Alternate Names : Hysterical Neurosis Definition Conversion disorder is a psychiatric condition in which emotional distress or unconscious conflict are expressed through physical symptoms. Conversion disorder is one of several types of somatoform disorders, in which psychological problems produce physical symptoms. These disorders have typically been highly stigmatized, with physicians telling patients that the problem is "all in your head." Research on the connection between mind and body is steadily increasing our understanding of these disorders and should reduce the stigma, as it becomes clear that these conditions are real, cause real distress, and cannot just be turned on and off at will. The onset of symptoms in this disorder is usually very sudden and follows a stressful experience. Loss of function, such as in inability to move a limb, may unconsciously symbolize the underlying conflict associated with the experience. Risk factors include a history of histrionic personality disorder or

Diagnostic Criteria for Conversion Disorder
  • One or more symptoms or deficits affecting voluntary motor or sensory function that suggest a neurological or other general medical condition.
  • Psychological factors are judged to be associated with the symptom or deficit because the initiation or exacerbation of the symptom or deficit is preceded by conflicts or other stressors.
  • The symptom or deficit is not intentionally produced or feigned (as in Factitious Disorder or Malingering).
  • The symptom or deficit cannot, after appropriate investigation, be fully explained by a general medical condition, or by the direct effects of a substance, or as a culturally sanctioned behavior or experience.
  • The symptom or deficit causes clinically significant distress or impairment in social, occupational, or other important areas of functioning or warrants medical evaluation.
  • The symptom or deficit is not limited to pain or sexual dysfunction, does not occur exclusively during the course of the Somatization Disorder, and is not better accounted for by another mental disorder. Source: APA, 1994.

Hysterics suffer from reminisces. -Breur and Freud, Studies in Hysteria People can have symptoms of disease for all sorts of reasons, but there is a small but significant subgroup of patients for whom no physical cause serves to explain their symptoms. When there is a presumed psychological explanation for the problem, this is called a somatoform disorder . The most classical and specific of the somatoform disorders is called conversion disorder - so called because psychological stressors are converted into physical symptoms. Somatoform disorders in general - and conversion disorder in particular - used to be known as hysteria. The word is derived from the Greek word for uterus, because it was presumed by the Greeks that the problem originated there. It follows from this that for many years conversion disorder was presumed to be exclusively a female disorder - which isn't true, although it is more commonly diagnosed in women than in men. This theory led to early and misguided treatment recommendations ranging from

    General Discussion
Conversion disorder is a mental illness characterized by the loss or alteration of physical functioning without any physiological reason. These physical symptoms are the result of emotional conflicts or needs. The symptoms usually appear suddenly and at times of extreme psychological stress. A lack of concern over the debilitating symptoms (la belle indifference), which commonly accompanies this illness, may be a clue to distinguishing it from the physiological disorder it may mimic.

    Definition
A conversion disorder is a condition in which a person develops certain physical symptoms, such as paralysis or visual impairment , in response to severe psychological stress. No physical cause can be found for these symptoms.
What is going on in the body?
This disorder occurs most often among people 10 to 35 years old. Among older people, there is a much greater chance that unexplained symptoms are caused by a medical problem.
Usually, the disorder lasts only for a few weeks or months. During that time, a person may be unable to work or even carry out daily activities. Conversion disorder is not thought to make a person more vulnerable to real health problems.
Many people who suffer conversion symptoms have medical problems as well.
What are the signs and symptoms of the condition?
This disorder starts suddenly. Its signs and symptoms may include: What are the causes and risks of the condition?

Scientists in fields connected to neurobiology and psychiatry remain mystified about the cause of Conversion Disorder. The disorder is characterized by physical symptoms of a neurological disorder, yet no direct problem can be found in the nervous system or other related systems of the body. This fact alone is not unusual; many diseases and symptoms have unknown origins. Conversion Disorder, however, seems to stem from "trivial" to traumatic psychological events and emotions rather than biological events. The extreme symptoms often disappear as quickly as they appear without the patient consciously controlling or feigning them. Thus, Conversion Disorder serves as a significant example of how blurred the conceived demarcated divisions of mind/body/behavior can be. Conversion Disorder is diagnosed solely by its physical symptoms seen in patients. Symptoms can be divided up into three groups: sensory, motor and visceral. Sensory symptoms include anesthesia, analgesia, tingling, and blindness. Motor symptoms may consist of disorganized mobility, tremors, tics, or paralysis of any muscle groups including vocal cords. Visceral functions include spells of coughing, vomiting belching, and trouble swallowing

    Conversion Disorder
DSM IV Criteria The patient complains of symptoms or deficits affecting voluntary muscles, or deficits of sensory function that suggest a neurological or medical condition. The temporal relation of symptoms to a stressful event suggests association of psychological factors. Symptoms are not intentionally produced. Symptoms are not explained by an organic etiology. Symptoms result in significant functional impairment. Symptoms are not limited to pain or sexual dysfunction, and are not explained. Symptoms most commonly are sensory (blindness, numbness) and motor deficits (paralysis, mutism). Abnormalities usually do not have an anatomical distribution and the neurological exam is normal. Patients often lack the characteristic normal concern about the deficit. This characteristic lack of concern has been termed A la belle indifference. Conversion disorder can coexist with depression, anxiety disorders, and schizophrenia. Conversion symptoms often will temporarily remit after the disorder has been suggested.
