It is associated with irritation or wear on the underside of the kneecap, or patella. What is going on in the body? Normally, the cartilage on the underside of the kneecap glides smoothly over the surface of the thighbone, or femur. The cartilage may become soft or rough due to the normal aging process, overuse, or injury. What are the causes and risks of the condition? Possible causes of CMP include:

    Alternate Names : Chondromalacia Patellae (CMP) Attribution Runner's knee is a condition causing pain in the front of the knee. It is often due to excess wear on the underside of the kneecap. It is also called chondromalacia patellae, or CMP. What is going on in the body? When the knee is bent and straightened, the kneecap glides back and forth over the end of the femur, or thighbone. The underside of the kneecap is in contact with the femur. Like the rest of the knee joint, this surface is covered with cartilage. Too much stress between the kneecap and the femur can damage the cartilage. The joint surface becomes softened and roughened. This wear causes joint irritation and pain. Sometimes the joint swells because it produces extra fluid to improve lubrication. The contact pressure between the kneecap and femur varies according to activity. Deep knee bends exert a force equivalent to five times the body weight. Climbing stairs is equivalent to three times the body weight.

    Patellofemoral syndrome
    Patellofemoral syndrome, sometimes called chondromalacia, causes pain in the knee. It is caused by overuse of the knee. It is a common problem of active young adults and teenagers, especially girls. Softening of the cartilage under the kneecap causes the pain. This can happen from overuse, injury, or weak muscles. If the kneecap does not move smoothly over the lower end of the thigh bone as the knee is bent or straightened, it can cause the cartilage to wear away. Symptoms begin with a dull pain that starts slowly in the front of the knee. There may be a grating or grinding feeling when the leg is straightened. There is usually some tenderness. When the knee is not being used, there may not be any pain. The pain will be worse when using stairs or when standing after sitting. A healthcare provider should see anyone with knee pain to make sure what the cause is. Treatment for this problem starts with rest of the knee. This means no sports activity for several weeks until the pain is better. Non-steroidal anti-inflammatory drugs (NSAIDs) may be used for the pain. Aspirin or medicines that have aspirin in them should not be given to anyone younger than 18 years of age. Taking aspirin is a risk for a serious illness called Reye's syndrome. Non-steroidal anti-inflammatory drugs (NSAIDs) should be taken with food or milk to prevent stomach upset. NSAIDs should be avoided by anyone who has been diagnosed with asthma, ulcer disease, or a bleeding disorder. Taking NSAIDs increases the risk of bleeding with trauma, dental work, surgery, or if taking blood thinners (anticoagulants).

    It is easy to twist a knee, bang a knee, land the wrong way, there is a lot of reasons, arthritis comes up, those are some of the reasons. Also a lot of Chondromalacia patella, that is where the knee bone kind of cracks a little bit against the femur, the thigh bone. Chondromalacia patella. Malacia means break down, condro means cartilage , patella means the back of the knee bone, which is call the patella, (and there) is the thickest cartilage in the body. It starts rubbing the wrong way against the tract that it slides in and with repetitive use like running or stair climbing, things of that nature, any sport really, the repetitive use just wears out the back of that knee cap and begins the arthritis process and pain. This is also one of the easiest things to heal with Prolotherapy . It is usually just a couple of injections with sugar water and it starts a little inflammatory cycle and builds up collagen there.

    Chondromalacia Chondromalacia is a diagnosis referring to damage, and subsequent pain, to the cartilage under your kneecap. It can be caused by several factors, and produces discomfort with such activities as walking up and down stairs, kneeling, squatting, or getting up from a seated position. It can occasionally produce swelling in the knee and a sensation of giving way or "catching" ("locking"). It is a very common problem and is directly related to the amount of pressure between the kneecap and the femur bone beneath it. This pressure is normally 2-3 times your body weight when simply descending stairs; one can imagine the magnitude of the pressure when running! All the following suggestions and treatments are directed at decreasing the pressure between the kneecap and the underlying femur. The usual precipitating cause of pain in the patient with chondromalacia is either trauma (an injury such as a fall on the knee or a car accident) or a developmental abnormality (Malalignment) of the knee, which may predispose a patient to knee cap pain. Some patients suffer from a kneecap that repeatedly dislocates (usually towards the outside of the knee), which not only causes pain, but buckling of the leg and damage to the undersurface of the kneecap. There is a "normal" relationship between the thigh muscle, the patella, and the point of attachment of the patellar tendon. This is described by the Quadriceps Angle (Q-angle). In those people in which the Q-angle is larger than normal, there is a greater tendency for the kneecap to track abnormally in the groove on the front of the femur with knee bending, leading to increased pressure in certain parts of the kneecap-femur joint.

The kneecap has to be able to move freely up and down when we bend or straighten our knee. If there has been an injury or overuse of the knee, the structures that allow it to move freely can wear down. Over time, the undersurface of the kneecap becomes worn out and inflamed. SIGNS AND SYMPTOMS THAT SHOULD BE EVALUATED BY A HEALTHCARE PROVIDER INCLUDE: * knee pain that gets worse when kneeling or walking up and down stairs and * knees ache if sitting in one position too long, like at a movie, or on a plane. The best TREATMENT is to rest the knee. A brace or tape may be needed to allow the knee to rest. Special exercises may also be taught to strengthen the knee and correct abnormal movement. OTHER HOMECARE MEASURES THAT CAN HELP ARE TO: * avoid activities that require deep knee bends * wear shoes that support the arch * strengthen the upper leg muscles (hamstrings and quadriceps) that support the knee * use ice to help with pain and swelling and * take anti-inflammatory medication like aspirin or ibuprofen MEDICATION CAUTIONS: Do NOT give aspirin (acetylsalicylic acid) or aspirin-containing products to anyone age 18 years or younger because of the risk of a serious illness called Reye’s syndrome.

    Pathology
    First used by Aleman in 1917, chondromalacia patella was the name given to a degeneration of the patellar articular cartilage (Kipnis and Scuderi, 1995). True chondromalacia patella involves the degeneration of the articular facets of the patella, with resulting ragged fronded edges, (Welsh and Hutton, 1990), and is a very common injury amongst both men and women of our sporting population (Williams, 1990).
    Along with osteoarthritis, chondromalacia patella is a type of cartilage damage resulting in anterior knee pain. It is also named as a cause of the symptoms of patellofemoral pain syndrome (Reid, 1992). When compared to the number of patients who are seen with patellofemoral pain, and those with actual changes to the articular cartilage of the patella, chondromalacia patella is a rare condition though (Nofthall, 1990).

Pain develops when your kneecap doesn't track properly in the groove at the end of your thigh bone (femur) or when there is a muscle imbalance. As a result, the cartilage underneath your patella (kneecap) becomes inflamed, eventually softens, and can then deteriorate and progress to arthritis. This is a very common injury that accounts for many of the overuse injuries treated in sports clinics. Signs and Symptoms If you have runner's knee, you'll usually feel pain in the front of the knee and hear cracking and popping noises beneath or on both sides of the kneecap. Symptoms are most prominent when you kneel down, go down stairs, do squats, or do activities that put force against your knee. Stiffness may occur simply from prolonged sitting (sometimes called "moviegoer's knee"). Pain usually increases gradually over a period of time, often a year or longer, until it becomes severe enough that you need to seek medical attention.

    Chondromalacia is probably the single most common condition suffered by our kneecaps, sometimes causing pain and sometimes not. "Creaky" kneecaps are generally afflicted with chondromalacia, but medical studies have shown that people with such noisy kneecaps rarely notice anything abnormal about them until they actually become painful. Congenital (inborn) problems with a person's knees such as patellar malalignment (a natural imbalance of patellar posture) can cause chondromalacia by early adulthood. Malalignment affects patellar movement patterns and causes uneven stress to be placed on the patellar joint surface, perhaps similar to the abnormal pressure and friction that produces the uneven wear seen on automobile tires that are out of alignment. Over time, this may lead to advanced patellofemoral arthritis, occasionally occurring as early as the fourth decade of life. Chronic patellar overloading caused by obesity and/or many years of repetitive knee bending/squatting also seems to be associated with premature patellar cartilage breakdown, even in knees with normal patellar alignment. Chondromalacia patella (which can be considered an early stage of localized, knee joint arthritis) will sometimes remain clinically silent (cause no symptoms) for many years, whereas in other cases it becomes annoying and obvious early on. Patellar pain, combined with crepitation (a sensation of grinding, catching or creaking as one moves the knee), is typical. Sometimes inflammation causes fluid and swelling to develop in the knee. Activities such as stair climbing, squatting and kneeling often become uncomfortable, and in rare cases, almost impossible.

    Common Injuries Chondromalacia: A softening or wearing away and cracking of the cartilage under the kneecap, resulting in pain and inflammation. Symptoms: Pain beneath or on the sides of the kneecap. Pain can worsen over a year or so and can be more severe after running hills. Swelling may also present. In severe cases, you can feeland actually heargrinding as the rough cartilage rubs against cartilage when the knee or quadricep is flexed. Causes: Excessive pronation (when the arch collapses too much and the foot
    rotates too far inward) can cause the kneecap to twist sideways. Fatigued or
    weak quadriceps muscles, which aid in proper tracking of the kneecap, can
    prevent the kneecap from tracking smoothly. A muscle imbalance between weak quads and tighter hamstrings can also pull the kneecap out of its groove. Treatment: Decrease running considerably. Ice the knee for 15 minutes two or three times a day. Once the pain and swelling are gone, strengthen quadriceps by doing "step-ups". Stand on a step or box at least 4 inches high. Keep your involved quadriceps tight while you lower the opposite leg slowly toward the floor. Then raise it back up to the box, and relax. Complete 3 sets of 10 repetitions for each leg. Continue increasing repetitions as strength increases. If chondromalacia isn't responding to self-treatment after
    four weeks, see an orthopedic physician.

    by Stephen M. Pribut, DPM Description: The knee is a complex joint. It includes the articulation between the tibia and femur (leg and thigh) and the patella (knee cap). The most common knee problems in running relate to what is called the "patellofemoral complex". This consists of the quadriceps, knee cap and patellar tendon. What is now called patellofemoral pain syndrome (PFPS) is also known as runner's knee. For many years runner's knee was considered to be chondromalacia of the patella. This essentially means a softening of the cartilage of the knee cap. ...certain mechanical conditions may predispose you to a mistracking knee cap. pain in this area.
    Anatomy Causes:
    • Wide Hips (female runners) Knock Knees (Genu Valgum) Subluxating Patella Patella Alta (high patella) Small medial pole of patella or corresponding portion of femur Weak Vastus Medialis Weak Quadriceps Muscles Tight Hamstrings or calf muscles Pronation of the feet
