Definition
Cor pulmonale is an increase in bulk of the right ventricle of the heart, generally caused by chronic diseases or malfunction of the lungs. This condition can lead to heart failure Description
Cor pulmonale, or pulmonary heart disease, occurs in 25% of patients with chronic obstructive pulmonary disease (COPD). In fact, about 85% of patients diagnosed with cor pulmonale have COPD. Chronic bronchitis and emphysema are types of COPD. High blood pressure in the blood vessels of the lungs ( pulmonary hypertension ) causes the enlargement of the right ventricle. In addition to COPD, cor pulmonale may also be caused by lung diseases, such as cystic fibrosis , pulmonary embolism and, pneumoconiosis. Loss of lung tissue after lung surgery or certain chest-wall disturbances can produce cor pulmonale, as can neuromuscular diseases, such as muscular dystrophy . A large pulmonary thromboembolism (blood clot) may lead to acute cor pulmonale.
Any respiratory disease or malfunction which affects the circulatory system of the lungs may lead to cor pulmonale. These circulatory changes cause the right ventricle to compensate for the extra work required to pump blood through the lungs. The right ventricle has thin walls and is crescent-shaped. The resulting pressure causes the right ventricle to dilate and bulge, eventually leading to its failure.

Causes, incidence, and risk factors: Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure. Any condition that leads to prolonged high blood pressure in the arteries or veins of the lungs (called pulmonary hypertension ) will be poorly tolerated by the right ventricle of the heart. When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale. Almost any chronic lung disease or condition causing prolonged low blood oxygen can lead to cor pulmonale. A few of these causes include:

Cor Pulmonale
Right ventricular enlargement secondary to a lung disorder that produces pulmonary artery hypertension (eg, intrinsic pulmonary disease, abnormal chest bellows, depressed ventilatory drive). Cor pulmonale does not refer to right ventricular (RV) enlargement secondary to left ventricular (LV) failure, congenital heart disease, or acquired valvular heart disease. It is usually chronic but may be acute and reversible.
Etiology
Acute cor pulmonale usually results from massive pulmonary embolization but often occurs as acute reversible exacerbations of chronic cor pulmonale in patients with COPD, usually during acute respiratory infection. Chronic cor pulmonale is usually caused by COPD (chronic bronchitis, emphysema) and less often by extensive loss of lung tissue from surgery or trauma, chronic unresolved pulmonary emboli, primary pulmonary hypertension (see below ), pulmonary veno-occlusive disease, scleroderma, diseases leading to diffuse interstitial fibrosis, kyphoscoliosis, obesity with alveolar hypoventilation, neuromuscular diseases involving respiratory muscles, and idiopathic alveolar hypoventilation.

General Information
DEFINITION Congestive heart failure resulting from raised blood pressure in the lungs. This is a complication of disorders that slow or block blood flow in the lungs. BODY PARTS INVOLVED Lungs; heart; blood vessels. SEX OR AGE MOST AFFECTED Both sexes and all ages, but most common in men over 40. Early stages:
  • No symptoms (usually).
Later stages:
  • Weakness and fatigue. Shortness of breath with exertion. Frequent fainting. Swelling of the ankles and feet caused by fluid retention. Distended neck veins. Bluish skin. Chest pain. Enlarged liver and swollen abdomen.

  • Primary diseases of the heart, including rheumatic heart disease and congenital heart disease.
RISK INCREASES WITH
  • Prolonged bed rest for any illness. This increases the chance of blood-clot formation. Smoking. Living at high altitudes. Occupational exposure to lung damaging materials.

Peer Review Status: Externally Reviewed by the Department of Internal Medicine Virtual Hospital Editorial Board Other than the development of bronchogenic carcinoma, pulmonary hypertension and cor pulmonale may be the most serious consequences of COPD. Pulmonary hypertension is suspected on clinical grounds based on a loud P2, paradoxical splitting of S2, and prominent pulmonary arteries on CXR. Paradoxical splitting is due to the fact that during systole the pressure in the pulmonary artery increases until the pulmonic valve closes before the aortic valve. At that point, the splitting decreases on inspiration as the pulmonic valve closure is delayed. Without treatment, pulmonary hypertension can progress to cor pulmonale: right heart failure associated with pulmonary disease and pulmonary vascular disease in the absence of congenital heart disease or left sided heart disease. These findings are specific but insensitive, present in only one third of the patients with cor pulmonale.

GENERAL INFORMATION: What is it? Cor pulmonale (kor pull-muh-nall-e) is when the right side of your heart begins to fail. Heart failure means that the pumping power of the heart is weaker than normal. Causes: Cor pulmonale is caused by high blood pressure in the lungs. This high blood pressure may happen if you have lung problems, such as pulmonary embolism (m-bull-ih-zum). A pulmonary embolism is a blood clot closing one of the arteries in the lung. Chronic obstructive pulmonary (pull-mun-air-e) disease or "COPD" can also cause cor pulmonale. Or, other problems with the blood vessels in the lungs can cause right-sided heart failure. Signs and Symptoms: You may have one or more of the following signs or symptoms.

Pathophysiology
As described elsewhere in this monograph, COPD is, in most patients, a mixture of two or three related pulmonary diseases. These include asthmatic bronchitis and chronic bronchitis, which involve inflammation of the airways, bronchi, and bronchioles; and pulmonary emphysema, which affects the lung parenchyma through destruction of alveolar walls and their associated capillary circulatory framework. In emphysema, the capillary bed is progressively and irreversibly destroyed, which eventually raises pulmonary vascular resistance and pulmonary artery pressure. Persistent and worsening pulmonary hypertension, in turn, finally results in cor pulmonale. Cor pulmonale usually appears only in the very advanced stages of emphysema.

Cor pulmonale

Es la insuficiencia del lado derecho del coraz³n causada por una presi³n arterial alta prolongada en la arteria pulmonar y en el ventr­culo derecho del coraz³n.
Causas, incidencia y factores de riesgo: Normalmente, el lado izquierdo del coraz³n produce un nivel mayor de presi³n sangu­nea con el fin de bombear sangre al cuerpo; el lado derecho bombea a trav©s de los pulmones bajo una presi³n mucho m¡s baja. Cualquier condici³n que lleve a un aumento prolongado de la presi³n en las arterias o venas de los pulmones (denominada hipertensi³n pulmonar) no ser¡ bien tolerada por el ventr­culo card­aco derecho. La falla o incapacidad de ©ste para bombear contra estas presiones inusualmente elevadas se denomina cor pulmonale. Casi cualquier enfermedad o condici³n pulmonar cr³nica que produzca disminuci³n prolongada del ox­geno sangu­neo puede llevar a cor pulmonale. Algunas de estas causas son:
