What Is Complex Regional Pain Syndrome (CRPS)?

Three years ago, Mr. Gordon stepped "the wrong way" when getting out of bed one morning; the resulting foot pain was excruciating. Since that day, even lightly touching the skin of his foot causes Mr. Gordon pain. Sometimes his foot swells and, when it’s cold outside, the pain worsens. X-rays taken at the time of this mishap did not reveal a fracture. While Mr. Gordon continues to experience pain, his doctors aren’t sure what is causing it. Since all his test results are "normal" and none of the treatments have helped him, his doctors assume that his pain is "psychological" in origin. Complex Regional Pain Syndrome (CRPS), formerly known as Reflex Sympathetic Dystrophy (RSD), is a disease in which the portion of our nervous system that normally conducts pain becomes oversensitive. The result of this "sensitization" is spontaneous pain. This kind of pain persists despite the fact that there is no apparent cause (such as a fractured bone or infection), although the symptoms may emerge following a fall, injury, cut, or burn. Researchers have discovered that microscopic changes in nervous tissue can produce the chronic pain associated with CRPS. Not every case of CRPS is the same and there is no reliable test for diagnosing this syndrome. It is, as doctors often say, a "diagnosis of exclusion" (that is, if all other causes of pain have been ruled out, doctors may attribute the pain to CRPS). Similarly, there is no treatment "recipe" for CRPS. Treatment regimens are usually tailored to the individual’s particular form of pain.

Volunteers needed for complex regional pain syndrome study

Washington University pain researcher Anthony H. Guarino, M.D., instructor of anesthesiology, is joining scientists from around the country to test the drug called CC-5013. "These patients can have very severe pain," Guarino says. "The pain often lingers, and although available drugs can help, some evidence suggests that CC-5013 might have more success regulating this difficult-to-treat pain." The drug also is being tested for treatment of various cancers. It is thought to act by regulating certain chemicals produced in the body that contribute to pain and inflammation following injury. To be eligible for the study, volunteers must be at least 18 years old and have had pain from CRPS for at least one year. Those who qualify will receive a daily dose of either the study medication or an inactive placebo for 12 weeks. After 12 weeks, all study subjects will receive the drug. If the drug is helping to control pain from CRPS, subjects will be able to remain on the drug and in the study for about a year.

Overview The nature of CRPS is puzzling, and the cause isn't clearly understood. Women are more likely to be affected by CRPS than men are. The overriding symptom is extreme pain frequently described as burning. Other symptoms can include sensitivity to touch, skin changes, swelling, weakness, and decreased function of the hand/foot. Introduction:
Complex Regional Pain Syndrome (CRPS) is reported to be one of the most difficult and painful conditions that the podiatrist is likely to treat (Perlman, 1992). It is a syndrome with a primary symptom of constant, burning pain (Wyngarden and Bleyaert, 1991). Other symptoms include hyperesthesia, swelling, hyperhidrosis and trophic changes in the skin and bone of the affected extremity (Schwartzman and McLellan, 1987). There are a number of events that can lead to CRPS with trauma being a significant factor (McNerney, 1991). The resultant pain is usually greater than the initial injury (Perlman, 1992) and there are a number of other aetiological factors associated with the syndrome development.
In 1994 the International Association for the Study of Pain reclassified regional sympathetic dystrophy syndrome (RSDS) as Complex Regional Pain Syndrome Type 1. Causalgia is now referred to as Complex Regional Pain Syndrome Type 2 (Canadian RSD Network, 1998). Back to Contents

Incidence CRPS can affect any one but is most common in females aged 40 to 60 years of age (Johnson, 1989). The ratio of female: male is 3:2 and the average incidence is 1% to 15% of the population (Purdy and Miller, 1992).
