What Causes It? Most cases of constipation are caused by changes in diet or physical activity, including not drinking enough fluids. Psychological factors, particularly depression, may cause constipation. Chronic abuse of laxatives can also lead to chronic constipation.     What is constipation?
    Many of us think "constipation" means stools that are not as soft as usual. This isn't quite true: stool consistency changes all the time, depending on what and when we eat and drink and when and how often we have bowel movements. Strictly speaking, "constipation" refers to infrequent or incomplete bowel movements. We also use the term to refer to stools that are hard or difficult to pass. (Bear in mind that soft but bulky stools, like you have after eating high-fiber foods, can also be hard to pass.) diarrhea. On the other hand, there are also many things including certain other chemicals that can, so to speak, push water out of the stool, making the stool harder. Also, if the residue is minimal to begin with, chances are that the resulting stool will be much smaller and harder. Stool frequency, like stool consistency, also changes with what and when we eat and drink. Some people have 3-4 bowel movements each day, while others have one bowel movement every 3-4 days. Every 3-4 days may be perfectly normal for the latter, but awfully uncomfortable for other people. In practice, a constipated patient is one who has fewer or harder bowel movements than usual and who is physically uncomfortable because of it.
    How to treat it (and whether to treat it)
    Treating yourself or your child for constipation just because the bowel movements aren't as frequent as usual is pretty common, but it's also unnecessary and can get you into trouble. With older children and adults, you can prevent a lot of problems by adjusting the diet to increase the amount of residue, and many of the over-the-counter laxatives are nothing more than fiber supplements which produce increased residue.

CONSTIPATION An occasional episode of constipation is not cause for alarm. Stools seem unusually hard. There is unproductive straining. Veterinary assistance may or may not be needed. Here is what you need to know:
  • Straining unproductively can be a symptom of either constipation or large intestinal diarrhea. In either case, small amounts of mucous, gooey, or even blood tinged stool can be passed and there is a lot of pushing involved. Difficulty urinating can also appear as straining. The point is that if all you have noticed is straining, it may not be constipation. Straining to urinate is often an emergency situation so if there is any question about the pet’s ability to urinate, see the vet right away.
    It may be tempting to buy a commercially prepared enema at the drug store and attempt to relieve the pet’s problem at home. Some commercially prepared products are toxic to pets so it is important that human constipation products, be they enemas or laxatives, not be used in pets without specific veterinary instruction.
    One of the purposes of the colon (large intestine) is the storage of stool. Many pet owners become alarmed if the pet has not passed a stool in a few days, especially in a post-anesthetic situation. In fact, the colon can easily store several weeks worth of stool so if only a few days have passed it may be worthwhile to wait a little longer.

Constipation is usually defined as the presence of hard or thick, pasty stools which cause discomfort as they are passed. In severe cases, the child has an inability to pass the hard stool on his/her own. The direct cause of constipation is not enough water in the stool. This occurs because of the diet not having enough water-retaining elements (fiber) or because the stool is kept in the rectum too long, allowing the colon to reabsorb more water than usual. In children with Down syndrome, two factors exist to make constipation more likely: low muscle tone and decreased motor activity. Both of these make the colon more likely to retain stool for longer periods, leading to loss of water from the stool. If left untreated, constipation can lead to (1) rectal fissures: tears in the rectum leading to bright red blood on the surface of the stool and on the toilet paper, but not mixed in with the stool; (2) impaction; (3) stretching of the rectum leading to the loss of the sensation of the need to have a bowel movement. This last problem can further lead to the weakening of the rectal muscles (sphincters) and cause the child to have bowel movement accidents (in medicalese, "encopresis"). The usual treatment of constipation is dietary: increasing fiber, fruits and vegetables. In bottle-fed babies, the introduction of a stool softener is used, such as corn syrup (Karo), malt barley extract (Maltsupex) or lactulose (Diphulac). Lactulose is also used in children and adults. All of these soften the stool by adding water to the stool. Another way of adding dietary fiber is through products such as Metamucil, Citrucel, or fiber wafers. Increasing the amount of fluids the child drinks is helpful; and in older children, decreasing the amount of constipating foods (milk products, bananas, white rice) may also help.

Complaints related to constipation or fecal soiling account for 3% of all outpatient visits and 10 to 25% of visits to gastroenterologists.The normal frequency of stooling ranges widely. Many people do not have a bowel movement on a daily basis and are not constipated. Ninety-five percent of preschool children have bowel movements every other day, every day, or two or three times a day. The average amount of the stool is 5 to 10 mg/kg/day. Transit time increases with age, which means that the frequency of stooling decreases as one gets older. Longer transit times are associated with harder, drier, stools that may lead to bleeding and subsequent withholding of stool.
CLINICAL PRESENTATION Many infants and children who are constipated present with complaints of infrequent stooling. Some parents are concerned that facial grimacing and grunting during defecation are indicative of constipation in infants. Fecal soiling, which represents overflow incontinence, may also occur in some constipated children. About 60% of constipated children experience recurrent abdominal pain. Poor appetite and poor growth may also be noted secondary to maintenance of a sense of fullness from the feces-filled colon. Some children maintain a withholding posture, where they keep their legs clamped together in an effort to suppress the urge to defecate.

LAB TEST AND IMAGING STUDIES If the history suggest hypothyroidism or infantile botulism appropriate studies should be ordered. Anorectal manometry may also be carried out by a gastroenterologists. Similar studies that monitor the peristaltic wave can be conducted to diagnose pseudo-obstruction. GI assessment may include the use of radiographic studies, including flat plates of the abdomen. Diagnostic radiographs using contrast material such as barium enemas help establish the diagnosis of imperforate anus and Hirschsprung’s disease. It is suggested that the barium enema be attempted without prior cleanout to better document the presence of a dilated, stool-filled rectum.

What is constipation? When a person has hard or very infrequent bowel movements, the condition is called constipation . Constipation is a common problem for children and adults with disabilities. A variety of factors, including feeding problems and some medicines, can cause constipation. Why should constipation be prevented? Constipation can cause stomach pain, decreased hunger, and fussiness. Bowel movements can become painful. Pushing to pass a hard stool can cause hemorrhoids and bleeding around the rectum. Pushing can also force bowel tissue out through the rectum. If constipation becomes serious, the stool may become stuck. This blockage may require surgical treatment. Ways to prevent constipation Fluids - Drinking plenty of fluids is the best way to prevent constipation. One way to tell whether your infant is drinking enough fluids is to count the number of wet diapers. If you count fewer than 5 wet diapers each day, your baby needs more fluids. You should also give your child more fluids if the urine is dark yellow or has an odor. Ways to give your child enough fluids include:
