Definition Cleft lip and palate are congenital abnormalities (present at birth) that affect the upper lip and the hard and soft palate of the mouth. The following is a list of expectations for a child born with a cleft lip and palate. Although each child is unique, this is intended to give a general idea of what to expect:
  • An infant with cleft lip and palate usually must be bottle-fed (with breast milk or formula) using special nipples. Soon after birth and in preparation for surgery, the lip may need to be taped to help reduce the width of the cleft. A hearing test is recommended , and ventilating tubes are usually placed in the ears at the time of lip surgery to reduce the risk of ear infections. Ear infections are more common for a child with a cleft palate. An eye examination is usually recommended because associated eye anomalies are not unusual. Lip surgery will take place around 10 weeks of age (or 10 lbs). Lip surgery takes about 2-3 hours and the infant usually remains in the hospital one night.

Audiology Update
Audiology Update
Emily had a follow-up visit with a local audiologist this past week and the news was good. He said her hearing was perfect, and that her ear canals were three times wider than when we first took her in (about six weeks before she got the tubes put in). At that time he told us that she could not hear well enough to learn to talk. This is a scary thing to hear from an audiologist, but we knew it was only due to fluid and the problem would correct itself when she got the tubes (which it did). Plus the folks at NYU did not want Emily to start with speech until her palate was fixed so she would not develop bad habits. Now she's making many normal sounds and saying a few words ("hi" and "bye" and "mama"). She's doing so well that her speech therapist thinks that she doesn't need therapy anymore! She's going to come in once a month to follow her progress and reevaluate her again as she begins talking more.

Causes, incidence, and risk factors: Cleft lip and palate are facial and oral malformations that may occur separately or together. They may also occur in association with other syndromes or birth defects. The cause of these malformations may be mutant genes or teratogens (an agent that causes abnormalities in a developing fetus, such as certain viruses or chemicals). As well as being disfiguring, these abnormalities can cause feeding difficulties, problems with speech development, and ear infections. Risk factors include a family history of cleft lip or palate and presence of another birth defect. The incidence of cleft lip and palate varies with different races; approximately 1 out of 1,000 Caucasians are affected. The incidence of cleft palate alone is 1 out of 2,500 people.

The Nature and Treatment of Clefts The word "cleft" refers to an opening or space. We know that all clefts are different. They can occur at one or more different places on the face, such as the lips, the palate, or the gum ridge (alveolus). In human development, the right and left sides of the face fuse near the midline. A cleft results when this fusion does not properly occur. Clefts can be of different sizes; some are more severe than others. The Duke University Cleft Lip and Palate Program utilizes the team approach for the care and treatment of cleft conditions. This team consists of a group of professional specialists who will cooperate in the treatment of your child. The treatment of cleft lip and/or cleft palate is so complex that no one specialist can answer all of the questions and handle all of the problems that may arise.
