Knee Problem: Chondromalacia Patella
This disease is also known as softening of the cartilage of your knee cap or "anterior knee pain". This is a big medical word for a condition where the shiny cartilage surface of your patella (knee cap) is softened due to many factors including abnormal pressure across the joint surface or hormonal changes in your body. This pain is usually located over the front of your knee and is described as a deep aching pain. It is sometimes associated with swelling and is usually worse when your knee is bent for long periods of time such sitting in a car or bus. The pain is also worse with such activities as squatting, kneeling or stair climbing (either up or down stairs). The pain is sometimes improved with rest and the use of pain medication. It is sometimes associated with mild or moderate swelling of the knee and some people report a grinding feeling in their kneecap. It is more common in younger females especially after a growth spurt where the knee must carry more weight.

I've been suffering with pain in the front of my knee that my doctor has diagnosed as chondromalacia. I'm in my early 20s and thought that only teenagers got this. Is this true? Pete

Teenagers more commonly develop this condition, but young adults of your age can suffer with it too. Often, doctors will call it by its other names of chondromalacia patella and patellofemoral pain syndrome. In chondromalacia, the cartilage that lines the back of the kneecap becomes damaged. For teenagers, it's often the extra pressure that the knees have to cope with during the growth spurt that sets the problem in motion. For adults and teenagers, strenuous exercise can be the trigger for the damage to start, as can repeated injuries to the knee. Why the condition occurs at all though isn't really understood. The knee, since it's usually only one knee that's affected, feels stiff after sitting down and it may click when it's moved. Any action where the knee is bent and straightened, running or going up and down the stairs, for example, will bring the pain on. Over time, the condition tends to improve so that it doesn't cause any further problems. Physiotherapy helps to strengthen the supporting muscles and ligaments around the knee joint and prevents future recurrences. When the knee is uncomfortable the use of

Chondromalacia of the patella
Chondromalacia of the patella is the grating or grinding sensation during extension of the knee. The cause is thought to be related to overuse, trauma and/or abnormal forces on the knee, such as a mildly abnormal alignment of the patella (knee cap) and femur. Symptoms include knee tenderness, pain in the knee after sitting for a prolonged period of time, knee pain that is worse with stairs or getting out of a chair, and a grating sensation in the knee.

Chondromalacia Patellae (also known as CMP Patello-Femoral Pain Syndrome , or Runner's Knee ) is a discomfort or dull pain in the knee or just under or around the knee. It is most common in young adults, soccer players, cyclists rowers , and runners . Causes

Treatments

Go figure, but there seems to be a fixation with the idea that we are literally running our knees into the ground with every step we take. Turns out that knee injuries are not inevitable and, though common, are easily corrected if properly managed. Knee pain need not deter you from your ideal running program.
Runner's Knee
(Chondromalacia of the patella) Description: Pain around and sometimes behind the kneecap. One of the most common injuries among runners, runner's knee most often strikes as runners approach forty miles per week for the first time. Even after taking a few days off, the pain seems to come right back, sometimes even intensifying, after the first few miles of the next run. The pain often feels worst when running downhill or walking down stairs, and the knee is often stiff and sore after sitting down for long periods. You might hear a crunching or clicking sound when you bend or extend your knee. The sure-fire test for runner's knee: sit down and put your leg out on a chair so that it's stretched out straight. Have a friend squeeze your leg just above the knee while pushing on the kneecap. She should push from the outside of the leg toward the center. At the same time, tighten your thigh muscle. If this is painful, you're looking at runner's knee.

Cool Runner posted Aug-05-2005 06:20 PM I tried this at the med tent, but its a slow board and got no response.... Just got back from the doc after having xrays and MRI's of the knee done and he diagnosed me with having chondromalacia, which apparently is the beginning stages of arthritis. Naturally, he said running is out now. Any successful runners with this condition and what do you do to keep the pain/condition from flaring up? Thanks in advance! IP: Logged jansd
Cool Runner posted Aug-05-2005 06:57 PM I have never heard that chrondromalacia was the beginning of arthritis. I think it is often a catch-all term for when doctors can't find anything really wrong with your knee. I was told I had chrondromalacia when I was in college, back in the 1970s. Well, i've been running since then, and I don't have any problems with my knees. At the time, I was not told to curtail any exercise. Instead, my doctor put me on a program to strengthen the muscles that support my knees. I guess it worked. If I were you, I'd get a second opinion or at least do an internet search on chrondromalacia. IP: Logged Run Page Run

Chondromalacia is a condition wherein the cartilage on the back of the patella (knee cap) is irritated and painful because it rubs against the medial femoral condyle. What are the common symptoms/complaints? Patients complain of pain felt under the knee cap on the medial side of the knee particularly while going up or down stairs and hills. Also pain and stiffness after prolonged sitting. How is it caused? During a normal gait cycle, the femur and the tibia rotate in unison (i.e. internally during pronation and externally during supination). However, Chondromalacia occurs when a person over-pronates because the tibia is locked into the Talus and therefore continues to rotate internally past the end of the contact phase (where supination starts at the beginning of midstance in a normal gait) while the femur receives its orders from the brain and begins to rotate externally at midstance. The resulting counter rotation of the femur and the tibia causes the patella to rub against the medial femoral condyle instead of moving smoothly up and down in its normal track, which causes the pain felt by the patient and the damage to the cartilage. (Chondromalacia Patella)
Runners Knee refers to pain primarily on the inside of the knee, or beneath the kneecap can also occur. It is more commonly seen in runners as opposed to walkers due to high level of activity, particularly mileage. The key element in this injury is due to overpronation resulting in poor tracking of the patella causing a repeated stress of the bone moving abnormally from side to side over the groove of the thigh bone (femur), gradually softening the cartilage under the kneecap. The result is the surface of the joint can become rough. Pain may be just a prelude to further destruction of the surface (traumatic arthritis). This injury can be confused with " Patella tendonitis " which is usually pain beneath the Chondromalacia Patella bone. In either case it is the "tracking" of either the bone or tendon which needs to be addressed.
