5.03 Costochondritis
Presentation
The patient's age is usually in the mid-teens through the thirties, and he complains of a day or more of steady aching with intermittant stabbing chest pain, perhaps following a period of frequent coughing or unusual physical stress, localized to the left or right of the sternum, without radiation, but worse with taking a breath, changing position or moving the arm overhead. He may be concerned about the possibility of a heart attack (though he may not voice his fear) but there is no associated nausea, vomiting, diaphoresis, or dyspnea. The mid anterior costal cartilages (connecting ribs to sternum) are diffusely tender to palpation, without swelling or erythema, exactly matching the patient's complaint. The rest of the physical examination is normal, along with normal vital signs and pulse oximetry.
What to do:
  • Perform a thorough history and physical examination. Give special attention to the character of the pain (onset, severity, quality, radiation, duration, relationship to movement), associated symptoms (shortness of breath, nausea, vomiting, diaphoresis, cough), and past history of pre-existing cardiac risk factors (family history of coronary artery disease, smoking, hypertension, diabetes mellitus, elevated cholesterol, cocaine use, age >33 for men and >40 for women). Read the nurse's note for critical details the patient has not repeated to you. Look for pleural or pericardial rubs and arrythmias and obtain a cardiogram and chest x ray when there is any suspicion of a cardiac or pulmonary disorder. The presence of costochondritis does not exclude the possibility of myocardial infarction, pericarditis, pulmonary embolus pneumothorax, pneumonia, or pleural effusion.

What is costochondritis?
Costochondritis is an inflammation of the cartilage that connects the inner end of each rib with the breastbone (sternum). It can occur in any age group and is most common in young adults. Exactly how common it is in the UK is not known but in the US costochondritis has been shown to account for 10 per cent of chest pain episodes in the community and 30 per cent of people with chest pain presenting as an emergency to hospital.

My mother, who is 40, was diagnosed with costochondritis two months ago after suffering from pain. She is also clinically depressed after her sister passed away suddenly six months ago, and is on medication for this. I have tried to find information about this disease everywhere but have failed. Please can you explain what this is and where I can find out more about it so that I may understand what is happening? I am really worried about her.
Answer
In making the diagnosis it is important to consider other causes of chest pain. The most obvious is a heart attack (myocardial infarction). Costochondritis may also be confused with infection (sepsis) or rheumatoid arthritis. The onset may be rapid or gradual. The condition makes coughing and deep breathing painful.
I hope your mother's discomfort rapidly disappears.

Costochondritis is simply inflammation of the rib joints,
and attachments of intercostal muscles to the ribs, due to
persistant "bracing" of the intercostal muscles. So far,
the only known cause of this persistant tension of the
intercostal muscles is chronic stress-effect storage in
the hypothalamus. The well known fight or flight mechanism is the only known way the hypothalamus has to respond to ANY form of stressor. See the glossary and search engine for stress. The stressors, for which physically running or fighting are NOT appropriate, result in that readiness being stored in the hypothalamus. Nearly all stressors in our present day culture cannot be resolved by running or fighting.

Costochondritis is a common form of inflammation of the cartilage where ribs attach to the breast bone, the sternum. The inflammation can involve multiple cartilage areas on both sides of the sternum but usually is on one side only. The cause(s) of costochondritis is(are) not known and may involve several factors. Possible causes include heredity (genetic predisposition), viruses, and trauma (injury). Costochondritis can be an independent condition by itself or sometimes be a feature of a more widespread disorder. Costochondritis is a frequent cause of anterior chest pain. It is important to rule out cardiac disease as a source of the pain. The pathogenesis of costochondritis is unknown. The 2 nd to 5 th costal cartilages are most frequently affected. Palpation of the affected areas cause localized pain as well as widely referred pain. The condition occurs most frequently in women. Treatment involves the use of NSAIDS and application of heat. If pain is resistant to these therapies injection of the costochondral joint with local anesthetic and steroid might be of benefit.

What is costochondritis?
Costochondritis is inflammation of the joint between a rib and the breastbone (sternum) or between the bony part of the rib and the rib cartilage. (Cartilage is a tough rubbery tissue that lines and cushions the surfaces of joints.) It is more common in women than men and tends to occur more often in people over 40 years old.
How does it occur?
Sometimes costochondritis is caused by:
  • an injury to the chest; for example, from falling or getting hit by something in the chest an infection, such as a cold or flu.
Many times the cause cannot be found.

    costochondritis
    The rib cage flexes during respiration to allow more air to enter than would be the case if it were rigid. Costochrondritis is a condition in which there is pain which is localized in the flexible joints (costochondral junctions) of the rib cage, where the ribs attach somewhat flexibly to the breast bone (sternum). For reasons unknown, adolescents going through puberty seem to be prone to developing inflammation of these joints, often after exercise or a blow to the chest. The pain may be sharp and sticking at first, but it usually becomes a dull aching pain. The most common sites of this pain are where the left fourth and fifth ribs attach to the sternum. This is right in the area of the heart, and therefore can create a good bit of anxiety in both the child and the parents. The key to diagnosis is that the pain can be reproduced by pressing on the involved area of the chest wall. This pain is not as sharp and intense as that of a rib fracture (which might be considered if there is a history of a blow to the chest). The treatment of costochondritis is anti-inflammatory medicine such as ibuprofen or naprosyn.

    Costochondritis
    I was recently diagnosed with costochondritis. What is it exactly and how should I treat it? My doctor has said that I can continue with vigorous exercise as normal. Is this really sensible with a fractured sternum? Rebecca
    Dr Trisha Macnair responds
    The prefix costo- simply means something which is to do with the ribs, the prefix 'chondr-' means related to the cartilage and '-itis' means inflammation. So if you put all of these together you get 'costochondritis', which means inflammation of the cartilage of the ribs. In particular, costochondritis affects the cartilage which joins the ribs to the breastbone. Mechanical problems or trauma usually cause it There are a variety of possible causes. Most common is trauma such as damage to the sternum. You mention a fractured sternum so I guess that in your case whatever broke your sternum also led to inflammation in the tissues which surround it. Other causes include:
    • mechanical pressure or stress on the sternum viral infections which cause inflammation of cartilage
    Symptoms may be mistaken for heart disease Costochondritis causes tenderness over the breastplate and pain if you put pressure on the ribs in this area. The pain is made worse by anything which moves the ribs and pulls on the cartilage which connects them to the sternum.
