What are cleft lip and cleft palate? A cleft lip is a separation of the upper lip. The cleft can occur on one or both sides of the lip. A cleft palate is a separation of the top of the mouth. Like cleft lip, cleft palate also can occur on one or both sides of the top of the mouth. Very often, someone who has a cleft lip also will have a cleft palate. What causes cleft lip and cleft palate? Cleft lip/palate is the fourth most common birth defect in the United States. One out of 700 babies is born with a cleft. Currently, it is not completely understood what determines cleft formation. Most information suggests a combination of both genetic/hereditary factors and environmental factors such as illness, alcohol abuse or nutritional deficiency during pregnancy. Regular prenatal care can help reduce the risks of birth defects. How are cleft lip and cleft palate treated? Cleft lip and cleft palate are usually treated surgically. The surgery should take place as a baby. Parents should consult a cleft lip/palate and craniofacial anomalies team. This team of professionals work together to make sure the baby receives the best care possible. Teams can be located by calling the Cleft Palate Foundation at 800-242-5338. What is a cleft lip and what is a cleft palate?
Cleft means 'split' or 'separation'. During early pregnancy separate areas of the face develop individually and then join together. If some parts do not join properly the result is a cleft, the type and severity of which can vary. Cleft Lip
A cleft lip is a condition that creates an opening in the upper lip between the mouth and nose. It looks as though there is a split in the lip. It can range from a slight notch in the coloured portion of the lip to complete separation in one or both sides of the lip extending up and into the nose. A cleft on one side is called a unilateral cleft. If a cleft occurs on both sides it is called a bilateral cleft.
A cleft in the gum may occur in association with a cleft lip. This may range from a small notch in the gum to a complete division of the gum into separate parts.

Cleft Palate Foundation CPF Cleft lip and cleft palate comprise the fourth most common birth defect in the United States. One of every 700 newborns is affected by cleft lip and/or cleft palate. A cleft lip is a separation of the two sides of the lip. The separation often includes the bones of the upper jaw and/or upper gum. A cleft palate is an opening in the roof of the mouth in which the two sides of the palate did not fuse, or join together, as the unborn baby was developing. Cleft lip and cleft palate can occur on one side (unilateral cleft lip and/or palate), or on both sides (bilateral cleft lip and/or palate). Because the lip and the palate develop separately, it is possible for the child to have a cleft lip, a cleft palate, or both cleft lip and cleft palate. Cleft lip and cleft palate are congenital defects, or birth defects, which occur very early in pregnancy. The majority of clefts appear to be due to a combination of genetics and environmental factors. The risks of recurrence of a cleft condition are dependent upon many factors, including the number of affected persons in the family, the closeness of affected relatives, the race and sex of all affected persons, and the severity of the clefts. A child born with a cleft frequently requires several different types of services, e.g., surgery, dental/orthodontic care, and speech therapy, all of which need to be provided in a coordinated manner over a period of years. This coordinated care is provided by interdisciplinary cleft palate/craniofacial teams comprised of professionals from a variety of health care disciplines who work together on the child's total rehabilitation.

Cleft lip can be an isolated event or can be found in conjunction with a cleft palate. It is caused by the failure of the normal structures to fuse together during the formation of the lip. The cleft is an opening in the lip. The skin, muscle, and oral lining tissue is present but has not fused leaving an opening. Cleft lips occur more often on the left side rather than the right side for reasons that we don't understand. More than half of cleft lips occur with a cleft palate where there is a failure of the two sides of the roof of the mouth to fuse in the midline to form a complete palate. Clefts of the lip, incomplete or complete, are associated with some degree of nasal distortion. The children with incomplete cleft lips have a smaller nasal deformity than the children with complete cleft lips. Clefts that involve the lip and the palate are more significant in that cleft palates can affect feeding, weight gain, middle ear function, speech and facial growth. Each of these issues may be noted at different ages, i.e. difficulty feeding and middle ear dysfunction are infant and early childhood issues. Speech difficulties present themselves later around two years of age. Facial growth and dental occlusion are preteen or adolescent issues. A craniofacial team is essential to provide the children and their families the care they need to give the best result.

The Cleft Lip and Palate Center brings together several different specialists to provide comprehensive inpatient and outpatient services at Children's Hospital of Wisconsin. We care for patients with cleft lip and palate disorders from birth into adulthood. Before Birth
Prenatal screening by ultrasound may detect cleft lips with or without cleft palates. In these cases, our team is available for consultation. Often, becoming informed of the issues children with cleft lip and palate disorders face helps prepare parents to care for the special needs their child will have after birth, including breathing and feeding. This early meeting includes a nurse clinician and plastic surgeon. Parent's questions are answered and preparations, when necessary, are begun. Newborn period
Most of the time cleft lips and/or palates are discovered at birth. Parents should remember that clefts do not hurt and their baby does not know he or she has a cleft. The two most important issues are to ensure the baby's airway is open and he or she can breathe and eat easily. In the early weeks of development, long before a child is born, the right and left sides of the lip and the roof of the mouth normally grow together. Occasionally, however, in about one of every 800 babies, those sections don't quite meet. A child born with a separation in the upper lip is said to have a cleft lip. A similar birth defect in the roof of the mouth, or palate, is called a cleft palate. Since the lip and the palate develop separately, it is possible for a child to have a cleft lip, a cleft palate, or variations of both. If your child was born with either or both of these conditions, your doctor will probably recommend surgery to repair it. Medical professionals have made great advances in treating children with clefts and can do a lot to help your child lead a normal, healthy, happy life. This information will give you a basic understanding of the operation when it can help, how it's performed, and what results you can expect. It can't answer all of your questions, since a lot depends on you individual circumstances. Please be sure to ask your surgeon if there is anything you don't understand about the procedure.

    The West of Scotland Cleft Unit consists of a coordinated, multidisciplinary group of professionals working in the field of facial cleft anomalies and based at three centres in Glasgow:

Cosmetic surgery is performed to reshape normal structures of the body in order to improve the patient's appearance and self-esteem. Cosmetic surgery is usually not covered by health insurance because it is elective. Reconstructive surgery is performed on abnormal structures of the body, caused by congenital defects, developmental abnormalities, trauma, infection, tumors or disease. It is generally performed to improve functions, but may also be done to approximate a normal appearance. Reconstructive surgery is generally covered by most health insurance policies, although coverage for specific procedures and levels of coverage may vary greatly. There are a number of "gray areas" in coverage for plastic surgery that sometimes require special consideration by an insurance carrier. These areas usually involve surgical operations which may be reconstructive or cosmetic, depending on each patient's situation. For example, eyelid surgery (blepharoplasty) - a procedure normally performed to achieve cosmetic improvement - may be covered if the eyelids are drooping severely and obscuring a patient's vision.
