What are cataracts?
One of the most common conditions of the eye are cataracts. Learn the reasons for its occurance and how it is treated alongwith the recent developments in the field.
Cataract: what is it? The word cataract literally means ‘curtain of water’ or ‘water fall.’ It is an almost universal occurrence in the elderly, produced because of the absorption of water, which causes the fibers of the natural, transparent lens to swell and become opaque. In the early stages, the swelling of lens fibers can lead to decreased vision, glare, colored haloes around light bulbs, and diminished sense of contrast. Driving at night becomes especially difficult because of scattering of light by the swollen and partially opaque lens fibers. In some cases, unequal cataract progression in different parts of the lens results in a change in the type or power of spectacles that the patient has been using. For instance, a person who has been using reading glasses may find that he no longer needs them and may feel that his vision is improving. This ‘second sight’ is actually the result of faster progression of cataract in the center of the lens than in the periphery and gradually, after the initial stages, will produce decreased vision at all distances and necessitate cataract surgery. Besides the above-described ‘senile cataract,’ cataract may also occur in young people because of ocular trauma, local eye disease or systemic diseases like diabetes. The main symptom produced is generally only gradually progressive painless diminution of vision, but in some cases as in those because of trauma or when the cataract has far progressed, pain, glaucoma, inflammation, and even total irreversible loss of sight may be encountered.

What is a cataract? When natural crystalline lens of the eye becomes cloudy, it is known as cataract. Symptoms of cataract: Feeling of a film covering the eye.

Is operation the only remedy? Yes, it is. Unfortunately, once the lens of the eye has become cloudy, there is no medical treatment or nutritional method available till date for clearing the cataract. It is now not necessary to wait till the cataract ripens to thick white opacity; it can be removed when it starts to interfere with normal daily activities such as reading the newspaper.

Treatment
Once a clouded lens develops, surgical removal is the only remedy. Each year about 1.5 million cataract operations are performed, making it the most common operation in the country in people over 65. Cataract surgery may be the oldest procedure in the world having been introduced to Europe from India by Alexander the Great's army. Naturally, cataract surgery has improved greatly since then. In the past, cataract surgery was not performed until the cataract had become well developed. Newer techniques, however, have made it safer and even more efficient to operate in earlier stages. In fact, modern cataract techniques not only remove cataracts but are also becoming important procedures for correcting astigmatism. Cataract surgery improves vision in up to 95% of cases and saves millions of Americans from blindness. Nevertheless, considerable evidence suggests that, because of the ease and relative safety of the procedure, it may be performed more often than needed. Patients having operations now tend to have better preoperative vision than those operated on ten or 20 years ago. In a study of 800 cataract operations, a quarter of the patients said that clouding had had no obvious effect on their lives before the procedure.

THE HUMAN EYE The eye has several structures through which light passes before it is processed by the brain into images on the retina, thus producing sight: - Light rays first enter the eye through a transparent dome at the front of the eye called the CORNEA. - Light then passes through a watery fluid in the ANTERIOR CHAMBER, and then through the PUPIL, which is the round, black opening in the IRIS, the colored part of the eye. The pupil allows the proper amount of light to enter the inside of the eye. - Light then passes through the LENS, which is located behind the pupil. The lens is a very important part of the eye for focusing light. In the normal eye, the lens is a flexible bag of protein which, in young people, is crystal clear. The lens has the ability either to flatten or to thicken, thereby bringing objects into focus. - After the lens has focused the light rays, the rays pass to the back of the eye through a clear, jellylike substance called the VITREOUS BODY. - Finally, the light rays reach the RETINA, which contains many sensitive nerve cells that react to light images projected onto them.

CATARACTS What Are Cataracts?
Although there are several different types of cataracts, they are all caused by a loss of transparency in the crystalline lenses of the eye. This loss causes a disruption in the transmission of light to the retina. Who is affected? Cataracts most commonly occur among people who are over 55, although it can affect younger people and, although very rare, has even been known to affect newborns. Twenty-three percent of those over 75 are affected by cataracts. On a Global basis, cataracts account for over one-half of all cases of impaired vision and affect over 20 million people. What causes the development of Cataracts? Chemical changes within the protein material of the lens are responsible for clouding and yellowish or brownish discoloration. These chemical changes seem to be part of the natural aging process, although they may also be caused by injury, inherited tendencies, certain diseases, or birth defects. Other contributing factors include exposure of the eye to ultraviolet light (sunlight), poor nutrition, cigarette smoking and certain drugs.

What causes childhood cataracts?
In the UK, no identifiable cause is found in more than one third of infants born with cataracts in both eyes, and in over 90% of children with only one affected eye. Approximately a quarter of children, usually those born with cataracts in both eyes, have other associated problems. For example, Rubella infection (German measles) can cause multiple disabilities, one of which is cataracts (in one or both eyes) resulting from infection early in pregnancy. These may be present at birth or develop in the first year of life. Rarely, other infections during pregnancy may cause cataracts in the newborn child. A number of other rare general conditions (syndromes) can also result in cataracts. Some of these can be checked for through blood tests. As some of these conditions are inherited many children with cataract, together with their families, are seen in hospital by a clinical geneticist, who can help with diagnosis and provide advice about the risk of cataracts in future children.

What is a cataract?
A cataract is a clouding of the lens in the eye that affects vision . Most cataracts are related to aging. Cataracts are very common in older people. By age 80, more than half of all Americans either have a cataract or have had cataract surgery.
A cataract can occur in either or both eyes. It cannot spread from one eye to the other.
Cataract In a normal eye, light passes through the transparent lens to the retina. Once it reaches the retina, light is changed into nerve signals that are sent to the brain.
The lens must be clear for the retina to receive a sharp image. If the lens is cloudy from a cataract, the image you see will be blurred.

These changes may involve both the nucleus and the cortex of the lens (#22006, #22008). Nuclear changes are characterized by progressive crosslinking and insolubilty of crystalline proteins. This leads to a progressive hardening of the nucleus and discoloration which is initially yellow and can eventually become a dark brown or brunescent color(#22009, #22262). Cortical changes may begin as small peripheral water clefts with globular degenerative changes of the cortex. This may eventually coalesce into dense bands of opaque cortical material. In addition, lens epithelial cells may migrate posteriorly and form swollen bladder cells(#22011, #22188). Also, anterior lens epithelial cells may undergo a fibrous metaplasia leading to a thick fibrous plaque between the anterior lens capsule and the anterior epithelial cells (#22012). WHAT IS A CATARACT?
WHAT SHOULD YOU DO IF A CATARACT IS SUSPECTED?
The first thing to do if your veterinarian indicates your pet has a cataract is to have your pet examined by a veterinary ophthalmologist. The lens is an important link of the total visual system, yet the health of the entire eye should be evaluated before the lens develops a complete cataract. Early evaluation of the eye with a cataract sometimes permits examination of the retina. If the cataract is complete and 'mature', the retina cannot be directly examined and an ultrasound or an electroretinogram examinations, or both may be needed to assess the health of the retina. At the time of the initial examination, the cataract may sometimes be identified as to cause, area of involvement and stage of progression. Not all cataracts lead to blindness. "Incomplete" cataracts may not impair vision significantly. If your pet has a cataract and has shown some visual loss, evaluation will include the consideration of and benefit of cataract surgery.
