Coronary heart disease
Coronary heart disease
(CHD), also called coronary artery disease (CAD) and atherosclerotic heart disease, is the end result of the accumulation of atheromatous plaques within the walls of the arteries that supply the myocardium (the muscle of the heart ). While the symptoms and signs of coronary heart disease are noted in the advanced state of disease, most individuals with coronary heart disease would have evidence of disease decades before the first symptoms arise. After decades of progression, some of these atheromatous plaques may rupture and (along with the activation of the blood clotting system) start limiting blood flow to the heart muscle . Current views are that an inflammatory process of the lining of the arteries, though poorly understood in specifics, promotes the disease progression.

Coronary heart disease, also know as ischemic heart disease, is commonly caused by atheromatous lesions of the coronary artery. Its major clinical manifestations are angina pectoris and myocardial infarction. Although the terms angina pectoris and acute myocardial infarction were not used in ancient times, descriptions of the clinical manifestations of coronary heart disease are contained in the ancient texts of traditional Chinese medicine. Etiology and Pathogenesis Precordial pain is the most prominent feature of coronary heart disease. According to traditional medical theory, obstruction in the heart vessels usually causes this pain. The vessels may be blocked by phlegm accumulation in the chest, which obstructs the yang qi, and /or by blood stasis either due to qi deficiency or due to qi stagnation.     Definition
    Coronary artery disease (CAD) , also known as coronary heart disease (CHD) or coronary atherosclerosis, involves the progressive narrowing of the arteries that nourish the heart muscle. Often there are no symptoms, but if one or more of these arteries become severely narrowed, angina may develop during exercise, stress, or other times when the heart muscle is not getting enough blood.
    Cause
    The narrowing is due to a buildup of fatty plaque (atherosclerosis) along the artery walls. These deposits are composed mostly of cholesterol, other lipids, and fibrous tissue, such as collagen. Coronary disease appears to be a lifelong process in some people, beginning at an early age and progressing slowly until the vessels become so occluded that the heart muscle no longer gets adequate nourishment. The underlying cause is unknown, although it is seen most frequently in people who live in developed industrialized nations.
    Diagnosis
    myocardial ischemia A presumptive diagnosis of coronary disease is based on a review of symptoms, health history, an electrocardiogram, and an exercise stress test, perhaps with a thallium scan. A more definitive diagnosis requires cardiac catheterization and angiography.

Coronary artery disease is caused by atherosclerosis, which is a gradual buildup of material inside the arteries. The blood paths get smaller and decrease the amount of blood that can flow. If a blood clot plugs one of these arteries or blood flow decreases enough, the muscle starved for oxygen dies and a heart attack occurs. Atherosclerosis is a complex disease which often progresses slowly for decades and becomes serious only in older age. For some people in their thirties, it develops rapidly.
