Incidence
The precise incidence of cor pulmonale is not known due to the practical difficulties involved in measuring right heart pressures. One study has estimated that in England and Wales alone, there may be approximately 60,000 people who are at risk of developing cor pulmonale and who might benefit from oxygen therapy. right ventricular hypertrophy +/- dilatation and may ultimately result in right heart failure . The pulmonary arterial hypertension associated with chronic respiratory disease occurs as a result of structural changes in the pulmonary vascular bed in response to chronic hypoxia. Acute cor pulmonale may also occur, most commonly as a result of pulmonary embolic disease. Respiratory Causes of Cor Pulmonale Cor pulmonale may occur as a result of many forms of chronic lung disease whether obstructive, restrictive or due to poor respiratory drive. Although chronic obstructive pulmonary disease ( COPD ) is by far the most common underlying respiratory cause accounting for 80-90% of all cases, other lung disease may produce cor pulmonale including:-

In 1985 the Ontario legislature established the Industrial Disease Standards Panel (IDSP) to investigate and identify diseases related to work. The Panel is independent of both the Ministry of Labour and the Workers' Compensation Board. At the end of each fiscal year the WCB reimburses the Ministry for the Panel's expenditures. The Panel's authority flows from section 95 of the Workers' Compensation Act and its functions are set out as follows: (8) (a) to investigate possible industrial diseases; to make findings as to whether a probable connection exists between a disease and an industrial process, trade or occupation in Ontario; to create, develop and revise criteria for the evaluation of claims respecting industrial diseases; and to advise on eligibility rules regarding compensation for claims. Occupational Disease Panel
When the Workers' Compensation Act was amended in 1985, the Occupational Disease Panel was created. The Panel was set-up to provide the Workers' Compensation Board with scientific and policy advice about the recognition and compensation of work-related diseases. The need for the Panel was identified by several individuals, including Professor Weiler, who studied the workers' compensation system and identified systemic under-compensation of disease claims. The Panel is a decision-making body that is composed of representatives of the public and the scientific community and technical and professional persons. It has a small staff and is assisted by consultations with renowned experts in the areas of medicine and science. The statute specifically sets out the following mandate ( Workers' Compensation Act , R.S.O. 1990, c. W. 11, s.95):

Cor pulmonale

Cor Pulmonale
Definition
Cor pulmona is characterized by hypertrophy of the right ventricle resulting from disease of the lungs, except for lung changes in diseases that primarily affect the left side of the heart and excluding congenital heart disease; acute cor pulmonale is characterized by dilation and failure of the right side of the heart due to pulmonary embolism. In both types, characteristic electrocardiogram changes occur, and in later stages there is usually right-sided cardiac failure.
Radiographic Appearance
The chest x-ray may reveal right ventricular hypertrophy, manifest as filling of the retrocardiac space on the lateral film, and increased PA size. Image 2
Pathology
Enlargement of the right ventricle occurs from pulmonary hypertension, elevated blood pressure in the lungs. Pulmonary hypertension is usually caused by lung disease (most commonly chronic bronchitis or emphysema).
Any disease that causes chronically low blood levels of oxygen may lead to pulmonary hypertension and cor pulmonale. This condition does not include right ventricle enlargement that results from congestive heart failure or heart valve disease. Cor pulmonale is usually chronic (long-term), but it may be acute and reversible.
