A cleft is a birth defect that occurs when the tissues of the lip and/or palate of a fetus do not properly fuse very early in the pregnancy . A cleft lip, sometimes referred to as a harelip, is an elongated opening between the upper lip and the nose. It may involve one or both sides of the lip and may occur with or without a cleft palate. A cleft palate, in which the roof of the mouth abnormally opens into the floor of the nose, may also occur without a cleft lip. Description
One of every 30 babies is born with some type of birth defect. Approximately one in 700 has a cleft, the fourth most common birth defect in the United States (congenital heart defect is the most prevalent). Twice as many boys as girls are afflicted with a cleft lip, both with and without a cleft palate. However, twice as many girls as boys are afflicted with a cleft palate without a cleft lip. Clefting occurs most often in Asians, Latinos, and Native Americans (one of 500 births) and least often in people of African descent (one of 1000 births). Babies born with cleft lips or cleft palates may have trouble breathing, as well as swallowing. They cannot adequately suck, so they cannot nurse and must be fed with a special bottle or a bulbar syringe. Later, when their teeth erupt, the upper and lower teeth and jaws are often misaligned, resulting in difficulty chewing. Thus, clefting leaves children vulnerable to

What is cleft lip and palate?
Cleft lip and palate are incomplete formations of the upper lip and palate (roof of the mouth) leaving a cleft (gap) in the affected area. Clefts occur when parts of the lip or roof of the mouth don't fuse together during pregnancy. Cleft lip affects how the lip works and distorts nearby structures like the nose. Cleft lip can occur alone or with cleft palate. Cleft lip can be unilateral (on one side of the lip) or bilateral (on both sides). Unilateral cleft lip is the mildest cleft form. This form does not affect speech or the development of teeth. In cleft palate, the inside of the nose and mouth are not separated. This leaves an opening in the roof of the mouth. A cleft palate affects speech because the muscles at the back of the palate, which are needed for normal speech, are not formed properly. A submucous cleft palate is a cleft that is covered by the mucous membrane lining the roof of the mouth. This makes the cleft hard to see when looking in the mouth.

Cleft lip and palate affects one baby in about 700 - it is a condition that children are born with. Cleft lips are more common in boys while cleft palates are more common in girls. We do not know what causes cleft lip and palate - in some cases there is a family history, but in most cases the cause is unknown. Your baby's lips began to form at about the 6th week of pregnancy. The palate forms during the seventh to twelfth week. Clefts form because the normal face and mouth structures do not close when they should.
Breastfeeding your cleft lip and palate baby
Babies with clefts usually take a little longer to feed than babies without clefts. If your baby's cleft is not too severe, you may be able to breastfeed. Some parents choose to express breast milk and give it with a bottle. A squeeze bottle and special teat designed for babies with cleft palates are available. It is really up to you to choose which method suits your needs (see Feeding your baby with a cleft lip and/or palate fact sheet).

    These common facial malformations appear in about 5,000 infants born in the United States each year. Approximately half of these infants have both types of cleft, with the remaining cases divided evenly between cleft lip and cleft palate. Cleft lip and palate can be quite disfiguring, in addition to creating a number of potentially serious difficulties with feeding, hearing and speech. Fortunately, however, cleft lip and palate can be surgically repaired with excellent results, although the process is sometimes lengthy and complicated. Sophisticated therapeutic techniques also can help an affected child learn to eat and speak normally. HOW DO CLEFT LIP AND PALATE DEVELOP?
    About the fourth week of gestation, the tissues of the face begin to form. During this time, the primary palate, which eventually forms the upper lip and gums, and the secondary palate, which eventually forms the roof and back of the mouth, start to grow. During the fifth and sixth weeks of gestation, clefts are present in the primary palate on each side of the upper lip; these clefts run into the openings that will become the nostrils. The clefts normally fuse by the seventh week, creating the facial area between the nostrils and mouth. A cleft lip results when these clefts in the primary palate fail to fuse.

    Cleft deformities involving a cleft lip (opening in the lip) and/or cleft palate (opening in the roof of the mouth) occur in about 1 in 700 births throughout the world and are the most common craniofacial birth defect. Although it isn't usually know what causes a child to have a cleft deformity, about one third of cases are in children with a positive family history and other children in the family with a cleft deformity. It is also known to be more common for children with cleft deformities to have a mother who smokes. Cleft deformities are also associated with a number of syndromes, including Pierre Robin and Stickler syndromes and many others.     What is a Cleft Lip and/or a Cleft Palate?
    Before Cleft lip and/or cleft palate are anomalies that occur when a baby's lip and/or palate do not fully form prior to birth. A cleft lip is a separation of the two sides of the upper lip. The separation often includes the gum line and/or bones of the upper jaw, as well as the nose. eats Cleft lip and cleft palate can occur on one side (unilateral cleft lip and/or palate), or on both sides (bilateral cleft lip and/or palate). Because the lip and the palate develop separately, it is possible for the child to have a cleft lip or a cleft palate alone, or both together. These clefts, or separartions, normally are present in early fetal development. The lip usually closes by 5 to 6 weeks after conception, and the palate by 10 weeks. The lip or the lip and palate together fail to close in approximately 1 in every 1,000 babies born. Some ethnic groups (such as eskimos or asians) have a higher number of babies with cleft lip. Males are affected more frequently than females.

    Signs and Symptoms A cleft lip and palate is noted at birth by the presence of a gap or cleft in the upper lip either on one or both sides of the lip together with a cleft in the palate (the roof of the mouth).     Cleft Lip and Palate Program
    One in every 600 to 700 children in Ontario is born with a cleft of the lip and/or palate. This notch or separation in the lip or palate can mar appearance and affect both tooth and bone structure. Because of its effect on physical appearance, it can be stressful for both the child and his or her family. Clefts may be caused by heredity, nutritional deficiencies, infection or various drugs. The problem occurs about the seventh to ninth week of pregnancy, when the unborn baby's face is developing. Through surgery, specialized dental treatment and speech therapy, the problem can be corrected eventually. However, these treatments can be very expensive. To help families of children and young adults who must cope with this expense, the Ministry of Health and Long-Term Care has established the Cleft Lip and Palate Program. Through this program, the ministry will provide 75 per cent of the cost of dental treatment by a certified specialist who is associated with correcting the cleft lip and/or palate. The Program also covers 75 per cent of treatment costs of severe acquired (non-genetic) dental dysfunction for patients up to 22 years of age.
