Conditions Affecting Arteries of the Upper Extremities It is relatively rare for atherosclerosis to affect the upper Extremities and cause symptoms. Even diabetic patients who are frequently affected by circulation disturbances of the legs are rarely troubled by occlusion of the upper extremity arteries. The reason for this sparing of the upper Extremities from arterial problems is not entirely clear but it is a well documented observation. Traumatic disorders of the arteries of the upper Extremities can lead to severe circulation disorders however. I have had occasion to treat two major league baseball pitchers with aneurysms of the upper extremity arteries(1) and a tennis player with dissection (a tear of the inner lining of the artery resulting in occlusion of the vessel) of the major artery in the arm (2). In spite of the relatively infrequent occurrence of arterial circulation disorders of the upper Extremities, the diagnosis must be considered in anyone with upper extremity pain of a chronic nature.

Arteries of the Brain The vertebrobasilar system is also called the "posterior circulation" of the brain. There are three main vessels which provide circulation to the back of the brain two vertebral arteries and one basilar artery. The back of the brain contains structures which are crucial for sustaining life. For instance, the brainstem controls respiration, swallowing, and the level of consciousness. Other structures of the back of the brain are the occipital lobes (the vision areas of the brain) and the cerebellum (which controls motor coordination).

The resulting artery narrowing, called a stenosis, can eventually reduce blood flow Edema Excessive build-up of fluid in the tissues which may result from fluid (plasma) leaking from the blood vessels. Embolism A blood clot that travels from the site where it formed to another location in the body. Sometimes, a piece of atherosclerotic plaque (see atherosclerosis), small pieces of tumor, fat globules, air, or other materials can act in the same manner as a blood clot. Emboli (plural of embolism) can lodge in a blood vessel and block the flow of blood in that location.

Purpose of review: Disorders of the pulmonary circulation might develop as a primary disease process of the pulmonary vascular bed or, more often, as the acute or chronic consequence of pulmonary or cardiac pathologies. For the anaesthesiologist and intensivist it is particularly interesting to gain insight into the regulation of the pulmonary circulation since pulmonary hypertension and concomitant right heart failure contribute to high perioperative mortality rates in patients at risk, especially after cardiac surgery. Therefore, modulation of the pulmonary circulation may be a life-saving therapy in patients suffering from acute or chronic pulmonary circulatory disorders. Furthermore, routinely performed intra-operative interventions such as the use of volatile anaesthetics or cardiopulmonary bypass systems may have relevant side effects on the pulmonary circulation. Recent findings: This review focuses on new insights into the modulation of pulmonary circulation during general anaesthesia with volatile anaesthetics and anaesthesiological management during cardiopulmonary surgery. Recent publications in the field of cardiopulmonary bypass surgery, one-lung ventilation and heart and lung transplantation are discussed. Furthermore, the role of conventional and experimental therapeutic strategies to modulate pulmonary circulation in intensive care medicine is reviewed.

Angina
Angina, or angina pectoris, is a recurring discomfort. It's usually located near the center of the chest. The pain or discomfort occurs when the blood supply to part of the heart muscle doesn't meet the heart's needs. As a result, the heart doesn't get enough oxygen and nutrients.
The discomfort occurs most often during exercise or emotional stress. That's when the heart rate and blood pressure increase, and the heart muscle needs more oxygen.
Anginal pain or discomfort is usually brief, lasting just a few minutes. People describe it as a heaviness, tightness, oppressive pain, burning, pressure or squeezing. Usually it's located behind the breastbone. Sometimes it spreads to the arms, neck or jaws. It may also cause a numbness in the shoulders, arms or wrists.
Angina and Heart Attack
Angina is different from a heart attack. Both relate to the blood flow through the coronary arteries (which bring blood to the heart muscle), but there's a key difference. With angina, the blood flow is reduced, especially when the heart must do more work. This reduced blood flow to the heart muscle is temporary and leads to discomfort in the chest.
