While they are the most common treatable cause of vision loss in older adults, cataracts sometimes are unrecognized or are considered just another normal consequence of growing older. In this article, Drs Trudo and Stark explain the causes, symptoms, and differential diagnosis of cataracts and discuss treatment options that can improve independence and quality of life for many patients. M ore than 3,000 years ago, cataract surgery was performed by using a technique called couching. An assistant held the patient's head steady while the surgeon inserted a needle into the eye and pushed the clouded lens out of the visual axis, allowing it to rest, or "couch," in the back of the eye (1,2). The operation resulted in a clearer but unfocused view of the world. Nonetheless, a few patients regained the ability to perform the daily tasks that the cataract had precluded. While surgical techniques and skills have improved dramatically since those early times, the surgeon's goal of improving vision and quality of life still holds true today.

A painless cloudiness that gradually covers or extends throughout the lens of the eye, a cataract blurs vision and gives images a brownish tint. The cloudiness is severe enough in some people that normal daily activities, such as driving and reading, become difficult or impossible. What are cataracts?
Normally, the lens focuses light on the retina to produce clear vision. The cloudiness of a cataract keeps light rays from passing through to the retina and thus blurs vision. Cataracts can cause double vision and glare from lights and can decrease the ability to see colors and to drive at night. One or both eyes may have a cataract. Cataracts often occur as a normal result of aging. They can also be caused by certain diseases (for example, diabetes), injury to the eye, overexposure to ultraviolet light from the sun, prolonged use of certain drugs (for example, steroids), smoking, or alcohol abuse. Cataracts seem to occur more frequently in some families. How are cataracts diagnosed?

In your eye you have a lens, which is normally clear like a window that has just been cleaned. If that lens gets cloudy, it's called a cataract (say: kah -tuh-rakt). Like a dirty window, a cloudy lens makes it difficult to see well. The person can still see, but it's blurry. The lens in your eye is made of protein and water. But as people get older, sometimes proteins in the lens stick together and cause cloudiness, or cataracts. At first, a person who has a cataract may not notice any difference in the way he or she sees. But over time, the person may find it harder to read and do other normal tasks. In fact, "cataract" means waterfall in Latin. Why? Because, if you have one, it can be like seeing through a waterfall.

What are cataracts? A cataract is any opacity or loss of transparency of the lens of the eye. The opacity may be confined to a small area of the lens or capsule, or it may affect the whole structure. A complete cataract affecting both eyes will result in blindness, whereas small non-progressive cataracts will not interfere with vision. What is a Cataract? Mature Cataract Immature Cataract Like a camera, eyes have a clear lens inside them that is used for focusing. A cataract is any opacity within a lens. The opacity can be very small (incipient cataract) and not interfere with vision. It can involve more of the lens (immature cataract) and cause blurred vision. Eventually, the entire lens can become cloudy, and all functional vision lost. This is called a mature cataract.

What is not a cataract? Normal geriatric dog eye,
with normal Nuclear Sclerosis All geriatric dogs develop a hardening of the lens (Nuclear Sclerosis) that causes the lens to have a greyish appearance. This does not usually interfere with vision. The photo to the right shows a normal geriatric dog eye, effected by normal Nuclear Sclerosis.

Here they are...Sam, Maggie and Casey.
Here's Sam, our 6 year old Yorkshire Terrier. He's abaout 4 pounds and sweet as can be.
Here's Maggie, she passed away last year from a stroke and is missed very much.
She was totally blind when we rescued her but had no problem getting around, even using the doggie door into the back yard.
I hope I do as well when I'm 80 yrs old!!!

They got along so well!
And here's our newest member of the family, Casey.
Casey is a Powderpuff Chinese Crested and we rescued him from the Canton, OH area.
He's very playful, loves his toys, and he and Sam get along great. Casey only has one eye from tangling with a car or a bigger dog. Either way, he lost the fight. But it doesn't affect his life, he's a very happy dog.

Cataracts
Cataract is the term given to the misting or hazing of the lens in your eye. Cataracts lead to poor vision and blindness, but in most cases can be easily remedied by a fairly minor operation.
Symptoms
When you develop cataracts, the vision slowly becomes more misty. You may see haloes around lights, and may find vision worse in bright sunlight. You may be dazzled more easily by bright lights. Your colour vision may not be as good as it was. It is not possible to correct the problems caused by cataracts by just getting new spectacles. In fact, the first you know about it may well be when the optician tells you that you have cataracts when you go for a routine eye test.
Causes
The majority of cataracts are caused by ageing of the eye. It is said that some degree of cataract will happen to all of us if we live long enough. Typically, though, this may be noticed from 50-60 years and onwards. Certain other conditions of the eye that cause inflammation (for example uveitis) and eye injury might lead to cataracts occurring earlier in life. Other conditions affecting your system as a whole eg diabetes are linked with a higher risk of developing cataracts. Certain drugs, including steroids, may make you more likely to develop cataracts early.
