What is cor pulmonale?
Cor pulmonale is enlargement of the heart's right pumping chamber (right ventricle) due to high blood pressure in the lungs. The most common causes are chronic lung diseases such as emphysema (also referred to as chronic obstructive pulmonary disease, or COPD), pulmonary fibrosis, chronic bronchitis, and sleep apnea. All these conditions can increase the resistance to blood flow through the lungs. This increases the workload of the right ventricle until it can't quite keep up with the demand. Cor pulmonale usually occurs late in the course of lung disease. Less commonly, cor pulmonale may be caused by small blood clots that repeatedly come from veins lower in the body and lodge in the lung blood vessels. Over time these clots, or emboli, tend to block blood flow and increase the work of the heart's right ventricle. Other conditions may also cause high blood pressure in the lungs.

    Cor pulmonale
    Definition: Failure of the right side of the heart caused by prolonged high blood pressure in the pulmonary artery and right ventricle of the heart.
    Alternative Names: Right-sided heart failure - cor pulmonale
    Causes, incidence, and risk factors: Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure. Any condition that leads to prolonged high blood pressure in the arteries or veins of the lungs (called pulmonary hypertension ) will be poorly tolerated by the right ventricle of the heart. When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale. Almost any chronic lung disease or condition causing prolonged low blood oxygen can lead to cor pulmonale. A few of these causes include:

    Los IECA reducen la morbimortalidad en pacientes con fallo ventricular izquierdo o congestivo. Su efecto a largo plazo en pacientes con cor pulmonale crónico (CPC) no ha sido investigado. Nos propusimos evaluar el efecto del enalapril sobre la capacidad de esfuerzo y función cardiorrespiratoria en pacientes con CPC. Métodos: estudio prospectivo, doble ciego, aleatorizado, controlado con placebo. Veintiocho pacientes con CPC, clínicamente estables, fueron asignados aleatoriamente a recibir enalapril o placebo, vía oral, durante 6 meses, adicionado a su terapia habitual. Antes del inicio y a los 6 meses se realizaron test de función respiratoria, de tolerancia al esfuerzo ("test de 6 minutos caminando") y ventriculografía isotópica.
