The virus then remains in the body forever and repeated outbreaks are triggered by several factors. 4.05 Oral Herpes Simplex (Cold Sore)
Presentation
Patients have swelling, burning or soreness at an intra- or extra-oral lesion consisting of clusters of small vesicles on an erythematous base, which then rupture to produce red irregular ulcerations with swollen borders and possibly crusting or superinfection. These lesions occur on the hard palate or gingiva or, more commonly, at the vermilion border of the lip.
What to do:
  • When there is any doubt of the diagnosis, scrape the base of a vesicle (warn the patient this hurts) smear on a slide, stain with Wright's or Giemsa, and examine for multinucleate giant cells (look for nuclear molding). This is called a Tzanck Prep, and establishes the diagnosis of herpes. Alternatively, this swab can be sent for viral cultures, which may take days to grow.
  • An equal mixture of Kaopectate and Benadryl elixir will coat and dry the area and reduce pain. Topical Orabase, or Xylocaine 2% Viscous Solution will also relieve the pain. Consider oral analgesics for continuous pain relief. Narcotic analgesics and mild sedation may be required to manage the most severe pain.
  • Instruct the patient to keep lesions clean, and avoid touching lesions (so as not to spread the virus to eyes, unaffected skin, and other people).

What are cold sores?
Cold sores are annoying, small, painful blisters on the lips and nearby skin, including in the nose and mouth. They usually appear when you are sick or stressed. They are also called fever blisters.
How do they occur?
Cold sores are caused by the herpes simplex virus. This virus also causes genital herpes. The fluid in the blisters contains live virus. The virus in this fluid can easily be spread from one person to another. The infection can be spread, for example, by kissing, by sharing food or drink, or by not washing your hands after touching the sores. Once you are infected, the virus continues to live in the body, even after the sores are gone. The virus may become active again and cause more cold sores during or after:

What are cold sores?
Cold sores are a cluster of painful 1 to 3 millimeter bumps or blisters on the outer lip. They happen only on one side of the mouth. Just before a cold sore develops, you may feel a tingling or burning on the outer lip at the same place where you had cold sores before.
What is the cause?
Cold sores are caused by the herpes simplex virus. The sores happen for the first time after you have had contact with someone with herpes. Thereafter, the virus lives in the sensory nerve and can cause cold sores again. About 2% of people have recurring cold sores. The sores come back because of sunburn, fever, friction, or physical exhaustion.

Introduction Figure 1 Herpes Simplex Infections There are two major forms of oral herpes simplex viral infections. Primary herpes, also known as acute herpetic gingivostomatitis, represents the initial infection by the virus and usually occurs in childhood. The second form is recurrent herpes simplex, which mainly occurs on the lips and is known as herpes labialis. Oral and perioral herpes infections are most often caused by herpes simplex virus, type I. Herpes simplex virus type II is usually responsible for genital herpes infections. One can acquire herpes simplex virus type II oral infections or herpes simplex type I genital infections through oral-genital contact. Clinically, HSV type I and type II oral lesions cannot be distinguished from each other. Figure 2 The herpes simplex virus causes vesicular eruptions on skin and/or mucous membranes. The virus is usually transmitted by direct contact when an individual has an active lesion or lesions. The viral particles are present in the vesicular fluid. Herpes simplex virus is a member of the Herpesvirus Family. Herpesviruses are composed of a DNA core, protein capsid and lipid envelop (see Figure 2). Other members of herpesvirus family include varicella zoster virus which causes chickenpox and shingles; Epstein-Barr virus which causes infectious mononucleosis and Burkitt's lymphoma; and cytomegalovirus which causes infection in immunosuppressed individuals.

Introduction
The herpes simplex virus (HSV), which causes cold sores (or a cold sore), is a latent disease that never leaves the body. Read on for tips on how to reduce their frequency and healing time! There are currently no known cures to rid the body of HSV.
Remedies
This section has known remedies that may help combat an outbreak.
Ice
HSV requires a specific environment to thrive: warm and wet. By chilling the area several degrees, it may be possible to prevent it from spreading, and even push the virus back into dormancy.
  • Immediately apply ice to the affected area, when first noticed. Hold the ice firmly to the skin for at least fifteen minutes.
  • Dry infected area. Repeat three times at five minute intervals.
  • Diet and Recommendations

Because of the connotations that the name "herpes" implies, it's no wonder that men and women are ashamed and embarrassed when they have a cold sore. IS THERE ANYTHING I CAN DO TO MAKE THE OUTBREAKS STOP COMPLETELY? Unfortunately, no. Herpes is a virus that remains in the body for life. Once the cold sore is healed, the virus retreats back into the nerve cells. Once there, it stays dormant but can "reactivate" at any time and cause a new cold sore to appear. Though the virus that causes cold sores is permanent, there are some things that you can do to decrease your chances of future outbreaks. While we examine those, let's also take a look at the "triggers" that are proven to bring on cold sores. · Sunlight-Many people are very prone to cold sores when they suffer from sunburn or spend too much time outdoors. This is easily remedied. A good sunscreen applied to the lips is your first line of defense against sun-induced cold sores, commonly referred to as fever blisters.

A young friend recently had such an outbreak following a mild over-indulgence over the holidays, i.e., sweets and margaritas, plus a head cold. He had been taking lysine 500 mg for some time. The lysine ran out and the next day the cold sores completely encircled his mouth. He is devastated and embarrassed. He cannot miss work, and to him this is a social catastrophe. What about the Lysine? Does this problem have anything to do with acid-base balance which I hear nothing about these days? A: Cold sores are painful, tiny blisters that appear around the lips. They are caused by the virus herpes simplex type 1. Once you are infected with the virus - and many people are during childhood - the herpes virus stays in the body and can cause a recurrence of cold sores, also called fever blisters. The initial infection causes no symptoms in most people, but about 15% will develop fluid-filled blisters inside and outside the mouth about 3 to 5 days after they are infected with the virus. These may be accompanied by fever, swollen neck glands and general aches. The blisters tend to merge and then collapse. Often a yellowish crust forms over the sores, which usually heal without scarring within 2 weeks.

    Cold Sores and Chicken Pox

    Years ago I suffered frequently from cold sores which were painful and unsightly. Then I stopped having them for many years. Now, recently at age 75 I had a small outbreak. I understand that the virus that causes chickenpox remains in your body and can be reactivated from time to time. Nearly everyone has chickenpox as a child, so wouldn't everyone have this virus? Are cold sores the same herpes virus? How can I get rid of this virus? Is there an antibiotic I can take for this? What causes these outbreaks? Stress? Citrus fruits?
    A: The chicken pox virus is in the same family as the cold sore virus (herpes simplex, but is a different virus (varicella) altogether. Stress, excess alcohol, poor nutrition, lack of sleep, and certain foods that are high in arginine (nuts, chocolate, popcorn) may trigger the virus. Taking 500 mg a day of lysine may prevent outbreaks. Acyclovir, an antiviral medicine, taken as a single 800 mg pill or applied as a cream at the first onset of tingling may prevent their eruption or hasten healing of the sores.
