A Fact Sheet for Physicians

Depressive disorders, which include major depressive disorder (unipolar depression), dysthymic disorder (chronic, mild depression), and bipolar disorder (manic-depression), can have far reaching effects on the functioning and adjustment of young people. Among both children and adolescents, depressive disorders confer an increased risk for illness and interpersonal and psychosocial difficulties that persist long after the depressive episode is resolved; in adolescents there is also an increased risk for substance abuse and suicidal behavior . Unfortunately, these disorders often go unrecognized by families and physicians alike. Signs of depressive disorders in young people often are viewed as normal mood swings typical of a particular developmental stage. In addition, health care professionals may be reluctant to prematurely "label" a young person with a mental illness diagnosis. Yet early diagnosis and treatment of depressive disorders are critical to healthy emotional, social, and behavioral development.
Although the scientific literature on treatment of children and adolescents with depression is far less extensive than that concerning adults, a number of studies-mostly conducted in the last four to five years-have confirmed the short-term efficacy and safety of treatments for depression in youth. Larger treatment trials are needed to determine which treatments work best for which youngsters, and studies are also needed, however, on how to best incorporate these treatments into primary care practice.

While it is fairly well known to the general public that clinical depression is common (sometimes referred to as "the common cold of mental illness"), affecting 10 to 15 % of the adult population at some point in life, it is not commonly known that depression in childhood is also a major health problem. In fact, depression in children is arguably more significant of an issue that is adult depression. For example, about 5% of children at any given time suffer from clinical depression; this naturally occurs during critical phases in child development, and not only can show itself with behaviors and feelings not commonly viewed as part of a "depression," but can interfere with the normal developmental processes of childhood.

    Depression is dangerous. It distorts normal perceptions, causes behavior changes and can result in self-mutilation or suicide. It is not uncommon in children, but, for many reasons, it often escapes diagnosis. Depression was not even recognized as a childhood disease by the psychiatric profession until 1980. Before that time, it was commonly believed that children could not suffer from depression. What, after all, could a child in a healthy family and community environment be depressed about? DSM-IV , the “Bible” of the psychiatric profession, which identifies symptoms of mental illnesses, recognizes that children can be depressed. Nevertheless, depression is frequently overlooked as a diagnosis by the health care profession. Among possible reasons are these: The historic denia l that depression exists in children.

    A once bubbling, engaging child becomes sullen and with-drawn, or irritable and a bully on the playground. School tardiness or absence becomes frequent, and grades drop. A parent cannot seem to “get through” to a child to discuss what’s wrong, and tension in the family rises. Does my child have a mental disorder? The question is difficult, even frightening, for a parent to voice. Understandably, it is easier to overlook or explain away subtle signs of illness that may occur periodically at worst and are set against the rapid changes of childhood or the turmoil of adolescence. “It’s just a phase.” “He’ll grow out of it.” “She’s under a lot of stress.” “We need to assure him that we love him.” “No one in our family has a mental illness.” Yet the concern that sparks a parent’s question may be justified. One in five American children and adolescents has a mental or behavioral disorder that interferes with their ability to learn in school or to establish healthy relationships with family members and friends. For one in 10 youngsters, a mental disorder will lead to moderate to severe impairment in one or more facets of their life.

As many as 8 percent of adolescents attempt suicide today. And completed suicides have increased by 300 percent over the last 30 years. (Girls make more attempts at suicide, but boys complete suicide four to five times as often as girls.) It is also known that 60-80 percent of suicide victims have a depressive disorder. A 1998 study showed, however, that only 7 percent of suicide victims are receiving mental health care at the time of their death.
