Constipation is a common problem associated with opioid administration. Tolerance to the constipating effects of opioids either does not occur or occurs very slowly during chronic therapy. Constipation may worsen with time because of the disease process (e.g., intestinal obstruction, paralytic ileus due to spinal cord compression, decreased food and fluid intake due to anorexia); therefore a careful search for other causes should be performed (Glare and Lickiss, 1992) . Mild constipation can usually be managed by an increase in fiber consumption and the use of a mild laxative such as milk of magnesia. These cathartic agents should be administered on a regular schedule, barring contraindications. Severe constipation occurs as a result of the inhibition of peristalsis by opioids and can be treated with a stimulating cathartic drug, e.g., bisacodyl, standardized Senna concentrate, or hyperosmotic agents (e.g., lactulose or sorbitol). Oral laxatives can be taken at bedtime, and rectal suppositories can be used in the morning if needed. Stool softeners or emollient laxatives, e.g., docusate, are of limited usefulness because of colonic resorption of water from the forming stool. They should not be used as the sole regimen but may be useful when given in combination with stimulant laxatives to ease defecation, especially in bedridden patients

M ore than 2,500 years ago, Hippocrates noted that "the intestines tend to become sluggish with age." A stooling frequency of fewer than three times per week may still be considered normal if the pattern does not represent a change in the patient's baseline stooling frequency and defecation is not associated with discomfort. Constipation, defined as decreased or difficult evacuation of the feces, has both functional and organic causes. It is a problem that occurs in fewer than 2 percent of persons in the nonelderly population but affects as many as 26 percent of men and 34 percent of women over 65 years of age. At least 75 percent of elderly hospitalized patients and nursing home residents use laxatives for bowel regulation. Complaints related to constipation account for over 2.5 million physician visits every year.

The longer it takes for the stool to move through the large intestine, the more fluid is absorbed and the drier and harder the stool becomes. Delirium is a malfunction of the brain . It has no specific cause. People experiencing delirium may be in and out of consciousness and often have problems with attention, thinking, perception, emotion, memory , muscle control, sleeping, and waking. Delirium often occurs during the last 24 to 48 hours of life.

    • Symptoms are severe and last longer than 3 weeks
      Recent and significant change in bowel habits, for instance, constipation alternates with diarrhea
      Severe pain in the anus during a bowel movement
      Symptoms of other diseases in addition to constipation (For example, tiredness, fatigue, poor tolerance to cold weather may suggest the need to assess your thyroid function for hypothyroidism, an underactive gland.)
      Constipation for 2 weeks or longer with returning abdominal pain, which might be a sign of lead poisoning Rectal bleeding
      Anal pain and hemorrhoids
      Anal fissures or cracks in the mucous lining (severe pain during defecation in the anal area)
      Fecal impaction (immovable intestinal contents) in very young children and in older adults

    Constipation refers to the compactness of the stools and the difficulty passing them, not the frequency of bowel movements. The consistency and number of stools varies according to age and from baby to baby. Newborns have several stools a day that are soft and the consistency of seedy mustard, especially if breastfed. Formula-fed infants usually have fewer, firmer and darker stools. Once solid food enters the diet, the stools become more formed and less frequent, and some babies may have a bowel movement without difficulty only once every three days, but daily is preferable. To tell if your baby is constipated, look for the following signs:
    • In a newborn, firm stools less than once a day with straining and difficulty passing them
    • Dry, hard stools and pain on passing them
    • Hard, pebble-like stools passed by a baby who strains during a bowel movement, drawing her legs up on her abdomen, grunting, and getting red-faced
    • Streaks of blood along the outside of the stool
    • Abdominal discomfort along with hard, infrequent stools

    Constipation may occur because:
    • Too much water is removed by the colon, causing dry or hard stools,
    • Stool moves too slowly through the colon, or
    • The patient is unable to expel stools.
    Normal bowel habits among healthy people vary greatly from three times a day to three times a week. In some individuals, constipation may refer to infrequent bowel movements (less than 3 per week). While troublesome, constipation is usually not serious. It should be treated and corrected to reduce abdominal discomfort and other related symptoms and to prevent the development of complications. Untreated chronic constipation may lead to or aggravate more serious problems such as hemorrhoids or fecal impaction (partial blockage due to hard stool). Measures to Treat Constipation
