Overview Symptoms Treatment Prevention Definition: The conversion of emotional distress or unconscious conflict into a physical symptom. It is one of the general class of Somatoform disorders.
Alternative Names: Hysterical neurosis
Causes, incidence, and risk factors: The symptom onset of this disorder is usually very sudden and follows a stressful experience. The loss of function may symbolize the underlying conflict associated with it. Psychodynamic theory interprets the cause of the symptoms as a defense mechanism that absorbs and neutralizes the anxiety generated by an unacceptable impulse or wish. Risk factors include a history of histrionic personality disorder or dependent personality disorder

Introduction Clinical Features Care Needs and Mobility Considerations Further Evidence 19.9.1 Introduction (i) In dissociative and conversion disorders the predominant symptoms are physical . The term conversion disorder implies that in the affected person anxiety has been replaced by (or "converted into") physical symptoms. It is assumed that the physical symptoms serve a function in that they enable the affected individual to avoid situations with which they cannot cope. These disorders are also forms of neurosis; but in view of their importance and the critically important need to differentiate them from malingering where an apparent disability is out of proportion to the physical condition,this separate section is devoted to them and related disorders. (ii) An alternative name for these disorders is hysteria. Although this term is still in use, many psychiatrists avoid it because colloquially it is used to describe exaggerated and extravagant displays of emotion. This is not the meaning of hysteria when used in the clinical context.

Body Dysmorphic Disorder Conversion Disorder Hypochondriasis Pain Disorder ... Somatoform Home In conversion disorder, a psychosocial conflict or need is converted into dramatic physical symptoms that affect voluntary motor or sensory functioning. The symptoms often seem neurological, such as paralysis, blindness, or loss of feeling (anesthesia). Most conversion disorders begin between late childhood and early adulthood, and are diagnosed twice as often in women as in men. Symptoms usually appear suddenly and during times of extreme stress, and last for several weeks. Diagnostic Criteria: A. One or more symptoms or deficits affecting voluntary motor or sensory function that suggest a neurological or other general medical condition. B. Psychological factors are judged to be associated with the symptom or deficit because the initiation or exacerbation of the symptom or deficit is preceded by conflicts or other stressors. C. The symptom or deficit is not intentionally produced or feigned (as in Factitious Disorder or Malingering).

Symptoms often disappear as quickly as they come, and might recur later in another stressful time. Freud’s view of conversion disorder: The energy cathected (invested) in a repressed instinct is diverted into sensory-motor channels, impairing functioning Stress + repressed thought is converted into physical symptoms. Through psychotherapy, Freud hoped to uncover the repressed thought and release the energy blocking the sensory-motor systems. Dangers in diagnosing conversion disorder : There may in fact be an underlying neurological disorders, such as an unrecognized stroke which produce similar symptoms. 2 studies done in the 1960’s showed approx. 60% of patients given a conversion diagnosis had some underlying organic problem. Malingering Will people fake symptoms of mental illness just to avoid negative consequences elsewhere in their life.

Among these are dissociative disorders. These disorders are relatively rare and may present initially as only nocturnal events. These fugue states may last hours, with loss of identity and memory. Patients usually have some diurnal symptoms and may have a history of physical or sexual abuse. They can occur in association with post-traumatic stress disorder. During polysomnographic monitoring, patients with dissociative disorders frequently demonstrate wakefulness before the onset of the spells. Simultaneous time-synchronized video monitoring is required to correlate the behavior with the electrographic data. Conversion disorders also present as nocturnal events.     Definition: Conversion disorder is a psychiatric condition in which emotional distress or unconscious conflict are expressed through physical symptoms.
    Alternative Names: Hysterical neurosis
    Causes, incidence, and risk factors: Conversion disorder is one of several types of somatoform disorders, in which psychological problems produce physical symptoms. These disorders have typically been highly stigmatized, with physicians telling patients that the problem is "all in your head." Research on the connection between mind and body is steadily increasing our understanding of these disorders and should reduce the stigma, as it becomes clear that these conditions are real, cause real distress, and cannot just be turned on and off at will. The onset of symptoms in this disorder is usually very sudden and follows a stressful experience. Loss of function, such as in inability to move a limb, may unconsciously symbolize the underlying conflict associated with the experience. Risk factors include a history of histrionic personality disorder or dependent personality disorder
    Symptoms: The symptoms of conversion disorder involve the loss of one or more bodily functions. These may include blindness, paralysis or the inability to speak. The loss of physical function is involuntary and diagnostic testing does not show a physical cause for the dysfunction.

She has become increasingly weak and has been unable to function- particularly with her hands. She has also has difficulty walking, and has lost weight and become lethargic. She has been hospitalized, and biopsies have shown a change in the musculature- particularly of the thenar area (the muscle between the thumb and forefinger). A diagnosis of amyotrophic lateral sclerosis, or Lou Gerhig's Disease, has been made. Patient gives a history of being very unhappy and isolated. She feels that the marriage to her husband of 27 years deteriorated. He spends very little time with her and she feels quite alone, particularly since the children have grown. HOME PAGE Case Studies: A Case of Conversion Disorder and Rage October 19, 1984 P.T. is a 21 year-old man who was referred to this office for psychiatric problems secondary to a work related accident. He was working on a construction site and driving a front-end loader when a dump truck crashed into the loader. He was trapped under the loader for approximately 25 minutes. He was taken to Shock Trauma at the University of Maryland Hospital. After a careful evaluation, it was found that although there were some severe strains, there were no actual physical injuries that required any type of surgical intervention. He was sent home on muscle relaxants and pain killers. Since he has been home he has found that he is quite limited physically. He is bent over, and walks with a great deal of difficulty- even with the assistance of a recently purchased cane. He has been followed by a number of physicians, all of whom feel that there is no obvious physical injury. They suspect that this could be a conversion disorder whereby he believes that he has been physically injured, but indeed has no obvious physical injury to the point that any type of surgical intervention is required. Patient gives a history of being raised in an extremely volatile household. His father, a policeman, would come home after duty and be extremely assaultive to the family. His father would also drink heavily. He remembers his mother being struck, and he and his brother being thrown around the room and being quite terrified. He would often urinate in his pants when these traumatic episodes would occur. He was a bed-wetter until the age of thirteen.

    Conversion disorder is a mental illness characterized by the loss or alteration of physical functioning without any physiological reason. These physical symptoms are the result of emotional conflicts or needs. The symptoms usually appear suddenly and at times of extreme psychological stress. A lack of concern over the debilitating symptoms (la belle indifference), which commonly accompanies this illness, may be a clue to distinguishing it from the physiological disorder it may mimic.
