Description of Unit Investigations
Nationwide, bloodstream infections with Candida species have increased by 219-487% between 1980-1989, and these organisms now account for 10% of all nosocomial bloodstream isolates. This incidence equals that of Escherichia coli and surpasses Klebsiella species. In addition to hematogenously disseminated candidal infections, mucocutaneous candidal infections are becoming increasingly problematic, especially in patients with the acquired immunodeficiency syndrome (AIDS); 80% of these patients have candidal infections. Adherence to epithelial cells is the first step in colonization by Candida and subsequent establishment of mucocutaneous infection. Similarly, adherence to intravascular structures is considered to be a critical step in the egress of the blood-borne fungi from the intravascular compartment, as they hematogenously infect target organs. Because the mortality rates associated with candidal infections remain disturbingly high despite the presence of antifungal agents with excellent in vitro activity against Candida , optimal therapy requires strategies to increase host resistance to candidal infection (such as blocking adherence), combined with the use of antifungal agents. After more than two decades of study, a single integrated understanding of the adhesive process in candidiasis has not evolved. One of our primary objectives in forming a

Candidiasis
Clinical Features Oropharyngeal infection (OPC): white mucosal patches. Vulvovaginal infection (VVC): pruritus, vulval erythema, with or without discharge. Systemic infection usually presents as fever and chills unresponsive to antibacterial therapy. May manifest as renal or hepatosplenic infection, meningitis, endophthalmitis, endocarditis, osteomyelitis and/or arthritis. Etiologic Agent Candida albicans and C. glabrata . Less commonly, C.tropicalis C. parapsilosis , and C. krusei . Rarely, other Candida species. Reservoir Forms part of the normal microbial flora of the mouth and gastrointestinal tract. Incidence Fourth most common cause of nosocomial bloodstream infections. Incidence is 8 cases per 100,000 in the general population. Higher incidence among neonates and African-Americans. OPC used to be a common opportunistic infection in HIV-infected persons (prior to the introduction of highly active antiretroviral therapy [HAART]). Sequelae None with appropriate antifungal therapy. Mortality rate is almost 50% with bloodstream and disseminated infection.

flucytosine (Ancobon, 5FC) Drug description
Flucytosine is used to treat serious fungal infections caused by Candida or Cryptococcus. Cryptococcus usually infects the membranes that cover the brain and spinal cord, causing cryptococcal meningitis. Cryptococcal meningitis is usally treated with intravenous amphotericin B . Sometimes a doctor will use a combination of amphotericin B and flucytosine to try and bring the cryptococcal infection under control more quickly. However, recent studies show that this combination may be no better than amphotericin B alone due to the side effects of flucytosine. Other new studies have shown that the combination of flucytosine and another antifungal drug, fluconazole , may be just as effective as intravenous amphotericin B at treating cryptococcal meningitis. Flucytosine and fluconazole are both pills. Side effects
Bone marrow toxicity can cause anemia, leukopenia, pancytopenia, thrombocytopenia and, rarely, agranulocytosis and eosinophilia. Other side effects can be nausea, vomiting, anorexia, bloating, diarrhea, and, rarely, bowel perforation. Reversible elevations in liver enzymes have been reported. Occasionally, rashes may be a side effect. Some side effects that have only been seen very rarely are confusion, hallucinations, headaches, sedation, vertigo and liver enlargement.There has also been a case of severe allergic-type reaction to the drug in at least one person with AIDS. This type of reaction is called anaphylaxis. Dosage
The usual dosage of flucytosine for adults and children is 50-150mg/kg daily, administered in equally divided doses at 6 hour intervals.

Women Alive Candida Esophagitis by Lisa Mark PA-C Winter '95 An infection frequently underestimated in HIV infected women, is Candida esophagitis, (a yeast infection in the esophagus). It is closely related to thrush (a yeast infection in the throat) and Candida vaginitis (a yeast infection in the vagina) and the only difference is it's location in the esophagusthe tube that connects the mouth with the stomach.
Background
In one study, patients at risk for AIDS were found to have Candida esophagitis as the only initial opportunistic infection. Another study demonstrated that Candida esophagitis was the most common AIDS-defining diagnosis in those women, occurring more frequently than PCP. Yet, another study showed that women were more likely than men to have Candida esophagitis (19% vs. 9.7% respectively).
Common symptoms of overgrowth and dissemination of Candida in the esophagus are: painful swallowing, pain behind the breast bone, a sensation of food sticking in the chest, and nausea. The majority of women with esophageal candidias also have oral thrush. Yet, a study of 110 people with esophageal symptoms showed that the presence of oral thrush was not a good indication of underlying esophagitis. While 77% of patients with oral thrush had Candida esophagitis, 40% of patients with Candida esophagitis had no thrush, and 27% had viral (CMV or herpes) esophagitis. The results show that the cause of esophagitis cannot be predicted by symptoms alone, or the presence of thrush.

Thrush
Thrush is the widely used term for a common fungal infection caused by Candida albicans. Usually this takes the form of trivial vaginal or mouth infections, although it can affect the body more widely and seriously on rare occasions.
Symptoms
Vaginal Thrush
This causes itch and a cheesy discharge. The area is often redder than usual. It is often painful too, especially on intercourse, and may be associated with frequency of passing water and burning or pain on passing water. It may, therefore be confused with bladder infections ( cystitis ). Thrush, however, is usually associated with itching, which cystitis is not.
Oral Thrush
This may cause a sore mouth and throat, and as well as redness, it is characterised by white discharge on the surface. Babies, especially those being bottle fed, may suffer from this, as well as adults.
Nappy rash
One cause of a persistent nappy/diaper rash is infection of the area with thrush.
Causes
The cause is a fungus which is found widely in nature (Candida albicans), and indeed, is frequently present in the mouth and motions of people in good health. It seems that it is at periods when the defence mechanisms are down that it actually causes a problem. This can be when physically or mentally "run down", in women either pregnant or on the oral contraceptive pill, in diabetes and people either on steroids, or whose immune systems are suppressed.
