    COPD Overview
    Chronic obstructive pulmonary disease, or COPD, is a long-lasting obstruction of the airways that occurs with chronic bronchitis, emphysema, or both. This obstruction of airflow is progressive in that it happens over time.
    Chronic bronchitis is defined as a chronic (ongoing, long-term) cough not caused by another condition that produces sputum (mucus) for 3 or more months during each of the 2 consecutive years.
    In chronic bronchitis, the mucous glands in the lungs become larger. The airways become inflamed, and the bronchial walls thicken. These changes and the loss of supporting alveolar (air space) attachments limit airflow by allowing the airway walls to deform and narrow the airway lumen (the inside of the airway tube). Emphysema is an abnormal, permanent enlargement of the air spaces (alveoli) located at the end of the breathing passages of the lungs (terminal bronchioles). Emphysema also destroys the walls of these air spaces.
    There are 3 types of emphysema: centriacinar emphysema, panacinar emphysema, and distal acinar emphysema or paraseptal emphysema.

    The Burden of COPD
    Chronic Obstructive Pulmonary Disease (COPD), which encompasses both chronic bronchitis and emphysema, is one of the commonest respiratory conditions of adults in the the developed world. Chronic Obstructive Pulmonary Disease poses an enormous burden to society both in terms of direct cost to healthcare services and indirect costs to society through loss of productivity. The exact prevalence of COPD is difficult to determine because of problems with definition and coding. Sometimes it is difficult to differentiate between COPD and chronic severe asthma, and patients with mild to moderate disease may not be identified as suffering from COPD. In the western world, COPD is probably the fourth commonest cause of death in middle aged to elderly men after ischaemic heart disease, lung cancer and cerebrovascular disease. In the UK, it is estimated that 18% of males and 14% of females aged 40-68 years may have developed features of COPD, and in the USA, 13.6% of males and 11.8% of females aged 65-74 years are thought to have COPD. According to statistics produced by the American Lung Association, 15 million Americans suffer from COPD and it claimed the lives of 87,000 Americans in 1992. In the UK, respiratory conditions are the third commonest cause of chronic sickness in working aged people age 45-64 years, is the commonest cause of respiratory related death (fig.1), and COPD accounts for 56% of days of certified incapacity due to respiratory conditions in males. In 1989, an estimated $7 billion was spent on provision of care for patients with COPD in the USA, with a further cost of $8 billion due to lost productivity.

Emphysema and chronic bronchitis are the most common forms of COPD. Causes, incidence, and risk factors Return to top The leading cause of COPD is smoking, which can lead to the two most common forms of this disease, emphysema and chronic bronchitis. Prolonged tobacco use causes lung inflammation and variable degrees of air sac (alveoli) destruction. This leads to inflamed and narrowed airways (chronic bronchitis) or permanently enlarged air sacs of the lung with reduced lung elasticity (emphysema). Between 15% and 20% of long-term smokers will develop COPD. Other risk factors for COPD are passive smoking (exposure of non-smokers to cigarette smoke from others), male gender, and working in a polluted environment. Rarely, an enzyme deficiency called alpha-1 anti-trypsin deficiency can cause emphysema in non-smokers.

Chronic Lung Diseases, such as Chronic Obstructive Pulmonary Disease (COPD), can be a stressful life event. Apart from the physical symptoms, a patient may feel depressed, anxious, worried, irritable, isolated and confused. Patient support is all about helping to manage and control symptoms, overcome negative feelings and build confidence again. Patient Support Groups can help people find enjoyment in a renewed quality of life through information, friendship, encouragement and shared experiences. Extending beyond individuals, it is often families and carers who need support as well.     Obwohl allgemein bekannt ist, dass Zigaretten rauchen für Herz- und Gefässerkrankungen den wichtigsten vermeidbaren Risikofaktor darstellt, wissen viele Leute nicht, dass Rauchen für die Lunge schädlich ist. Sie kennen zwar ihren Blutdruck und das Cholesterin, nicht aber ihre Lungenwerte. Auch in der Schweiz leiden rund 5 % der Erwachsenen unter einer COPD. Aufgrund der Rauchgewohnheit von Jugendlichen muss man davon ausgehen, dass die Häufigkeit dieser Erkrankung in den nächsten Jahren bei uns nicht ab- sondern zunehmen wird.
    Die kürzlich in den USA gestartete GOLD Initiative (global initiative for chronic obstructive lung disease) bezweckt das Bewusstsein für diese schlimme Erkrankung mehr ins Bewusstsein von Grundversorgern und Patienten zu bringen. In der Schweiz besteht diesbezüglich ein Nachholbedarf!
    Ziel des vor 3 Jahren gegründeten Advisory Board COPD (pdf File 135 KB) ist es, Grundversorger und Raucher für dieses Krankheitsbild COPD zu sensibilisieren.
