Emergency department presentation of patients with conversion disorder

Geisinger Medical Center, Department of Emergency Medicine, Danville, PA 17822, USA. OBJECTIVE: To describe the presenting symptoms and other features of ED patients diagnosed as having conversion disorder. METHODS: A retrospective chart review was carried out on the records of ED patients who had had final ED or in-hospital diagnoses of conversion disorder. Cases from 1982 to 1992 at a 566-bed rural tertiary care hospital with a residency program in emergency medicine were reviewed for patient age and gender, presenting signs and symptoms, ED diagnostic evaluation, disposition, and comorbidity.

From 1 to 3% of neurological patients still receive this diagnosis. (*) Choosing patients with one-sided paralysis was important, because it allowed very close comparisons between the healthy arm and the paralyzed one. Brain scans could then compare the left and right sides of the same person, at the same time and in the same brain. Patients were also compared during their period of hysterical paralysis and afterwards, so that each subject served as his or her own control. Such close comparisons are essential, because human beings and their brains are so variable. All patients were facing stressful life events when they came down with their paralytic symptoms, just as one might expect. But none had major psychiatric or neurological disorders. MRI scans showed no detectable physical impairment. Dozens of other tests confirmed the absence of other diagnoses. All these careful selection criteria allowed the researchers to focus just on the hysterical impairment and minimize extraneous influences.

The amobarbital sodium interview in conversion disorders: use of video feedback in therapy
RH Bradley, CL Zonia, and SJ Caputo The prognosis in treating acute conversion disorder is good, but it demands considerable time in the form of an intensive, and often extensive, hospital stay. With the increasing trend to limiting hospital stays, the family practitioner working with a psychiatrist in the hospital setting is pressured to find new approaches in order to limit the use of inpatient units. The authors describe a case of classic conversion disorder and outline a successful approach that uses a video recording of the interview, during which the patient is under the influence of amobarbital sodium. The videotape then provides valuable feedback during future therapy sessions in the treatment of this persistent disorder.

Alternate Names : Hysterical Conversion Attribution A conversion disorder is a condition in which a person develops certain physical symptoms, such as paralysis or visual impairment , in response to severe psychological stress. No physical cause can be found for these symptoms. What is going on in the body? This disorder occurs most often among people 10 to 35 years old. Among older people, there is a much greater chance that unexplained symptoms are caused by a medical problem. Usually, the disorder lasts only for a few weeks or months. During that time, a person may be unable to work or even carry out daily activities. Conversion disorder is not thought to make a person more vulnerable to real health problems. Many people who suffer conversion symptoms have medical problems as well.

Most conversion episodes will only end with support and reassurance from healthcare providers and family members. If the symptoms do not go away within a few weeks, psychotherapy is usually the best treatment. During therapy, the person is encouraged to:
  • discuss stressful events and situations learn how the disorder develops find healthy ways to cope with stress
  • Hypnosis has also been used successfully. While hypnotized, a person can be told that the symptom will soon disappear. A similar approach is to talk to the person after he or she takes a relaxing medication. This sometimes helps a therapist identify the underlying conflict. Someone who has been sexually abused usually needs help to overcome the painful effects of this trauma.

    Conversion disorder. Camouflage for brain lesions in two cases

    Department of Psychiatry, University of Wisconsin Hospital and Clinics, Madison. We present two cases of serious central nervous system disease initially misdiagnosed as conversion disorder. In each instance, multiple medical evaluations did not uncover the underlying neurologic disorder. Skepticism on the part of psychiatric evaluators led to eventual diagnosis. Common difficulties in the clinical application of the diagnostic criteria for conversion disorder are discussed in relation to the specific pitfalls of the two cases.     Definition: Conversion disorder is a psychiatric condition in which emotional distress or unconscious conflict are expressed through physical symptoms. Alternative Names: Hysterical neurosis Causes And Risk: Conversion disorder is one of several types of somatoform disorders, in which psychological problems produce physical symptoms. These disorders have typically been highly stigmatized, with physicians telling patients that the problem is "all in your head." Research on the connection between mind and body is steadily increasing our understanding of these disorders and should reduce the stigma, as it becomes clear that these conditions are real, cause real distress, and cannot just be turned on and off at will. The onset of symptoms in this disorder is usually very sudden and follows a stressful experience. Loss of function, such as in inability to move a limb, may unconsciously symbolize the underlying conflict associated with the experience. Risk factors include a history of histrionic personality disorder or dependent personality disorder Symptoms: The symptoms of conversion disorder involve the loss of one or more bodily functions. These may include blindness, paralysis or the inability to speak. The loss of physical function is involuntary and diagnostic testing does not show a physical cause for the dysfunction.

    Description The somatoform disorders are a group of mental disturbances placed in a common category in DSM-IV on the basis of their external symptoms. The terms hypochondriasis and hysteria were traditionally designated to these disorders and are still widely used.
