome each year in the United States.

Causes, incidence, and risk factors: The cri du chat syndrome is caused by the deletion of information on chromosome 5. It is likely that multiple genes on chromosome 5 are deleted. One deleted gene, called TERT (telomerase reverse transcriptase) is involved in control of cell growth, and may play a role in how some of the features of cri cu chat develop. The cause of this rare chromosomal deletion is not known, but it is expected that the majority of cases are due to spontaneous loss of a piece of chromosome 5 during development of an egg or sperm. A minority of cases result from one parent carrying a rearrangement of chromosome 5 called a translocation. Between 1 in 20,000 and 1 in 50,000 babies are affected. This disease may account for up to 1% of individuals with severe mental retardation. Infants with cri du chat syndrome commonly have a distinctive cat-like cry. They also have an extensive grouping of abnormalities with severe mental retardation being the most important.

The mental retardation must be addressed, and counseling is recommended for the parents. If a child is found to have this syndrome, his parents should have genetic counseling and a karyotype test to make sure that they do not have rearranged chromosomes that predispose to the child missing part of chromosome 5.
