TREATING INOPERABLE CIRCULATION DISORDERS
In November 2001, Paul S. van Bemmelen, MD, PhD, clinical assistant professor of surgery, and fellow members of our vascular surgery division published an article in the Archives of Surgery describing the clinical study of a nonsurgical treatment of inoperable circulation disorders. This novel treatment is good news for patients with poor circulation who are not good surgical candidates. The first author of the report, Dr. van Bemmelen is a pioneer in the use of external intermittent compression devices for treating circulation disorders in patients for whom surgery is not possible. Good news for patients with poor circulation who are not good surgical candidates Dr. van Bemmelen is the inventor of the patented arterial assist device called ArtAssist , which is currently used throughout the world. He has the longest clinical experience with the use of this device for the treatment of rest-pain, necrosis (black discoloration) of parts of the foot, and intermittent claudication (walking difficulty). Typical reasons to use this type of treatment are: advanced age and poor medical condition, multiple previous bypass attempts, lack of veins that could be used to make a new bypass, and lack of distal outflow arteries in the foot to which to connect a bypass.

  • Normally, the heartbeat starts in the right atrium when a special group of cells (called the "pacemaker" of the heart or sinoatrial node) sends an electrical signal. This signal spreads throughout the heart, from the atria to the ventricles, and causes a contraction of the organ. The route of the electrical signal must be followed for the heart to pump properly (www.americanheart.org)
    The heart contracts (beats) as the electrical impulse moves through it. This normally occurs 60 to 100 times a minute. The atria contract a split-second before the ventricles. This lets them empty their blood into the ventricles before the ventricles contract. Under some conditions almost all heart tissue can start a heartbeat. In other words, another part of the heart can become the pacemaker (www.americanheart.org).

Coauthors: G Tenti, S. Sivalogananthan, C. Drapaca, A. Sachs Disorders of the cerebrospinal fluid (CSF) circulation including hydrocepahlus pseudotumour cerebri, and syringomyelia are common. They share impaired CSF absorption, at increased pressure, yet the biomechanical processes that lead to deformation of the brain and spinal cord are poorly understood. Treatments designed to bypass the absorption defect, such as CSF shunts are fraught with complications. On average 50% of patients require as second operation with 2 years, and some require 20 or more surgical procedures. The mortality rate in shunted children is 12% at 10 years. New shunt designs, developed over the last 5 decades, have not improved patient outcomes. Three prospective ranomized clinical trails of shunt valve design, and endoscopic placement of shunt brain catheters failed to show any benefit from the use of new technology and in some cases outcomes were worse. Accurate biomechanical models of the brain and CSF circulation would dramatically improve physicians ability to identify patients who need treatment, to design new and effective shunt designs, and to understand and prevent the comlications of treatment.

Conditions affecting arteries of the upper extremities
It is relatively rare for atherosclerosis to affect the upper extremities and cause symptoms. Even diabetic patients who are frequently affected by circulation disturbances of the legs are rarely troubled by occlusion of the upper extremity arteries. The reason for this sparing of the upper extremities from arterial problems is not entirely clear but it is a well documented observation. Traumatic disorders of the arteries of the upper extremities can lead to severe circulation disorders however. I have had occasion to treat two major league baseball pitchers with aneurysms of the upper extremity arteries(1) and a tennis player with dissection (a tear of the inner lining of the artery resulting in occlusion of the vessel) of the major artery in the arm (2). In spite of the relatively infrequent occurrence of arterial circulation disorders of the upper extremities, the diagnosis must be considered in anyone with upper extremity pain of a chronic nature.

Prominent Vascular Surgeon Joins UC Irvine College of Medicine and UCI HealthSystem Irvine, Calif., Oct. 19, 1998 Dr. Peter F. Lawrence, a prominent vascular surgeon, has been named associate dean for program development, a new position, at UC Irvine's College of Medicine. He also was named vice president of UCI HealthSystem. Lawrence will be responsible for creating new programs for the UCI Medical Center that bring together physicians and researchers from a variety of disciplines to treat diseases such as cardiovascular disorders, diabetes and neurological disease. Usually reserved for seriously ill, hospitalized patients, this collaborative approach to providing patient care allows advances in medical research to be integrated into programs that prevent and treat disease. Lawrence also will help train medical students and residents in vascular surgery, a discipline that specializes in blood vessels throughout the body. A practicing surgeon, Lawrence will diagnose and treat patients with circulation disorders in arteries and veins. His research interests include: new ways of detecting the formation of fatty atherosclerosis plaques in the carotid artery leading to the brain, the epidemiology of circulatory disorders like aneurysms (dangerous ballooning of artery walls), and more effective techniques for attaching blood vessels in bypass operations in order to improve circulation. He is the author of more than 80 research publications and 25 textbook chapters on vascular disorders, and is an editor of the Journal of Vascular Surgery.

Clinical trials underway for HGF genetic medicine in the United States AnGes MG Group's U.S. subsidiary is now going ahead with the clinical trials of HGF genetic medicine for peripheral arterial diseases (arteriosclerosis obliterans). As the Group has already obtained the U.S. FDA's approval for its IND (Investigational New Drug) application trials at the beginning of this year, the subsidiary is already proceeding with the Phase II of clinical trials for which the first round of administration has been implemented. Doing that, our Group became the first Japanese corporation to conduct clinical trials for genetic medicine in the United States.
The HGF genetic medicine regenerates the blood vessels to improve the condition of patients with clogged capillaries due to arteriosclerosis or similar blood circulation disorders. Application of the medicine, currently under development, is principally different from all conventional drugs, as it enables effective treatment in cases when general pharmacological therapy is insufficient, and where surgery may not improve a patient's condition. We are developing medication mainly to treat PAD patients with progressing blood circulation disorders of the lower extremities (arteriosclerosis obliterans, Buerger's disease), as well as those with progressing ischemic heart disease affecting the blood circulation in the heart (angina pectoris, myocardial infarction).
