ETIOLOGY and EPIDEMIOLOGY The common cold is caused by a multitude of organisms. 90 percent of the cases of the common cold are due to viruses. Table 21-1 contains a list of the organisms that cause the common cold. The common cold is worldwide in it's distribution and is spread person-to-person. Most commonly the viruses are spread during hand-to-hand contact (shaking hands, etc.). The average child (less than 5 years of age) will get 5-7 colds per year and the average adult will get 1-2 colds per year.

What is the common cold?
The common cold, also known as a viral upper respiratory tract infection, is a contagious illness that can be caused by a number of different types of viruses. Because of the great number of viruses that can cause a cold, the body never builds up resistance against all of them. For this reason, colds are a frequent and recurring problem. In fact, on average, preschool children have 9 colds a year; those in kindergarten, 12 colds a year; and adolescents and adults, 7 colds per year. What are the symptoms of the common cold?
Symptoms of a common cold include nasal stuffiness and drainage, sore throat, hoarseness, cough, and perhaps a fever and headache. Many people with a cold feel tired and achy. These symptoms typically last from 3-10 days. How is the common cold spread?

The normal cell cycle takes approximately 24 hours to complete. The cell cycle is an immensely complicated process of activation and deactivation of numerous enzymes in specific phases of the 24-hour cycle. See the diagram below. This sets off a biochemical chain reaction that eventually dephosphorylates (activates) Cdc2 in the nucleus, thereby allowing cell division to occur. One further comment. It should be obvious that the p53 mutation that exists in most cancers could be overcome by phytochemicals that induce the synthesis of the G1 blocker p21Cip1. This is true. Resveratrol, ginseng, EGCG and a host of other phytochemicals can all induce apoptosis in a wide diversity of cancer cells, including breast and prostate cancer. Preparing these compounds so they can be used therapeutically is another story and a topic for another essay. Stay tuned to this web site for further information. The next scheduled essay will discuss how quercetin, a common flavonoid found in many foods (and sold in semi-purified form in health food stores), can inhibit the activity of the VPR protein (thereby preventing widespread immunosuppression), reduce the activity of HIV reverse transcriptase by 100% (thereby PREVENTING infection of new cells) at concentrations of 1-2 micrograms/ml, and reduce overall HIV synthesis by 80% in cell lines in concentrations as low as 40 microM.

Pleconaril, manufactured by ViroPharma, appears to cut one day off the length of the typical cold, and can lessen the severity of the muscle weakness, nasal congestion, runny nose, cough and sore throat that accompany the illness. The treatment appeared to help Jamie Garris, one of more than 2,000 people who participated in the research. Introduction: Common cold
Common cold: Almost everyone is familiar with the feeling you get when you start a common cold, such as a sore throat, runny nose, sneezing, or tearing. Colds are very common but are more common in children and decline with age. They are rarely dangerous to healthy adults or children, but may occasionally be dangerous to infants, the elderly or other at-risk groups. Correct diagnosis of a cold usually means distinguishing between cold and the more serious flu , which is not always easy, but is important because of newer antiviral medications for the flu. There are also other conditions that start like a cold (e.g. rare but serious whooping cough ) and numerous conditions that have cold-like symptoms or flu-like symptoms . Although there are numerous different subtypes of colds, diagnosis of the specific subtype of cold is rarely performed and not usually important for treatment.

Drug research comes closer to curing the common cold
May 18, 1999 Web posted at: 4:27 p.m. EDT (2027 GMT) From Medical Correspondent Rhonda Rowland (CNN) A study released in this week's Journal of the American Medical Association (JAMA) has encouraging results that may bring medical science one step closer to curing the most infectious disease there is: the common cold. Research on the drug, tremacamra, was conducted at Medical University of South Carolina. Unlike over-the-counter cold remedies, which offer limited relief and side effects such as drowsiness, the study authors found no side effects from tremacamra. Tremacamra comes in the form of a nasal spray or powder. The study of 196 participants found the drug reduced by half all cold symptoms including runny nose, cough, sneezing, nasal stuffiness, headache, sore throat, malaise and chills.

Relenza, a powder spray inhaled through the mouth, has already been proven to reduce the duration of a bout of flu by a day or two. The study released Sunday shows it also cuts the chances of catching the flu from an ill family member by 79 percent. The drug is the first of a new class of anti-viral medicines that are effective against both major strains of the flu, influenza A and B.

    What is the common cold?
    The common cold is one of the most common illnesses, leading to more doctor visits and absences from school and work than any other illness every year. It is estimated that during a one-year period, people in the US will suffer one billion colds. Caused by a virus that inflames the membranes in the lining of the nose and throat, colds can be the result of more than 200 different viruses. However, among all of the cold viruses, the rhinoviruses and the coronaviruses cause the majority of colds.
    When is the "cold" season?
    People are most likely to have colds during fall and winter, starting in late August or early September until March or April. The increased incidence of colds during the cold season may be attributed to the fact that more people are indoors and close to each other. In addition, many cold viruses thrive in low humidity, making the nasal passages drier and more vulnerable to infection.

What causes the common cold?
When someone says, "I have a cold," what he or she means is, "There is something in my body that is causing me to have the set of symptoms that we call a 'cold.'" The set of symptoms normally includes things like a runny nose, sneezing, coughing, "chills" and a headache. It does not include a fever normally, if there is a fever it's called "the flu." There are many different viruses that can cause cold symptoms, but about half of the time a cold is caused by a class of viruses called rhinoviruses The rhinovirus gets into the cells lining your nose and starts reproducing. It arrives from other people it is not cold weather that causes a cold, but the fact that cold weather causes people to congregate together indoors, which makes transmission of the virus easier. The virus generally moves from someone else's hands to your hands (either directly or through some intermediate surface like a door knob), and from your hands into your nose or eyes. What is a cold?
Sneezing is caused by the irritation of the soft lining of the nose, which is characteristic of the common cold. A cold is a contagious viral disease which infects the soft lining (mucous membrane) of the nose. There are more than 100 different viruses which can result in a cold. If you dread red eyes and sniffles, a cure for the common cold is under development. Researchers from Agouron Pharmaceuticals have been working on a remedy for the common cold that fights the virus that causes it. "We hope that it will be a cure for the common cold," said Amy Patick, a biologist with Agouron Pharmaceuticals. At a major infectious disease conference in San Diego last week, the Interscience Conference on Antimicrobial Agents and Chemotherapy, researchers presented their results of testing a new compound, AG70-88.
