Dear Alice, I suffered from chondromalacia for years. Arthroscopic surgery a few years ago helped, but what did the trick was developing the muscles that hold my knee in place and allow them to track properly. Spinning classes 2 - 3x/week for the last two years *really* helped with this. I am also sure to stretch it well, including the ITB. Anyone with chondromalacia should see a good sports- and fitness-focused physical therapist, and with their blessing, try spinning! [back to top]

What is chondromalacia Patella?
The term chondromalacia patella refers specifically to a pathological condition resulting in structural changes in the cartilage surface of the kneecap. These changes may be as mild as softening and discoloration of the normally firm, white glistening surface. if the process continues, fissuring and fraying may occur with the more advanced stages resulting in full thickness cartilage loss with exposure of the underlying bone. The process may be localized to one small area or it may involve the entire surface of the patella. Because of these variations in the cartilage changes, the symptoms experienced by patients with chondromalacia may vary.

    Author: William C. Shiel Jr., MD, FACP, FACR What is the chondromalacia patella? The most common cause of chronic knee pain is the chondromalacia patella. Also informally called "housemaid's knee" or "secretary's knee" and medically termed chondromalacia patella, this condition is named the chondromalacia patella because the pain is related to how the kneecap (the patella) slides over the lower end of the thigh bone (the femur). What are the symptoms of chondromalacia patella? The symptoms of chondromalacia patella are generally a vague discomfort of the inner knee area, aggravated by activity (running, jumping, climbing or descending stairs) or by prolonged sitting with knees in a moderately bent position (the so called "theater sign" of pain upon arising from a desk or theater seat). Some patients may also have a vague sense of "tightness" or "fullness" in the knee area. Occasionally, if chronic symptoms are ignored, the associated loss of quadriceps (thigh) muscle strength may cause the leg to "give out." Besides an obvious reduction in quadriceps muscle mass, mild swelling of the knee area may occur.

  • Back to Encyclopedia Index Overview First used by Aleman in 1917, chondromalacia patella was the name given to a degeneration of the patellar articular cartilage (Kipnis and Scuderi, 1995). True chondromalacia patella involves the degeneration of the articular facets of the patella, with resulting ragged fronded edges, (Welsh and Hutton, 1990), and is a very common injury amongst both men and women of our sporting population (Williams, 1990). Along with osteoarthritis, chondromalacia patella is a type of cartilage damage resulting in anterior knee pain. It is also named as a cause of the symptoms of patellofemoral pain syndrome (Reid, 1992). When compared to the number of patients who are seen with patellofemoral pain, and those with actual changes to the articular cartilage of the patella, chondromalacia patella is a rare condition though (Nofthall, 1990). The knee is involved in around 10 % of all sporting injuries, (Turner and Avil, 1995). While treatment of knee complaints is not in the realm of podiatry practice, knee pathology can exert an influence on the overall lower limbâs functioning (Turner and Avil, 1995). This may result in biomechanical compensatory mechanisms in the foot, where podiatry can have a positive effect on such a scenario.

    Why? Ridiculous? Of course. The two doctors have used the word chondromalacia to mean two different conditions. Had both her doctors used better, more specific terms M.J. would not have to fight her insurance company.
    Investigators in the early 20th century examined the kneecaps of cadavers and noted that the cartilage was soft. In some cases, the cartilage appeared frayed and/or ulcerated. They made the assumption that these findings in cadavers could explain knee pain in the living.     What is the patella? The patella is the kneecap bone. It lies within the quadriceps tendon. This large tendon from the powerful thigh muscles (quadriceps) wraps round the patella and inserts into the top of the lower leg bone (tibia). The quadriceps muscles straighten the leg. The patella acts like a fulcrum to increase the force of the quadriceps muscle. The back of the patella is covered with smooth cartilage. This helps the patella to glide over the lower part of the thigh bone (femur) when you straighten your leg. What is chondromalacia? Chondromalacia (sometimes called chondromalacia patellae) is damage to the patella cartilage. It is like a 'softening' or 'wear and tear' of the cartilage. The roughening or damage can range from slight to severe. What causes chondromalacia? Chondromalacia occurs most often in young adults and teenagers. It is more common in women. The reason why damage occurs to the cartilage is not clear. It is thought that the patella may rub against the lower thigh bone instead of gliding smoothly over it. This may damage the patella cartilage. Situations where this is more likely include the following.
    • Overuse of the knee, such as in certain sports.

Over half are bilateral Aetiology In the older age-group the articular cartilage breaks down as part of the natural aging process leading to osteoarthritis in the knee. In teenagers the articular cartilage "softens" from excessive and uneven pressure on it as a result of structural changes with rapid growth, and muscle imbalance around the knee. During periods of rapid growth any valgus deformity of the knee is accentuated, increasing the Q-angle . Excessive pressure is placed on the lateral side of the patella. Often the vastus lateralis tends to be more powerful than the vastus medialis, encouraging the patella to track or dislocate laterally. The uneven and excessive pressure on the patella leads to cartilage "softening" and breakdown.
