Answers to a complex, painful condition Reflex Sympathetic Dystrophy Background: In 1864, a condition of hot, burning pain, known as "causalgia", was first described by soldiers of the Civil War who had suffered gunshot wounds. (1) ‘Mitchell described a hyperesthetic state in the affected limb and coined the term causalgia from the Greek, Kausos ( heat ) and Algos ( pain ). Since that time, there have been other terms that have been named this disease that made the diagnosis of RSD quite difficult, such as Sudeck's Atrophy, Shoulder-Hand Syndrome, acute atrophy of bone, traumatic vasospasm, or Sympathetically Maintained Pain & Complex Regional Pain Syndrome, which RSD is Type 1. I Complex expresses varied clinical features found in these conditions Regional emphasizes that in the majority of cases it involves a region of the body, usually an extremity, but may occur on another part of the body or spread to different areas of the body Pain is considered essential to the diagnosis of CRPS types I and II and includes pain that is spontaneous or evoked such as allodynia or hyperalgesia. In rare cases otherwise resembling CRPS, pain may be minimal or absent. Although motor symptoms or signs are not directly included in the classification, tremor, dystonia, and weakness are found in many patients with CRPS. It is also recognized that some patients may not have all of the criteria that will clearly classify them as having CRPS type I or II."     Conservative Management of Causalgia
    (Reflex Sympathetic Dystrophy) Causalgia may be described as an uncommon pain syndrome characterized by posttraumatic pain most commonly affecting the upper or lower extremities. Clinically, the presenting pain is usually paroxysmally produced by local friction, or even air currents, which affect the nerve supply innervating that given region where the pain syndrome is symptomatically manifested. This pain phenomenon varies in its clinical presentation from immediately post-injury to several weeks later. The character of the injury may be as trivial as a sprain but any type of trauma may be etiological. Although the etiology is idiopathic, it is most often due to complete, or incomplete, laceration of the nerve, or plexus, involved. The cardinal symptom is severe burning pain of paroxysmal character. There may be an intolerance to dryness in the area of pain and possible cutaneous coolness, color changes involving redness, or cyanosis, with glossiness, and edema. The pain is commonly exquisitely hyperalgesic. The symptoms may not present for several weeks following the etiological event, but gradually cutaneous symptoms indicative of vasodilation appear. Later, the skin may become cold, cyanotic, pale, diaphoretic, thin, and glossy with brittle and ridged nails. These cutaneous arteriovenous characteristics usually exist even in the presence of blood flow studies which show equality with the uninvolved extremity. These findings suggest that such nerve damage may affect surface vasodilatation without influencing larger deep vessels. Reduction, or complete elimination, of the pain by sympathetic block is diagnostic.

    Three cases of reflex sympathetic dystrophy (causalgia) were associated with cervical spondylosis. Two of them were related to sudden cervical trauma. Patients showed burning pain, hyperalgesia in a segmental distribution, and edema of the arms, hands, and fingers bilaterally. Oral administration of guanethidine sulfate was effective in all cases. Our results support the hypothesis that hypersensitivity to norepinephrine plays a role in reflex sympathetic dystrophy.
