Cor pulmonale
Definition
Failure of the right side of the heart caused by prolonged high blood pressure in the pulmonary artery and right ventricle of the heart.
Alternative Names
Right-sided heart failure - cor pulmonale
Causes
Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure. Any condition that leads to prolonged high blood pressure in the arteries or veins of the lungs (called pulmonary hypertension ) will be poorly tolerated by the right ventricle of the heart. When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale. Almost any chronic lung disease or condition causing prolonged low blood oxygen can lead to cor pulmonale. A few of these causes include:

V Whitman, RC Stern, P Bellet, CF Doershuk, J Liebman, TF Boat, G Borkat and LW Matthews The effects of acute diuresis in patients with cor pulmonale secondary to cystic fibrosis were studied. Both hemodynamic parameters and arterial blood gas changes were investigated. The major effect noted was a significant reduction of systemic venous pressure. Pulmonary artery and pulmonary artery wedge pressure either remained constant or declined slightly. No consistent changes were noted in cardiac output or arterial blood gases. Acute diuresis of moderate degree appears to be a safe and

It also discloses evidence of severe chronic lung disease. right-sided chamber enlargement indicates right ventricular failure or severe tricuspid regurgitation. Echocardiography and MRI usually indicates increase in thickness of the right ventricular myocardium, and increase in the size of the right-sided chambers. Echocardiography demonstrates associated pulmonary and tricuspid regurgitation. Doppler echocardiography can be used to estimate the pulmonary artery pressure by measuring the peak velocity of the tricuspid regurgitant jet and applying the modified Bernoulli equation

These goals are achieved through long-term oxygen therapy, bronchodilator and vasodilator therapy, aggressive treatment of pulmonary infection, and anticoagulation. Cor pulmonale
